About VI Aesthetics

In 2005, Dr. Abdala F. Khalil and team founded VI Aesthetics with the mission to restore and renew their customers’ confidence in their own skin health with a portfolio of chemical peels that specifically address a variety of skin concerns including anti-aging, pigmentation and acne. The VI Peel® portfolio systems contains a synergistic blend of powerful ingredients suitable for a variety of skin types. The peel improves the tone, texture and clarity of the skin. It reduces or eliminates solar lentigentines (age spots), treats hyperpigmentation (including melasma), and softens fine lines and wrinkles. The VI Peel® is also an excellent choice to address acne and reduce or eliminate acne scars. The VI Peel® stimulates the production of collagen, for healthier, firmer more youthful skin. This self-neutralizing peel provides dramatic results with virtually no pain, no skin preparation and minimal downtime.

Suitable for a variety of skin types and conditions and is safe for face and body treatments.

Key Ingredients:

• Trichloroacetic acid (TCA) is a non-toxic chemical (trichloracetic acid), which has been used to perform skin peels for over 20 years. It is a relative of vinegar (acetic acid.) When TCA is applied to the skin, it causes the top layers of cells to dry up and peel off over a period of several days to one week. TCA provides excellent exfoliation properties while stimulating collagen production.

• Retinoic acid (Vitamin A) Vitamin A promotes collagen and elastin production. Helps to increase water levels within the epidermis, normalizes cell turnover and is a melanogenesis inhibitor.

• Salicylic acid is a beta hydroxy acid (BHA) with anti-inflammatory properties and is an effective anti-acne ingredient working as a keratolytic, clearing out dead cells.

• Phenol has the ability to trap free radicals, work as a chemical cautrerant to exfoliate the skin, has preservative properties and in low percentages is an effective numbing agent.

• L-Ascrobic acid (Vitamin C) is a very stable , water soluble form of vitamin C, an antioxidant, photo protectant (protects from UV damage). It helps stimulate collagen production, brightens the skin, and supresses melanogenesis.

Peel For Pigmentation and Sun Damage

Hyperpigmentation is a condition in which patches of skin become darker than the surrounding skin. Although it is usually harmless, it can be difficult to effectively treat. VI Peel® Precision Plus can help restore the skin to its natural tone. The Precision Plus is a powerful booster and towelette system designed specifically to treat stubborn pigmentation.

VI Peel®

Patient Selection:

VI Peel® Application - To be applied in a practitioner’s office only

Step 1 - Cleanse Cleanse the patient’s skin using the VI Derm® Cleanser and make sure all makeup and oils have been removed.

Step 2 - Degrease Each VI Peel® Box contains 1 Acetone Prep Towelette for each peel patient. Degrease all areas on the skin where the VI Peel® solution will be applied. It is important to remove all oils from the stratum corneum for optimal peel penetration. For those patients with more oily and acneic skin, it is recommended to do extra passes using 100% pure acetone. Apply Vaseline® to lips, corner of eyes and over any permanent makeup as a barrier ointment.

Step 3 - Apply VI Peel® Solution Use caution when opening. Remove the top. Point the vial away from you and your patient. With your thumb over the top, remove metal seal and gently remove the stopper. Pour the entire solution into the plastic container provided in the box. Remove the 2x2 gauze from packaging and have it ready for application. You may have a fan available for your patient for comfort.1st Pass - Apply solution using firm pressureFold the 2x2 gauze in half. Dip a portion of the gauze in the solution and squeeze out excess solution. Start at the forehead and apply peel with firm pressure, working down the face and neck area. Once the first pass has been applied, wait a couple of minutes to see how the patient’s skin is reacting. Avoid the eyelids and under eye area until the last pass.2nd Pass - Apply solution using firmer pressureApply solution with firmer pressure. If frosting is observed, the application can be slowed down and more time allowed between passes. If it’s very light frosting, you may apply over that area again. If there’s heavy frosting, avoid that area and continue with the rest of the face or treated area.

Contraindications:

• Pregnancy• Breastfeeding• Immune disorders • Phenol allergy • Aspirin allergy• Inflamed skin with open

wounds• Isotretinoin (Accutane®) use

3rd Pass - Apply more vigorously for deeper penetrationApply vigorously, especially to the areas of concern, such as the fine lines, wrinkles, rougher texture or pigmentation.4th Pass - Most likely the last pass, (varies from patient to patient).Continue layering the VI Peel® until you have used all of the solution. Eye area application - To be completed on the last pass.For the under eye area, inform the patient to look upward and lightly apply peel solution from tear duct to outer edge of the eye. For the upper eyelid/brow bone area, have the patient close their eyes and lightly apply solution. Never apply to lower eyelid. Use of light pressure is key as the skin in these areas is thinner.

Step 4 - Explain take home kit to patientFour hours post treatment: Cleanse the skin with VI Derm® Cleanser (included in the take home kit). Rinse with cool or warm water and pat dry. (NO HOT WATER ON SKIN FOR 7 DAYS).Apply the 1st towelette. The towelette is to be rubbed vigorously over the entire treated surface of the skin. Advise patient to avoid passing towelette over eyelids and lips. Problem areas may be rubbed more vigorously for best results. The skin should not be cleansed after the towelette application. Patient should wash hands thoroughly after application of towelette. Makeup can be worn. Use the Post Peel Protectant as a moisturizer before applying makeup. NO OTHER personal hydrating products should be applied.IMPORTANT - Fitzpatrick Skin Types 4, 5 and 6 must apply the Post Peel Protectant approximately 10 minutes after the towelette has been fully absorbed.Night 1 - One hour before bedtime:Cleanse the skin with VI Derm® Cleanser. Rinse with cool or warm water and pat dry. Apply the 2nd towelette provided in the post peel package. Remember to wash hands after application.*If the VI Peel® application was applied later in the evening, the patient may use the 2nd towelette the following morning.IMPORTANT - Fitzpatrick Skin Types 4, 5 and 6 must apply the Post Peel Protectant approximately 10 minutes after the towelette has been fully absorbed.Day 2 - Morning: Cleanse skin with VI Derm® Cleanser. VI Peel® Precision Application - To be applied in a practitioner’s office only

Step 1 - Cleanse Cleanse the patient’s skin using the VI Derm® Cleanser and make sure all makeup has been removed.

Step 2 - Degrease Each VI Peel® Precision Box contains 1 Acetone Prep Towelette for each peel patient. Degrease all areas on the skin where the VI Peel® solution will be applied. It is important to remove all oils from the stratum corneum for optimal peel penetration. Apply Vaseline® to lips, corner of eyes and over any permanent makeup as a barrier ointment.

Step 3 - Apply VI Peel® Precision Solution Use caution when opening. Remove the top. Point the vial away from you and your patient. With your thumb over the top, remove metal seal and gently remove the stopper. Pour the entire solution into the plastic container provided in the box. Add the Precision booster to solution. Squeeze the sides of the booster vial to release the entire contents of the booster. Mix the solutions together with the top of the booster tube and stir. Remove the 2x2 gauze from packaging and have it ready for application. You may have a fan available for your patient comfort.1st Pass - Apply solution using firm pressureFold the 2x2 gauze in half. Dip a portion of the gauze in the solution and squeeze out excess solution. Start at the forehead and apply peel with firm pressure, working down the face and neck area. VI Peel® Body Protocol with Precision or Precision Plus

VI Peel® Décolleté Protocol with Precision or Precision Plus Application - To be applied in a practitioner’s office only

Step 1 - Cleanse Cleanse the patient’s body parts being treating using the VI Derm® Cleanser and make sure all debris is removed.

Step 2 - Degrease Each VI Peel® Box contains 1 Acetone Prep Towelette for each peel patient. Degrease all areas of the skin where the VI Peel® solution will be applied. It is important to remove all oils from the stratum corneum for optimal peel penetration. For those patients with more oily and acneic skin, it is recommended to do extra passes using 100% pure acetone.

Step 3 - Apply VI Peel® Precision or Precision Plus SolutionUse caution when opening. Remove the white top. Point vial away from you and your patient. With your thumb over the top, remove metal seal and gently remove the stopper. Pour entire solution into the plastic container provided in the box. Add the Precision or Precision Plus booster to solution. Squeeze the sides to release the entire contents of the booster. Mix the solutions together with the top of the booster tube and stir. Remove the 2x2 gauze from packaging and have it ready for application. You may have a fan available for your patient for comfort.1st Pass - Apply solution using firm pressureApply peel with firm pressure on the body using 2x2 gauze. Once the first pass has been applied wait a couple of minutes to see how the patient’s skin is reacting. 2nd Pass - Apply solution using firmer pressureApply solution with firmer pressure. If frosting is observed, the application can be slowed down and more time allowed between passes. If it’s very light frosting you may apply over that area. If there’s heavy frosting, avoid that area.

Review any contraindications with patient and have consent form completed.**Precaution: A maximum of 2 vials can be done during 1 treatment. Never apply more than 2 vials within 48 hour due to toxicity. ** Have the patient wear loose clothing.

Setting realistic patient expectations for any aesthetic treatment can be a challenge. Explaining the VI Peel® to your patient, and what they should expect, is an important step during the consultation process. Some patients have unrealistic expectations about aesthetic treatments. The biggest misconception that practitioners face is that their patients think they will have heavy peeling or peel in sheets. There will always be exfoliation with the VI Peel®, but it may be at the cellular level, which is not always visible to the naked eye.

The degree of peeling is affected by many factors:

Skin preparation: Skin must be thoroughly cleansed and fully degreased with 100% pure acetone. This step is very important. It strips the skin of all oils and debris, allowing for the optimal penetration of the VI Peel®. If patient has oily skin it is suggested to degrease the skin twice.

Application Method: VI Peel® portfolio must be applied with the gauze provided in the peel kit. It is loosely woven material which makes it abrasive. The peel solution must be applied vigorously to drive the peel solution to the papillary dermis.

Products used at home: Does the patient use retinol products or do they exfoliate on a regular basis? When was their last VI Peel® or laser treatment? If a patient is having a series of VI Peel® treatments, the amount of peeling may vary between each VI Peel® treatment.

Reasons why a patient may not peel heavily: Just because a patient did not peel in sheets, doesn’t mean the peel was not effective. Sometimes a patient might be so well exfoliated that their skin does not need to peel heavily. It is still exfoliating at a cellular level, which is not visible to the naked eye. If a patient with very oily skin is not degreased properly, the peel may not penetrate to the papillary dermis, which is needed for an efficacious outcome.

Application technique: Did you cleanse the patient’s skin? Did you completely degrease the skin with 100% pure acetone? Did you use the gauze provided? Did you vigorously rub in the peel solution with multiple passes, and not use all the solution on the first pass? Did you use the full VI Peel® vial? Did you apply anything on top of their skin before they left the office?

Patient home care: Make sure the patient didn’t apply anything to their skin for 4 hours after solution is applied. This includes VI Derm® SPF 50+ or any other products. Did the patient leave the peel solution on for 4 hours? Did they follow the towelette protocol on day 1 and day 2? Did they exercise or raise their body temperature during the first 72 -96 hours ?

All of the above factors will affect the way a patient will peel. Remember, peeling will vary from light flaking to sheet peeling. Educating your patients and managing their expectations are keys to a successful VI Peel® treatment.

Practicioner / Physician Application NotesThe VI Peel® comes pre-mixed in a ready to use vial containing 5ml. of solution. The formulation includes a proprietary blend of TCA, retinoic acid, phenol, salicylic acid (each under 15%), Vitamin C and a mineral blend. No skin preparation is needed.

Please read the Practitioner Instruction For Use which is included in the VI Peel® Kit

• Begin by giving the patient a fan. They may fan the face during the application process for comfort.• The peel solution migrates underneath the surface of the skin. You may wish to apply Vaseline® or Aquaphor® to the

corners of the mouth and eyes to prevent possible post peel dryness or irritation.• Open the vial and carefully remove the rubber stopper facing away from you and the patient. Pour the contents of the

vial into the plastic cup provided, and use the 2x2 gauze to apply the solution. Make sure that you have enough solution in the gauze but not so much as to where it would be dripping onto the patient’s skin. The skin should look damp, but product should not be running down the face. Begin applying the solution ( do not rub it in yet) onto the face in an evenly distributed manner. The most common peel application technique is to begin at the forehead, working down systematically covering the whole side of the face down to the neck, then repeating on the other side. If you are applying the peel to any other than the face, please refer to the appropriate protocol.

• Apply solution evenly. The skins reaction will tell you when to stop. If you get heavy frosting, then discontinue application in that area. If you get a partial frost , avoid only those areas and continue with the application. Concentrate solution on areas of concern (breakouts, pigment, etc.).

• On the last pass of the peel apply solution to the brow bone and upper eyelid only.• Be careful to keep peel solution from getting into the eye.• If this does occur, use a clean dry gauze or Q-tip, swiping the eyes from one corner to the other. The peel will not

harm the eye; it will sting for a few seconds and then subside.• Turning pink, red or even frosting is normal. This reaction will subside shortly. Some patients may remain pink, red

blotchy. This is normal due to the activity of the peel. Some patients look fine when they leave the office, but then appear pink or red a few hours later. This is due to the peel penetrating more deeply into the skin causing the skin to flush. The peel is self-neutralizing; do not wash it off.

• Inform patients that with the first pass they may have slight tingling or burning for 30-45 seconds, but will subside quickly with some numbing as second pass begins.

• The patient is not to apply anything to the face prior to four hours post peel, this includes sunscreen.• After four hours the patient may apply makeup after applying first towelette and using post-peel protectant as

moisturizing base for makeup.• Remind the patient how crucial the post peel towelettes are; that they must be rubbed into the skin over all peeled

areas. Please instruct the patient to use all of the solution on the towelette, remembering to rub under the eye and over the brow line. Do not rub on the lower eye lids or lips. Wash hands after each towelette.

• Advise the patient that at any time the Post Peel Protectant or sunscreen included in the post peel kit (or combination of both) irritate or burn, they should discontinue use immediately. They can substitute with hydrocortisone cream, coconut oil or Aquaphor® that they have at home that will not irritate the skin. No regular skincare products should be used for the full 7 days of the exfoliation process.

Practicioner / Physician Application Notes (continued)• The patient should refrain from activities that will produce excessive amount of perspiration for the next 72-96 hours

after the VI Peel® is applied. Sweat may induce water blisters. The water blisters will subside in 30-40 minutes when the patients’ temperature cools down. It is best to avoid blistering altogether so there is no risk of complications.

Important points to discuss with the patient• Advise the patient to leave the peel on for at least 4 hours. They may cleanse after this time. • Do not apply anything to the face (or peeled area) including sunscreen prior to 4 hours post peel application. Make

up may be applied after the four hour time frame and they have applied the first towelette.• If significant discomfort is experienced when applying after any post peel product after treatment, discontinue use of

that product; cleanse the face with soap and water to stop the discomfort.• The POST PEEL PROTECTANT may sting upon application, but stinging quickly subsides. • Most patients look great the day of the peel and the next day. Skin will be tight and appear pink or tan. The patient’s

skin will look similar to the way it would look after sun exposure. If the patient tans well, their skin will most likely ap-pear tan. If not, it may appear pink.

• It is important to note that prior to peeling , any areas of pigmentation will appear darker. This change in color will be most evident on the second day, however it can happen immediately. This is normal; the darker skin will exfoliate during the peeling process.

• During the peeling phase, patients must avoid sun exposure. The use of sunscreen is essential. The patient may go out in the sun when exfoliation subsides, but their skin will be more sensitive to UV rays. Instruct the patient to apply sunscreen every 2 hours when outdoors or exposed to sun.

• During the week after the peel, the patient must use a gentle cleanser. We recommend the VI DERM® CLEANSER for all skin types, along with the products in the post peel kit ( Post Peel Protectant and SPF 50+ Sunscreen Protection).

• Do not cleanse with Clarisonic Brush until the peeling process is complete (usually day 7).• Instruct the patient to refrain from forcing or peeling the skin as it can cause scarring or PIH. If they would like to

trim excessive skin from the face, a small pair of scissors is acceptable. Never force or peel the skin off in sheets. The skin will slough when washed morning and night

• No hot water, warm and cool only.• The post peel towelettes are a crucial part of the peeling process and must be applied on the first and second days,

as directed by your practitioner.• The post peel towelettes acts as a catalyst to the peeling process. It is part of the reason that the VI Peel® portfolio is

so consistent and predictable for a variety of skin types and conditions. A small percentage of patients say that after using post peel towelettes (especially first night), they experience some itching. If itching is extreme, the patient may wash the face to remove the solution or suggest taking an antihistamine.

• The post peel towelettes are typically applied in the evening, the patient applies their first towelette 4 hours after the peel, followed by a towelettes(s) the first evening of the peel, and repeat again next evening (2nd evening).

• Even if burning is experienced during application of the first towelette on night one, instruct the patient to apply the second towelette the next evening. Usually by day two, the towelette is easily tolerated.

• Each morning apply the post peel Protectant (with 1% hydrocortisone) along with VI Derm® SPF 50+ Sunscreen Protection.

• Typically peeling begins on the third day, starting in the peri-oral area, peeling outward. The forehead and neck will be the last areas to peel.

• Days 3, 4, and 5 are the most aggressive days of peeling. By day 6 most exfoliation has subsided. The skin on the body peels slower than the face; usually starting day 4-7 and taking up to 10 days or more.

• If at any time a product (sunscreen or Post Peel Protectant) irritates the skin; have the patient discontinue use and apply a 1% hydrocortisone cream, coconut oil or Aquaphor

• Applying VI Derm® SPF 50+ and Post Peel protectant frequently during the peeling process. It will help to camouflage the peeling and aid in the hydration of the new skin. This skin will look best approximately 3-4 days after the peeling process is complete. This is when the normal pH balance has returned.

• These peels may be performed every 4-6 weeks until the desired results are achieved. Most wil see a remarkable improvement after only one peel, but patients dealing with hyperpigmentation or acne will most likely benefit from a series of 2-4 peels.

• VI Peel® portfolio is safe for patients 12 and up.• Suggested maintenance treatments are 2-4 times pear year.

Practitioner Training Tips

• Always go over contraindications with patient. • Wear double gloves if you are administering multiple peels in one day. This will protect the skin from drying out.• Degrease skin twice if patient has oily/acne prone skin.• You can apply solution with a bit more aggression to penetrate deeper into the skin as well as be a bit more

aggressive on troubled areas of this skin (pigment/breakout).• Once heavy frosting in an area, avoid area so no damage occurs.• For darker skin types always instruct patient to use the Post Peel Protectant 10 minutes after each darker green

towelette application to help prevent PIH.• It is recommended to use Aquaphor® or coconut oil once the peeling has begun if you have dryness, redness or

chapping of the skin.• No exercising for 3-4 days after receiving peel• Instruct patients to increase water intake post peel.• For patients who wear makeup, advise them to clean makeup brushes during the peeling process so they are using

clean brushes on fresh skin once the exfoliation process is complete. • When patient is applying the towelettes, be sure to advise to not wipe over the lower eye lid and to tuck their lip in so

the wipe does not touch their lips. Wash hands after each towelette.• Take before and after photos for your patients to track the progress of their skin during treatments.

• Kojic acid aids in fighting discoloration and brightens the complexion. This ingredient is effective in evening skin tone because it is able to separate copper that is bound to tyrosinase and decrease the number of melanosomes (packets of pigment) and dendrites (the arms that extend from the melanocyte cell that transfer melanosomes to the keratinocyte cells). It is water-soluble and antibacterial.

• Azelaic acid provides antibacterial action and melanogenesis suppression. Azelaic acid has the ability to inhibit tyrosinase activity and also be cytotoxic (kill) malfunctioning cells without disturbing the healthy surrounding cells.

• Alpha-Arbutin is an antioxidant and skin conditioner found in wheat, pears, bearberries, blueberries and cranberries. It acts as a melanogenesis inhibitor by suppressing the activity of tyrosinase and by preventing melanosome maturation.

• Glycyrrhiza Giabra (Licorice) Root Extract offers anti-inflammatory properties by preserving the effect of the hormone cortisol, allowing it to maintain an extended anti-inflammatory action. It is a melanogenesis inhibitor because it suppresses tyrosinase, giving it brightening properties.

• Undaria Pinnatifida Extract (Wakame) is brown seaweed native to the coasts of China, Japan, and Korea; it has protective characteristics for the cells as well as anti-edema benefits.
