The service area maps in this directory show the counties where Aetna Medicare plans are offered.

If coverage for an Aetna Medicare plan is offered in only part of a county, then a list of covered zip codes

is provided. Please take the time to become familiar with the service area map specific to you and your plan.

The two service area maps on the following pages are for Aetna Medicare Individual plans and Aetna

Medicare Employer Group plans.

Q: How do I know if I am an Individual plan member and which map should I use?

A: You can check your member ID card. Look for a 4-digit number on the front of your card that has a letter

before it. For example: H3623. You will see a 3-digit number after that number. For example: 001. If that

3-digit number starts with any number besides an 8, then you are an Individual plan member.

If you are still unsure if you are an Aetna Medicare Individual plan member you may contact us to find out.

Call our Member Services department at 1-800-282-5366 (TTY: 711), 8 a.m. to 8 p.m., seven days a

week. As an Individual plan member, you should use the Aetna Medicare plan Individual service area

map to understand where your plan is available.

Q: How do I know if I am an Employer Group plan member and which map should I use?

Section 1 – Introduction

This directory provides a list of Aetna MedicareSM

Plan (HMO) (PPO) network providers. To get

detailed information about your health care

coverage, please see your Evidence of Coverage.

If you are enrolled in a standard Aetna

MedicareSM

Plan (HMO), you will have to choose

one of our network providers who are listed in this

directory to be your primary care {physician/

provider} (PCP). The term “PCP” will be used

throughout this directory. Generally, you must get

your health care services from your PCP. Your PCP

will issue referrals to participating specialists and

facilities for certain services. For some services,

your PCP is required to obtain prior authorization

from Aetna Medicare. Except for emergency or

urgent care situations and for certain direct access

services, you will need to obtain a referral from

your PCP before seeking covered non-emergency

specialty or hospital care. You must use network

providers except in emergency or urgent care

situations or for out-of-area renal dialysis. If you

obtain routine care from out-of-network

providers neither Medicare nor Aetna Medicare

will be responsible for the costs.

If you are enrolled in an Aetna MedicareSM

Plan

(HMO) open access, you are not required to

choose a primary care {physician/provider} (PCP)

and are free to visit network providers without a

referral to receive covered services. The term

“PCP” will be used throughout this directory. You

must use network providers except in emergency

or urgent care situations or for out-of-area renal

dialysis. If you obtain care from out-of-network

providers neither Medicare nor Aetna Medicare

will be responsible for the costs.

If you are enrolled in an Aetna MedicareSM

Plan

(PPO), you have the flexibility to choose either

network providers to receive covered services or

out-of-network providers. With the exception of

emergency or urgent care situations or for out-of-

area renal dialysis, it may cost PPO members more

to get care from out-of-network providers. See

your Evidence of Coverage for details. Although

you are not required to select a primary care

physician (PCP), you are encouraged to do so. If

you receive covered services from an out-of-

network provider, it is important to confirm they

will accept your PPO plan and are eligible to

receive Medicare payment.

The network providers listed in this directory have

agreed to provide you with your health care,

vision, hearing and dental services. You may go to

any of our network providers listed in this

directory; however, some services may require a

referral. If you have been going to one network

provider, you are not required to continue to go to

that same provider. In some cases, you may get

covered services from out-of-network providers.

In cases where non-contracting providers submit a

bill directly to you, you should not pay the bill, but

should submit it to Aetna Medicare for processing

and determination of your cost-sharing amount,

if any.

Certain network health care services, such as

hospitalization or outpatient surgery, require prior

authorization from Aetna Medicare. This means

the service must be approved by Aetna Medicare

before it will be covered under the plan. Check

your Evidence of Coverage for a list of services that

require this approval. When you are to obtain

services requiring prior authorization from a

network provider, the provider is responsible to

obtain prior authorization from Aetna Medicare

before treatment. If your plan includes an out-of-

network benefit and you decide to receive covered

services from an out-of-network provider, we

recommend you ask your out-of-network provider

to contact Aetna Medicare for prior authorization

of those services.

Emergency care

If you need emergency care, you are covered 24

hours a day, 7 days a week, anywhere in the world.

Whether you are in or out of an Aetna Medicare

service area, we simply ask that you follow the

guidelines below when you believe you need

emergency care.

• Call the local emergency hotline (for example,

911) or go to the nearest emergency facility.

Notify your PCP as soon as possible after

receiving treatment. Or if a delay would not be

harmful to your health, call your PCP.

• If you are admitted to an inpatient facility, you,

a family member or friend on your behalf

should notify your PCP or Aetna Medicare as

soon as possible.

If you are traveling outside of the Aetna Medicare

Plan (HMO) (PPO) service area, you are covered for

emergency and urgently needed care. Emergency

care can always be obtained in or out of the

service area from the nearest available provider.

When in the network service area, HMO plan

members must use plan providers for urgent care

unless the providers are not available. When out of

the service area, urgent care may be obtained

from the first available provider. Urgent care may

be obtained from a private practice physician, a

walk-in clinic, an urgent care center or an

emergency facility. We will review the information

submitted to us by the provider who supplied your

care. If the nature of the urgent or emergency

problem does not qualify for coverage, you may

have to provide us with additional information.

Follow-up care after emergencies

All follow-up care should be coordinated by your

PCP. For HMO plans, follow-up care with out-of-

network providers may only be covered with a

referral from your PCP and our prior authorization.

Whether you were treated inside or outside your

Aetna Medicare service area, if your plan requires

referrals, you must obtain a referral before any

follow-up care can be covered. If your plan does

not require referrals you should contact our

Member Services department at 1-800-282-5366

(TTY: 711) before care is received at out-of-

network facilities. Suture removal, cast removal,

X–rays and clinic and emergency room revisits are

some examples of follow-up care.

For PPO plans, in-network and out-of-network

follow-up care after emergencies will be covered

under the terms and conditions of your plan.

Si usted está inscrito en un Plan (HMO) de

Aetna MedicareSM

estándar, tendrá que elegir

uno de nuestros proveedores de la red que se

enumeran en este directorio para que sea su

proveedor de atención primaria (PCP)

{médico/proveedor}. El término “PCP” se usará en

todo el directorio. Por lo general, debe obtener los

servicios de atención médica de su PCP. Su PCP le

Atención médica de emergencia

Si necesita atención médica de emergencia, usted

está cubierto las 24 horas del día, los 7 días de la

está dentro o fuera de un área de servicio de

Aetna Medicare, simplemente le pedimos que siga

las siguientes pautas si piensa que necesita

atención médica de emergencia.

emergencia y de urgencia. La atención médica de

emergencia siempre se puede obtener dentro o

fuera del área de servicio del proveedor disponible

más cercano. Cuando en el área de servicio de la

red, los miembros del plan HMO deben utilizar los

proveedores del plan para recibir atención médica

de urgencia a menos que los proveedores no están

disponibles. Cuando fuera del área de servicio, se

puede obtener atención médica de urgencia del

primer proveedor disponible. La atención médica

práctica privada, una clínica sin citas, un centro

médico de urgencias o un centro médico de

emergencia. Revisaremos la información que nos

presenta el proveedor que suministró su atención.

Si la naturaleza del problema urgente o de

emergencia no cumple los requisitos para recibir

cobertura, probablemente sea necesario

proporcionarnos información adicional.

Seguimiento médico después de emergencias

Todo seguimiento médico debe ser coordinado

por su PCP. Para los planes HMO, el seguimiento
