RE: Downhole Camera Inspection

Dear Bryan

This letter is informing WorkSafe that Pike River Recovery Agency (PRRA) intends to deploy a non-intrinsically safe or non-explosion protected camera, a Cals scanner, and lighting equipment down our existing Borehole 51. This work is required to determine the final geometry of the roadway between the Rocsil plug and the roof fall WorkSafe have previously confirmed that notification of high-risk activity is not required. However, WorkSafe pointed out that the use of plant in an ERZ0 that is non-intrinsically safe or non-explosion protected is prohibited by Regulation 193. WorkSafe are aware that cameras, Cals scanners and lighting have previously been used down Boreholes at this operation under PRRA’s management. Given the protocols employed by PRRA at the time, including no personnel underground at the time the equipment is deployed, WorkSafe did not lodge an objection to the activity. Therefore, please find attached:

• A detailed step-by-step plan outlining how the task will be performed safely along with the associated risks and control measures.

• Downhole camera operating procedure.

• Downhole Cals scanning operating procedure.

PRRA would like to conduct this work on 25th November 2020 weather permitting, or as soon as possible afterwards. If any further information is required, please contact me directly. Yours sincerely

Kevin (Dinghy) Pattinson Chief Operating Officer Te Kahui Whakamana Rua Tekau ma Iwa – Pike River Recovery Agency
