WELCOME TO AMERIGROUP COMMUNITY CARE! Amerigroup Community Care is a community-based health plan offering NJ FamilyCare benefits and more. We also provide long-term services and supports to those who qualify. This provider directory will help you and your Amerigroup network doctor work together to make sure you get the right care at the right time.

You can search the directory online at www.myamerigroup.com/NJ. You can print out the results of the online search. We’ll send you a printed directory if you ask. For more information about the providers listed here, call Amerigroup Member Services at 1-800-600-4441 (TTY 711).

WHAT IS A PROVIDER? A provider is any doctor, dentist, pharmacist, optometrist, specialist, hospital, or other medical group or facility that works together with Amerigroup to provide you with the right care, with access for members with disabilities.

WHO IS YOUR FAMILY DOCTOR? Your family doctor or physician, also called a primary care provider (PCP), provides your general health care. Your PCP will help you manage the health care needs of you and your family. At Amerigroup, all covered medical services or treatments will be arranged by your PCP, except obstetrics and gynecology (OB/GYN), dental, vision and emergency care.

WHAT IS A SPECIALIST? Your PCP can take care of many of your health care needs, but you may also need care from some other kind of doctor. This doctor is called a specialist. Amerigroup works with many different types of specialists. Your PCP can refer you to one if you need special care. Remember, unless you need dental, vision, pregnancy, post-pregnancy or gynecological care, Amerigroup recommends you get a referral from your PCP to see a specialist. We also recommend you get a referral from your PCP to go to a hospital, except in cases of emergency. See your member handbook for more information.

WHO IS YOUR FAMILY DENTIST? Just as you see your PCP for all your routine medical care, you will see your family dentist, also called a primary care dentist (PCD), for your dental care. By going to your PCD routinely, they will get to know you and what your specific dental needs are. If you have special dental needs, your PCD can refer you to a dental specialist.

WHAT IS EMERGENCY CARE? Emergency care is needed if your illness or injury is life-threatening or could seriously hurt your health. In an emergency, call 911 or go to the nearest hospital as soon as possible. Some examples of a medical emergency are: Very bad bleeding that does not stop Severe pain Chest pains/facial paralysis Very bad burns

Passing out Shakes called convulsions or seizures Trouble breathing Miscarriage Broken bones Throwing up blood Thinking of hurting yourself or others Drug overdose or poisoning Rape/sexual assault

WHAT IS URGENT CARE? Urgent care is when you need to see a doctor quickly, but you don’t need to go to the emergency room. If you aren’t sure you need emergency care, call your PCP or the Amerigroup 24-hour Nurse HelpLine at 1-800-600-4441 (TTY 711). They can help you decide. Examples of health issues needing urgent care are: Earaches Muscle sprains/strains Headaches Throwing up Fever over 101 degrees Fahrenheit Sore throat Minor burns or cuts Controllable bleeding

For urgent care that requires you see a dentist but not go to the emergency room, you can have an acute or chronic dental condition such as: Facial swelling due to infection A toothache A tooth that was knocked out or broken

Call your dentist immediately or call Amerigroup at 1-800-600-4441 (TTY 711). Your dentist can help you get needed treatment.

HOW DO I GET TRANSPORTATION TO A MEDICAL APPOINTMENT? Amerigroup covers ground and air transportation for members in cases of emergency only. Members receive all other medical transportation services through NJ FamilyCare Fee-for-Service. To find out more about getting a ride to your doctor visits, call LogistiCare at 1-866-527-9933 (TTY 1-866-288-3133).

For routine transportation to your doctor appointments, please call LogistiCare at 1-866-527-9933 (TTY 1-866-288-3133). Transportation does not require approval. However, you must call the LogistiCare number beforehand to have your need reviewed. If you can’t get a ride to your appointment, call Member Services at 1-800-600-4441 (TTY 711). If you need emergency care and have no way to get to the hospital, call 911 for an ambulance. If you don’t need emergency care but need an ambulance, a mobile intensive care unit or an invalid coach to get to a medical appointment, call LogistiCare at 1-866-527-9933 (TTY 1-866-288-3133).

If you are an Amerigroup member receiving Managed Long Term Services and Supports (MLTSS) benefits, Amerigroup will arrange nonmedical transportation for you. This includes rides to grocery stores, banks and places of worship.

MLTSS transportation services are based on your plan of care and must be approved ahead of time. When possible, you should ask family, neighbors, friends, public transit or community groups for help. MLTSS transportation services are set up by your MLTSS care manager. Call us at 1-855-661-1996 (TTY 711) to find out more.

HOW DO I GET CARE IF I HAVE A DISABILITY? All Amerigroup network doctors and hospitals will help members with disabilities get care. Members who use wheelchairs, walkers or other aids may need help getting into an office. If you need a ramp or other help, make sure your doctor’s office knows before you get there. This way, the staff will be ready to help you. If you want help talking to your doctor about your disability needs, call your MLTSS care manager at 1-855-661-1996 (TTY 711). A care manager will help you get the care you need.

HOW DO I CHOOSE MY OWN DOCTOR OR DENTIST? Please look at the list of PCPs in this directory. Family doctors include family practitioners, general practitioners, pediatricians (for children), internal medicine doctors and physicians. You may already know a family doctor, or a friend may be able to recommend one. You should choose a PCP who is easy for you to get to. Each person in your family can choose a different PCP. Call or visit the provider’s office to make an appointment. Your Amerigroup member identification card (ID) has the name and phone number of your PCP. If you want to choose a different PCP, please call Member Services at 1-800-600-4441 (TTY 711). Or you can visit our website at www.myamerigroup.com/NJ.

Your dental benefits are provided through LIBERTY Dental. You can choose a primary care dentist (PCD) by visiting the LIBERTY online directory at www.libertydentalplan.com. If you don’t choose a PCD, one will be assigned to you. You can choose a different dentist if you

aren’t happy with the one assigned to you. To choose a different PCD, call LIBERTY Member Services at 1-833-276-0848 (TTY 711). You should choose a family dentist who is easy to get to. Each person in your family can choose a different PCD. You may choose a pedodontist (children’s dental specialist) for your child’s PCD. Call or visit the dentist’s office to make an appointment. You’ll see your PCD for all your general dental needs, including checkups, cleanings, and routine fillings and extractions. If you have special dental needs, your PCD can refer you to a dental specialist.

HOW DO I CHOOSE A DENTIST FOR MY CHILD? Many dentists in the Amerigroup network proudly participate in the NJ Smiles program. This is a program just for children, ages 0-6 years old. We’ve made it even easier for you to choose your child’s dentist with the NJ Smiles Directory. You can view the directory online at www.myamerigroup.com/NJ.

If you need help making an appointment for a dental visit, please contact LIBERTY Member Services at 1-833-276-0848 (TTY 711) Monday through Friday, 8 a.m. to 8 p.m. Eastern time.

HOW DO I GET OUTPATIENT THERAPY? Your outpatient therapy care (including occupational therapy, physical therapy and speech pathology services) will be set up through the HN1 Therapy Network (HN1 TN). Each person in your family can choose a different therapist. Your therapy provider should call HN1 TN at 1-855-825-7818 (TTY 711) for preapproval for therapy services.

HOW DO I CHOOSE MY PHARMACY? We work with many pharmacies to fill your prescriptions. You may go to any of the pharmacies listed in this directory. If you have any questions, ask your PCP or call Member Services at 1-800-600-4441 (TTY 711).

HOW DO I CHOOSE AN EYE DOCTOR? Superior Vision provides eye care services for Amerigroup members. This directory contains a list of Superior Vision eye doctors. You should choose an eye doctor who is easy for you to get to. Call or visit the office to make an appointment. Have your ID with you when you go to your appointment. Identify yourself as an Amerigroup member covered through Superior Vision. This will allow the Superior Vision eye doctor to get information about your coverage. If you don’t identify yourself as an Amerigroup member, your claim won’t be processed correctly. Call Superior Vision at 1-800-879-6901 (TTY 1-800-735-2258).

HOW DO I CHOOSE A HOSPITAL? If you need to use a hospital, your PCP will refer you to one. They will phone Amerigroup for approval and set up your stay. You must use an in-network hospital unless you are out of town and unable to return or have a life-threatening emergency. In an emergency, go to the nearest hospital. If you have any questions, ask your PCP or call Member Services at 1-800-600-4441 (TTY 711).

WHAT ARE MANAGED LONG TERM SERVICES AND SUPPORTS (MLTSS)? We are here to help you take care of your physical and behavioral health and provide managed long term services and supports (MLTSS). Long-term services and supports provide help with daily activities like getting out of bed, using the bathroom, dressing, bathing, and eating. The MLTSS program helps you stay healthy and independent.

As an Amerigroup member in the MLTSS program, you will be given your own care manager. You’ll work with your care manager to help manage all your health and medical needs. Together, we’ll create a plan of care to help choose the right services and supports for you.

WHAT ARE MY MLTSS BENEFITS? Amerigroup makes it easy for you to get all your physical health, behavioral health and long-term care needs. In addition to your regular NJ FamilyCare benefits, MLTSS members can use services and supports like: Assisted living Community residential services Community transition services Home-based supportive care Home-delivered meals to members

18 years and older Day programs Home and vehicle modifications Nursing facility care Personal care assistant services Personal emergency response system

for members 18 years and older Respite care Behavioral health services

If you don’t have an MLTSS care manager, but you feel that you need these services, please call our MLTSS Care Management department at 1-855-661-1996 (TTY 711).

MANAGED LONG TERM SERVICES AND SUPPORTS PROVIDERS See the section Primary Care Providers to get a list of PCPs available to members enrolled in the Managed Long Term Services and Supports (MLTSS) program. MLTSS services include nursing facility, assisted living, and home- and community-based services. MLTSS members should also use the other sections of the Amerigroup provider directory to find providers for common medical needs that aren’t covered under MLTSS benefits. MLTSS members have access to all Amerigroup network providers.

All MLTSS services must be coordinated by

your MLTSS care manager. If you are anMLTSS member, your MLTSS care managerwill arrange all your care and services

among your providers.

Your care manager will: Ensure you get the right care at the

right time Help you stay healthy Help you stay out of the emergency room and out of the hospital

MLTSS members who need help with bathing, dressing or other activities of daily living can call our MLTSS Care Management department at 1-855-661-1996 (TTY 711).

WHAT ARE BEHAVIORAL HEALTH BENEFITS?

Your physical and behavioral health work together to maintain your well-being. These benefits are meant to help you with mental health care and substance use recovery. Depending on your eligibility, you can get behavioral health benefits to help you with your specific health needs. Amerigroup provides behavioral health services in an acute care inpatient setting for all members. If you are a client of the Division of Developmental Disabilities (DDD) or Managed Long Term Services and Supports (MLTSS) member, you may also be eligible for both mental health and substance use outpatient services. Some of the behavioral health services Amerigroup will cover are:

Adult partial care and partial hospitalization programs

Substance use disorder services Inpatient hospitalization

You should talk to your PCP if you think you may need any of these services. You can also find more specific details about your eligibility and services in your member handbook. You can call our 24-hour Nurse HelpLine at 1-800-600-4441 (TTY 711) to speak to a behavioral health specialist for a referral to a provider. You can also call our Behavioral Health Crisis Line at 1-877-842-7187 (TTY 711) to speak to a clinician if you feel that you are in crisis. The crisis line should not be used instead of 911 — call 911 if you are in need of immediate emergency services.
