Description:PermaFlo® is a light cured, radiopaque, methacrylate-based flowable composite. PermaFlo contains ~1µm average particle size with narrow upper limit particle distribution. PermaFlo is available in 9 shades. It provides superior polishability (particularly for a hybrid) and affords excellent translucency for matching, blending, and depth of cure. It is thixotropic, 68% filled mass to mass, and has ~6µm film thickness.

Indications:PermaFlo can be used for anterior and posterior restorations. It is also used for restoring missing subgingival tooth structure (where light cure is accessible) before endodontic procedures (referred to as the “donut technique”). It is highly recommended for Class III and V restorations, or for the lower layers when using incremental building of composites.

Procedure:1. Remove luer lock cap and attach a disposable Micro 20 gauge tip securely onto syringe. This tip can be used when intimate adaptation is required in narrow angles (e.g. class I, V, and proximal line angles of class II).2. For dentin bonded restorations, etch dentin/enamel 20 seconds using Ultra-Etch. Wash 5 seconds. Blow excess water off (leaving surface visibly wet). Apply Peak Universal Bond to preparation and air thin until milky appearance of filler disappears, maintaining a high gloss surface and no dry spots. Light cure 20 seconds if using a standard halogen light, 10 seconds if using a high energy light. Apply a thin layer of PermaFlo to ensure complete polymerization at pulpal

floor areas.3. For matching posteriors, or in areas of deeper layers, use PermaFlo shades closer to or slightly lighter than dentin. Use Translucent or other appropriate shades to match enamel for last increment. PermaFlo has tremendous chameleon properties. Apply first layer of PermaFlo 0.5-1.5mm and light cure. Follow with 2-4mm increments. Cure each resin layer 20 seconds with a high-quality curing light. Place light guide as close as possible to composite without touching. For sculptability and highest wear, use an appropriate paste composite for your last layer (e.g., Vit-l-escence™ or Amelogen®).4. Shape with appropriate finishing diamonds or multi-fluted carbide burs. Finish with strips, polishing cups, and wheels or disks as appropriate. We recommend Jiffy® Polishing system for shine. An excellent luster may be obtained with Ultradent® Diamond Polish. 5. Use PermaFlo for Class V restorations as shown in Figs. 3-7:a. Etch dentin/enamel 20 seconds using Ultra-Etch. Wash 5 seconds. Maintaining a high gloss surface and no dry spots. Light cure 20 seconds if using a standard halogen light, 10 seconds if using a high energy light.c. Restore using PermaFlo delivered through the Micro 20 Gauge tip. Light cure 20 seconds. 6. Use PermaFlo for the “donut technique” to achieve extension of root when retention of clamp would otherwise be difficult or impossible (Fig. 7). Make sure area is accessible to light from curing tip.a. Isolate tooth with Ultrapak® knitted displacement cord to ensure fluid control before hybrid layer formation.b. Prepare dentin for bonding by etching with Ultra-Etch, then applying Peak Universal Bond. Light cure.c. Place donut-shaped layers of PermaFlo. Cure between each layer. d. If using during root canal treatment, UltraTemp is recommended as temporary cement allowing easy access to complete the root canal. Use of the more opaque shades of PermaFlo to contrast with dentin, aiding in removal of material that extend onto root. e. Donut buildup functions as a matrix for composite core with direct post placement. The inside of the PermaFlo donut is freshened with a diamond bur and treated with PermaFlo® DC Primers to maximize adhesion of composite core. Finish with PermaFlo® DC composite luting/restorative resin.

Precautions:1. Partially cured material will have deficient physical properties, including strength, wear resistance, retention, marginal integrity and color stability. Incompletely cured material could also cause pulpal irritation. Use only high-quality curing units and check units periodically with a light meter.2. If restoration necessitates extensive shaping and sculpting for direct veneers, minimize light exposure from overhead light unit until all material is placed to prevent premature set.3. The oxygen inhibition layer should not be removed or disturbed during incremental layering because it optimizes the bond to each succeeding layer. 4. Placing posterior composite correctly is much more technique-sensitive than for amalgams. Remember to polymerize interproximal areas adequately and to check radiographically at regular checkup intervals.5. Any resin can be sensitizing to skin or can cause allergic reaction if individual is already sensitized. Therefore, wash area of skin exposed to resin completely to remove material. If dermatitis, rash or any other allergic reaction is observed, see a physician.6. Syringe caps should be replaced following use to prevent polymerization (as composite resins are light activated).7. Do not reuse tips to avoid cross-contamination.8. Use Ultradent® Syringe Covers and/or clean and disinfect syringes between patient.
