Projector Repair Request Form Please fill out the following form and include with your projector shipment. All information must be complete in order to process your repair in a timely fashion. Incomplete forms may experience delays in projector repair. Failure to do so may result in labor, parts, and shipping charges on your projector repair. If the projector is experiencing a “replace lamp message or lamp life warning indicator”, please make sure that the projector has had its bulb life hours reset. Projectors do not auto reset their life count when a bulb is replaced and with many projector models, they will auto turn off after power on to prevent a bulb that is exceeding end of life from exploding. Many times resetting the life hours of the bulb will correct the issue. NOTE: Only perform this if a new bulb has recently been installed. If the projector is experiencing “dimming of the image”, please first try a new, unused bulb in the projector to see if the image quality improves. Dimming is many times due to a bulb that is about to expire and the bulb filament is unable to maintain a proper “arc”. This is a pre-cursor to a bulb exploding. If a projector is shipped to us without a bulb or with an exhausted bulb and a new bulb is needed to service the projector, IPS global will purchase a bulb on your behalf. Your school/business will then be charged for the price of the bulb. IPS does not cover service or repairs associated to the DLP light engine assembly or the LCD light engine assembly. The DLP components include the DMD (Digital Mirror Device) chip, DMD/GPU processor, color wheel motor (including color palette disc), and color wheel motor encoder. The LCD light engine components comprise of the LCD screen(s), LCD polarizing film(s), and light alignment lens(es). If you would like assistance in preliminary troubleshooting, feel free to contact an IPS global technician at (800) 347-2913. Please make sure to fill out one copy of this form to accompany each projector being sent to IPS global for repair. You may ship multiple projectors in one box, and please make sure the paperwork included is submitted correctly per device.
