Orientation Workshop for Trainers of District Hospital Training January16-17, two days orientation workshop for trainers of District Hospital Training Program, was coducted by technical support of the Project, at Training Center of Malalai Maternity Hospital (MMH). Conversion of Polyclinics to District Hospitals, modules and lessons, appointing lecturers for each lesson, appointing clinical tutors and explanation of hand book and logbook were explained by Chief and Technical Assistants of MMH Training Department. District Hospital Training Program will be held in May 2007. Lectures in Isteqlal Hospital JICA RH PROJECT NEWSLETTER Quarterly ISSUE NO:3 Happy New Year MARCH , 2007 December 16 ~ 26, a Medical lecture course was held in Isteqlal Hospital for doctors and Nurses/midwives of all hospitals in Kabul City. It was the first time that such lecture courses had been held in Isteqlal hospital. The course was coordinated by the members of 6 Hospital meeting. Lecturers from Indra Gandhi, Ataturk, MMH and Isteqlal presented the lectures such as; important and advanced knowledge of Newborn management and OBGY. 196 participants were attended to this lecture course. The participants were satisfied with the contents of the course and expected conducting such lectures in the future. Third CoC Training December 2~ 27, 3 rd CoC training program was conducted for female doctors and midwives of polyclinics in Kabul City, at Training Center of MMH by technical and financial support of the Project. The program was facilitated by Training Department of MMH and observed by KPHD. 16 trainees were attended to this training program and learned theoretical and practical lessons. Continuum of care; from ANC, Delivery, PPC+ Newborn care and making decision of referral cases from community to hospital were the main contents of this CoC program which were taught by MMH, TDH, MoPH and Mir Wais Maidan CHC trainers. Participants received certificates and thank you letters were distributed to trainers. Training Department of MMH and the Trainers were satisfied with the result of the program and insisted for the continues of such Activities & News Referral Sheet workshop January 11, after three months pilot utilization of standard referral sheets, second workshop for pilot standard referral sheet was conducted to share feedbacks and to make next plan for the effective system of referral network. Doctors, midwives and nurses from different hospitals, polyclinics, CHCs and private clinics were attended to this workshop. Feed back of 6 hospital, polyclinics and CHCs were reported to this workshop. Pilot utilization of standard referral sheet was extended for further three months and focal point will be established in all facilities to improve referral system. General Directorate of Curative and Diagnostic and Kabul PHD will receive the referral sheet utilization report of polyclinics and CHCs. Happy New Year

Kabul Province is one the Provinces with more problems comparing other provinces. Considering the existing problems and increscent of population, the function is divided in urban and rural. Dr. Saheb Noor Tamana who was one of the advisors of General Directorate of Provincial Public Health, is assigned as Urban Health Deputy Director for KPHD.

Urban Health Concept in Kabul City

Urban Health is a new concept in Afghanistan which aims to improve health services, ensure access of people to the health services, to improve referral system and

to reduce the burden of the Tertiary Hospitals. The contents of the concepts are: 1)Referral linkage in a structure from Tertiary Hospital to community, 2) Referral standard in Urban Kabul (Referral Zone), 3) Referral mechanism according referral zone in Kabul City and, 4) Strengthen and rearrangement of health facilities with feasible implementation plan; such as: a) Conversion of Polyclinics to District Hospitals (DH)and CHC, b) reconsideration of standards of health facilities in Urban Kabul City, c) rearrangement of health facilities in Urban Kabul City and, d) project of ensuring the program by establishing implementation structure for Urban Kabul City.

Kabul Provincial Health Directorate (KPHD) is responsible for the logistic management of the urban health facilities and Directorate of Tertiary Hospitals will be responsible for technical issues of the health facilities. It should be mentioned that, there are eight polyclinics in Urban Kabul City which are neither under BPHS and nor EPHS. They will be gradually converted to DH and CHC. Two of the Polyclinics (Rahman Mina and Chardehe) upgraded to DH (urban DH). The staffs of DH are recruited by PRR process. It should be mentioned that Urban DH are adjusted based on BPHS.

Urban Health Deputy Director for KPHD

Supportive Supervision

As apart of national RH program , supportive supervision has been started by Reproductive Health Directorate in Jalalabad, Parwan, Kapisa, and Laghman provinces.

The purpose of the supportive supervision was to: a) observe Reproductive Health services in the province and, b) to support RH officers after Reproductive Health Officers (RHO) workshop .

During the visit, RH Directorate of MoPH discussed with Provincial Health Directorate and RH officers to visit the facilities with RH Officers and find the gaps of reproductive health services in the province.

5th Nursing Management Workshop

March 4-6, to increase the management skills of Nurses and Midwives and to prepare in-services trainings in the hospitals of Kabul City and other provinces, 5th Nursing Management Workshop was held at MoPH by Nursing Directorate of MoPH and technical and financial support of the Project.

20 nursing directors of province hospitals learned the in-service trainings methods, medical ethics ,solution of nurses and midwives problems and empowerment of nurses and midwives, through this three days workshop.

The participants visit the nursing and midwifery in-service training program at Rabia Bulkhi Hospital in the second day of the workshop.

All the participants satisfied with the concepts of the workshop and requested the continues of such workshops for all the nurses and midwives

Supportive Supervision in Urban Kabul Health Facilities

After holding CoC training programs, supportive supervision has been started by SRS team members (Reproductive Health Directorate, RH Officer of Kabul PHD, training Department of Malalai maternity Hospital and the Project) in Urban Kabul City CHCs which are not supporting by NGOs.

The purpose of the supportive supervision in Urban Kabul is: 1) to observe the Trainees' activities that are trained in CoC trainings, 2) to supervise the standards and protocols which increased by MoPH, 3) to find the main gaps, and 4) to fill the gaps.

In general, trainees’ performance improved after the training programs.

Lack of instruments and medicine, lack of support from government side, shortage of staff and need of capacity building were found as core problems of the CHCs.

According to the decision during review meetings of supportive supervision; some Antibiotics was provided by MoPH (Kabul PHD), the Project provides water tank and IEC materials and RH Directorate is responsible to provide guidelines and standards for the CHCs.

SRS members have communications with these facilities regularly to decrease their problems.
