1.0 INTRODUCTION

1.1 This Quality Assurance Manual was compiled for the purpose of achieving a controlled weld testing program with a determined effort to maintain a high standard of quality by regulating and systematically controlling the testing process, from receipt of material through completion of the non destructive and/or destructive test with all the required documented support history.

1.2 The Facility Representative is responsible for the enforcement of the policies

outlined in this QA Manual. He or she and the Test Supervisor overseeing the examination of welds have the complete support of the local Joint Apprenticeship and Training Committee (JATC) or Trade Improvement Committee (TIC) to make required decisions independently.

1.3 IW/AWS accredited test facilities must process and enter all applicable welder

performance qualification tests (WPQR’s) performed, directly into the on-line IW/AWS Welder Certification program for processing. Issuing welder performance qualifications using the program’s pre-qualified procedures, logo, forms, etc. without doing so is prohibited.

1.4 The Test Supervisor will have complete authority in the administration of the welder

qualification tests.

1.5 Non conformances to the requirements in this QA manual will be referred in writing to the Facility Representative for preventive or disciplinary action.

1.6 The JAC (Joint Apprenticeship Committee) and/or JATC (Joint Apprenticeship and

Training Committee) or TIC (Trade Improvement Committee) are committees formed with equal representatives from labor and management. There primary purpose is to oversee the apprenticeship training and journeyman upgrading program and funds.

3.0 STATEMENT OF POLICY

3.1 It shall be the policy that while the facility is operating as an AWS accredited welding test laboratory, it will adhere to all of the requirements in the AWS QC 4-89 Standard for Accreditation of Test Facilities for AWS Certified Welder Program and the QC 7-93 Standard for AWS Certified Welders and all of its supplements.

3.2 It is the policy of this Training Facility/Test Site to certify individuals only after they

have demonstrated the necessary proficiency to perform welding operations in the particular process. This will be evidenced by a minimal practice session prior to issuing the welder the necessary paperwork and test coupons.

3.3 The Facility Representative and the Test Supervisor have the responsibility of

maintaining the control necessary to meet the requirements of this QA Manual and all pertinent specifications outlined in the applicable codes and standards listed below:

3.3.1 Each ATF shall maintain current copies of all applicable codes and standards

pertinent to the qualification tests being performed at their facility. The underlined standards and codes below are mandatory at each facility.

4.0 APPOINTMENT AND RESPONSIBILITIES OF QUALITY ASSURANCE MANAGER

4.1 NATIONAL PROGRAM PERSONNEL

4.1.1 Quality Assurance Manager

The QA Manager heads the Quality Assurance Team and is responsible for setting goals and implementing the requirements of the Ironworker Welding Certification Program of North America. The program complies with the requirements of AWS QC4 Standard for Accreditation of Test Facilities for AWS Certified Welder Program and AWS QC7 Standard for AWS Certified Welders. The position maintains current qualification as an AWS/IW Assessor and as an AWS CWI. The position reports to the Ironworkers Welding Certification Test Committee (Labor and Management) of the Ironworker/AWS Welding Certification Program of North America.

Duties include, but are not limited to: Oversight of the administrative implementation of the program through the

national office in Washington, D.C., including the documents and records control system. The position is also responsible for organizing management reviews of the quality assurance system.

Oversight of the training and certification program for IW Assessors.

Design and implementation of the Assessment schedule for JAC/JATC/TIC

ATFs, including assignment of IW assessors, and reviewing their audit plans and reports.

Design and implementation of a corrective action system.

Obtaining new WPSs and PQRs as needed for the National Program.

The position is responsible for maintaining and updating the QA Manual and

related procedures with input from other Assessors, the AWS, and IW JAC/JATC/TIC ATF's. A tracking system for changes will be maintained.

4.1.2. IW/AWS Assessor, National Program

The IW/AWS Assessor is responsible for the assessment of JATC/TIC ATF’s under the Ironworker Welding Certification Program of North America as directed by the QA Manager. The Assessor is a member of the Quality Assurance Team and contributes to the maintenance and improvement of the Quality Assurance Manual. The position maintains current qualification as an IW/AWS Assessor and as an AWS CWI.

4.2 RESPONSIBILITIES and REQUIREMENTS OF IW/AWS ATF PERSONNEL – Adequate documentation shall be maintained for verification purposes.

4.2.1 Facility Representative (Technical Manager)

4.2.1.1 The Facility Representative should be a current CWI and oversee

the daily operation of all training at the facility on behalf of the Joint Apprenticeship Committee (JAC/JATC/TIC). The individual shall have at least five years of technical experience in inspection or testing of metals or welding.

4.2.1.2 The Facility Representative is responsible for the overall Quality

Assurance Program.

4.2.2 Quality Assurance Manager

4.2.2.1 The quality assurance manager must be a current CWI and is responsible for the maintenance of all files, tools, equipment, and maintenance procedures. The QA manager shall maintain current status as a qualified welder through the IW/AWS program.

4.2.3 Test Supervisor

4.2.3.1 The Test Supervisor(s) shall be a current CWI and be responsible for the execution of the QA program at the testing level and shall report to the Facility Representative. The test supervisor shall maintain current status as a qualified welder through the IW/AWS program.

4.2.4 Welding Instructor

4.2.4.1 The Welding Instructor is responsible for classroom and hands on

welding instruction. They are also responsible for the welding shop and welding operations when the shop is used for instructional purposes. The welding instructor shall maintain current status as a qualified welder through the IW/AWS program.

4.2.5 NDE Personnel

4.2.5.1 NDE Personnel, whether in house or a contract agency, NDE

personnel, must be familiar with all of the requirements of the Ironworker/AWS Certification Program of North America and must be qualified to perform non-destructive examination of welds.

4.2.6 QA Program Reviewer 4.2.6.1 Shall be a current JAC/JATC/TIC member. He/she shall attend

and take an active part in the Yearly Self-Assessments.

5.0 ADMINISTRATION OF QUALITY ASSURANCE MANUAL

5.1 The National level QA Manual outlines the policy, responsibilities, and implementation strategy for the Ironworker/AWS Welding Certification Program of North America.

5.2 The National Quality Assurance Manager is responsible for maintaining and

updating the National QA Manual and related procedures with input from the IW/AWS ATF assessors, the AWS, and the IW ATF's themselves. A tracking system for changes will be maintained.

5.3 The National Quality Assurance Manager and the IW/AWS ATF Auditing Team

will meet at least once annually to review the QA manual and auditing procedures.

5.4 Revisions to the QA manual will be numbered, dated, identified, and recorded.

5.5 The Facility Representative is responsible for maintaining the Quality Assurance Manual in current status and for implementing its provisions.

5.6 The Facility Representative is responsible for ensuring that all provisions of

the program and this Quality Assurance (QA) manual are followed without exception. Failure to do so may result in suspension of ATF accreditation.

6.0 GENERAL INFORMATION

6.1 The Facility Representative and the Test Supervisor will ensure that the test area is clean and generally free of hazards. They will further insure that all safety requirements in accordance with ANSI/ASC Z.49.1 are met.

6.2 All welding and testing operations will be conducted in accordance to applicable

sections of the appropriate code/specifications and test procedures used for performance qualification.

6.3 Subcontracting of NDE services will be allowed. Only qualified contractors will be

used. A file with the contractor qualifications will be maintained and reviewed as necessary. All NDE personnel shall be qualified to ASNT SNT-TC-1A recommended practice.

6.4 Testing may be performed off site under the supervision of a facility test supervisor.

All test materials, consumables, and calibration equipment will be supplied by the IW/AWS ATF. The completed test assembly shall be returned to the ATF where the test specimens will be removed, bent, inspected, and results recorded by the QA Manager/Test Supervisor.

6.5 All technical complaints, non-conformances, comments, and suggestions will be

referred to the Facility Representative. The Facility Representative will respond to and forward a copy of the response to the JAC/JATC/TIC. The JAC/JATC/TIC will make final determination on what action to take. A report of all issues will be made and filed at the Accredited Test Facility of the original complaint and with the National Quality Assurance Manager. The Facility Representative shall develop and maintain a file for all correspondence regarding these issues.

7.0 PROCUREMENTS, CONTROL AND STORAGE

7.1 The Facility Representative and/or Quality Assurance Manager is responsible for inspection and acceptance of delivered material.

7.2 All materials procured for testing purposes shall be traceable and/or identified to,

the original Material Test Report and/or the filler metal Manufacturer Report.

7.2.1 The Material Test Report and/or the filler metal Manufacturer Report and the corresponding stock will be assigned a specific designation.

7.2.2 The Material Test Report and/or the filler metal Manufacturer Report will

be attached to the purchase slip identifying the seller, date material was purchased, and amount of material purchased.

7.3 It is mandatory that the designation be recorded on the Material Mill Test Report

and/or the filler metal Manufacturer Report and corresponding stock immediately upon receipt of material.

7.4 Coupon lengths cut from stock shall be individually identified by the appropriate

stock designation.

7.5 Only waterproof steel markers, paint, or stamps shall be used.

7.6 All test materials should be stored on racks if possible and shall be kept in a secure area which is relatively moisture free.

7.7 All filler metals and fluxes shall be stored in a secure area and in accordance

with the applicable codes.

8.0 FABRICATION CONTROLS

8.1 Fabrication of the test plates and test joints shall be done under the direction and supervision of a Test Supervisor to the requirements of the appropriate code and test procedures.

8.2 Prior to fit up - the Test Supervisor shall:

8.2.1 Make available necessary test facility worksheets, fit up instructions, safety

rules, and verify that approved testing materials are used.

8.2.2 Visually check condition of base material for defects such as laminations, cracks, tears, or any other surface irregularities.

8.2.3 Check edge preparation for beveling, cleaning, and acceptable dimensions.

8.3 After fit-up and prior to welding the test supervisor shall:

8.3.1 Check joint fit-up, alignment and tolerances as specified in the appropriate

code and test procedures.

8.3.2 Check the cleanliness of weld joint and surrounding area.

8.3.3 Stamp weld test coupons in the exact location that test specimens are to be removed with the assigned welder test certification numbers and weld position.

8.3.4 A suggested method for assigning a welder test certification number is

to use the year followed by a sequential test number. The position shall also be shown. (Ex: 11-001-3G). A welder qualification test log should be kept identifying the welder and the welder test certification number.

8.3.5 Check test coupon(s) for proper positioning in accordance with the applicable

code.

9.0 WELDING QUALITY CONTROL

9.1 Prior to welding of test coupons the Test Supervisor shall:

9.1.1 Verify that approved electrodes, filler metals, and fluxes are being used.

9.1.2 Check minimum preheat temperatures as specified in the appropriate code and test procedures if necessary.

9.1.3 Check for proper voltage and amperage setting in accordance with welding

procedure specifications.

9.2 During welding of test coupon the Test Supervisor shall:

9.2.1 Check fit up and root pass for fusion to base metal and for discontinuities.

9.2.2 Check opposite side root preparation (back gouging, grinding, or chipping to clean metal operations) if needed.

9.2.3 Re-verify proper positioning (see 8.3.4).

9.2.4 Randomly inspect proper cleaning between passes.

9.2.5 Verify interpass temperatures in accordance with the appropriate code

and test procedures if necessary.

9.3 After welding is complete the Test Supervisor shall:

9.3.1 Verify acceptance of welded coupon by visual inspection.

9.3.2 Verify Stress Relief Heat Treatment, if needed, in accordance with the appropriate code and test procedures.

9.3.3 Place welded test coupon in a controlled area to cool prior to cutting or

testing.

10.0 DESTRUCTIVE AND NON-DESTRUCTIVE TESTING

10.1 All test coupons shall be subjected to Destructive Testing unless otherwise required.

10.2 Prior to cutting test specimens from the coupon, the Test Supervisor shall:

10.2.1 Verify proper location and stamping of weld test certification number and position on the coupon(s). On limited thickness test specimens all stamps will be placed on the face side of specimen.

10.2.2 Remove test specimens from test coupon by an approved thermal or

mechanical cutting method at locations prescribed in the appropriate code and test procedures.

10.2.3 Prepare test specimens in accordance with the appropriate code.

10.2.4 Re-verify stamp of welder certification test number and position before

testing.

10.3 Testing of specimens

10.3.1 Perform test(s) in accordance with the appropriate code, standard, and test procedure.

10.3.2 Observe test results and record discontinuities on test facility

worksheet and Welder Performance Qualification Test Record (WPQR).

11.0 DOCUMENTATION

11.1 All Nondestructive and Destructive test results will be documented on the Test Facility Worksheet and WPQR (Exhibit "A").

11.2 The Welder Performance Qualification Test Record (WPQR), including welder

photo must be forwarded to the National Office using the online IW/AWS Welding Program database. Only successful WPQR results need be forwarded.

11.3 A copy of the completed Welder Performance Qualification Test Record

(WPQR) (Exhibit "B") will be attached to the Test Facility Worksheet and kept in the welders file at the ATF.

11.4 Persons who fail the welder examination will not be re-tested unless they:

11.4.1 Practice in the process and position(s) failed and demonstrate to the

Welding Instructor proficiency in the process and position(s) to be tested.

11.4.2 An immediate retest may be made consisting of two welds of each type

and position that the welder or welding operator failed. All retest specimens shall meet all of the specified requirements.

11.4.3 Personnel re-testing will be assigned a new welder control number and

new paperwork will be issued. 11.4.4 Re-testing will be performed on all positions failed.

11.5 When a welder’s or welding operator’s qualification period of effectiveness has

lapsed, a requalification test shall be required. Welders have the option of using a test thickness of 3/8 inch (10 mm) to qualify any production welding thickness greater than or equal to 1/8 inch (mm.) per 4.32.1.3 of the AWS D1.1 structural code.

11.5.1 No immediate retest shall be permitted after failure of a requalification

retest. A retest shall be permitted only after further training and practice. 11.6 Copies of all paperwork will be retained on file at the testing facility for a minimum of

five (5) years.

11.7 Certifications shall be renewed annually providing the welder presents proof of having performed welding operations (continuity) in the process they originally qualified (Exhibit "C").

11.8 At the completion of testing and documentation, test specimens shall be stored in a

secure area for a minimum period of six months until properly destroyed.

12.0 CALIBRATION OF TOOLS

12.1 All tools, equipment, and measuring devices used for welder qualification testing will be inspected annually for conformity to original manufacturer specifications and performance standards.

12.2 Tools and equipment not meeting performance standards will be tagged for repair or

disposal and removed from service. (Exhibit "F")

12.3 All calibration equipment will be calibrated to a national standard at least annually, or as needed, and results will be recorded and equipment labeled.

12.4 A log of equipment calibration will be kept on file at training center.

12.5 Welding equipment will be maintained and cleaned as needed.

12.6 Welding equipment used for testing will be checked for proper amperage and

voltage annually or properly calibrated equipment will be used as specified in 12.3 (Exhibit "D”.)

12.7 Preventive maintenance on welding equipment shall be performed regularly

according to usage.

12.8 All preventive maintenance will be recorded in a maintenance log. (Exhibit "E")

13.0 AUDIT OF QUALITY ASSURANCE PERFORMANCE AT EACH IW/AWS ATF

13.1 An initial on site audit by a member of the IW/AWS ATF Assessment Team shall be conducted to evaluate the adequacy of procedures, inspections, testing, process control and record keeping in order to insure that the program complies with the IW/AWS program standards.

13.2 Internal self audits shall be performed by the Facility Representative, Quality Assurance Manager, and attending JAC member annually. These audits shall take place on the second and third year following the onsite audit. The completed internal audit form will then be forwarded to the National Quality Assurance Manager for review and approval, copies of all audits shall be kept on file at the ATF.

13.3 Onsite audits will be performed every three years by a member of the IW/AWS ATF

auditing team. 13.4 All audits shall include:

13.4.1 Examination of all quality control operations and documentation.

13.4.2 Review procedures with the Test Supervisor(s) to determine familiarity

with established requirements.

13.4.3 Review test methods to assure conformance with specified procedures.

13.4.4 Written notification of required corrective action.

13.4.5 Follow up of corrective action within thirty days of written notification. 13.5 Results of the Quality Assurance audit will be documented in detail and maintained

as a permanent in-house record.

13.6 Follow up review of deficient areas shall be made on a more frequent basis until required corrections have been completed.
