The 2015 Safety Day theme is related again to “THE BASICS” that reflect the three PDOIGolden Rules (Comply, Intervene & Respect).This year we will focus on Intervene “the
second pillar of the Golden Rules which is fundamental to our lives and our business”.

Objectives for our 2015 Safety Day

I know it’s not easy it takes courage Sometimes you receive push back and someI know it s not easy, it takes courage. Sometimes you receive push‐back, and some colleagues may even say “Its none of your business” but it is, its your right, your responsibility to intervene because we should all care.

What is an intervention?
You and I are responsibleto intervene with each other. Failure to intervene is like

Peer to Peer Intervention
What is different about peer to peer interventions?
Colleagues in the same team (peers) can and do look out for each other.Colleagues in the same team (peers) can and do look out for each other. Intervention with a peer can happen before a rule is broken OR to correctan unsafe act or situation. Incident Reporting
What is an Incident?A i id t i l d d i d t th t i j tAn incident is an unplanned or undesired event that can cause injury, asset damage and or impact environment or reputation.

What is a Near Miss?

What is an Incident with outcomes
An incident may have outcomes such as injury to personnel, asset damage, environmental or
reputational impact occurs. This includes a FAC, MTC, RWC too.
All incidents are Reportable irrespective of whether d blrecordable or not
Why Intervention is Important:
Let us review few incidents / conditions, where timely intervention could have yprevented incident / unsafe conditions:
Crane toppled – Dangerous occurrence
While offloading the concrete poles, the crane toppled. This happenedWhile offloading the concrete poles, the crane toppled. This happened because the outriggers were not extended in one side.
Pickup Rollover
A speeding Driver lost his control over thevehicle while travelling on rights of wayvehicle while travelling on rights-of-way(ROW) and the vehicle moved to the L/Hside edge of the road. Driver steered to hisright and simultaneously applied brake toright and simultaneously applied brake tocontrol the vehicle, which resulted into rollover.
They choose the Private ROW (Shortcut road) as decided to reach destination early.

One employee was standing into the wooden box, full of Master seal

• Driver lost control over the vehicle when he tried to move away from a b i th d d itbumpy area in the road and it swerved to the right side of the road then hit the shoulder and tipped over to the right side.

Tragic accident happened near Runip ( towards RIMA) road, wherehead‐on collision occurred between a trailer and a canter. As perinitial report 3 persons in the canter were killed on the spot.
