A Guest house in Kanyakumari near Kovalam beach road wasproposed by the Project Committee and after much discussions acceptedin the State Council. Earlier the Past President Council also acceptedthe proposal. We are on the process of purchasing 25 cents of land,5 fully furnished guest rooms, Kitchen, dining hall, small conference hallwith seaview. We are awaiting the legal validity and other mandatorycertificates. Once we get it the process of registration will be on and allmoney nearly 2 crores will be documented as white money.

We have also taken a positive resolution to help our FSS I andFSS II members (member having 5 years membership in FSS 1 or havemembership in both in FSS 1 and FSS-2) with the new IMA FSS FBSScheme. The members will be reimbursed up to Rs. 2,00,000/- forthe period of 5 years on their acute health care expenditures. Thisscheme will not cover chronic diseases. More so, only the treatinghospital scan claim their bill and these hospitals must have Nursinghome board membership. Member need to pay just 1000 as registrationfee only once for 5 years. System of preauth and online managementsystems will be in place.

The next state council meet is scheduled to be held in Andamanfrom 4th to 8th with great enthusiasm. Along with the tour schedule wewill have FSS, PPLSSS and NHB Management Committee will be heldon 5th and 6thnight. On 7th the State Council Meet will be held.The StateCouncil Members need to pay Rs. 10,000/- and accompanying personsRs. 25,000/-.

I am happy to note the Zonal sports meet were organized in aspectacular way by IMA Hosur by Dr. Dhanasekharan and Team. Mydream every zone shall organize a Zonal sports, Fine arts, Quiz andother competitions and at the end a zonal conference shall be hosted ineach and all these people shall be appreciated. I am requesting all Vicepresidents to come out with your Zonal meets in September.

IMA TNSB is hosting Inter State T20 Cricket Match in Chennai fromAugust 10th to 11th.The matches will be held in the Tamilnadu CricketLeague Ground. Attractive accolades are awaiting the winners. On 21st

we are having cricket selection match. Team to be selected and nurturedbefore the final match

IMA is a family. Let us not allow personal ego and petty politics toplay down cast of our Goals, ambitions and objectives. Let us not worrywho gets the credit but observe how much my fraternity or the communityis gaining through the work. I am really overwhelmed with joy to knowthe small branches like Thiruchehgodu hosting a fabulous state counciland family meet, Hosur youthfully celebrating sports day, Aranthangibuilding a fantastic IMA Home etc. There are many achievers. It is notthe money, but their mind, commitment and cohesive TEAM sprit hasdone these marvelous achievements. My dream and vision are everyIMA Family shall own a hut at least for them.

The pinky state Doctors day we celebrated with emollient euphemismon the day of the National Doctors day on July first itself in HotelGreenpark was a outstanding moment for us to relish the bonhomie withthe State Government and Accolade the elite privileged doctors of ourassociation.

I am honored to partake and adore with empathy of their exhilarationand happiness in more than 40 branches of our state during theDoctors day merriments and décor scores of our senior colleagues withDoctors day awards.

IMA is going to have a gargantuan Doctors day being hosted by theRotary International Clubs of Chennai on 28th in the Dr. MGR MedicalUniversity and 40 Doctors from Rotary and IMA will be revered. TheLions internationals will also celebrate Doctors day on 25th in Chennai.

IMATNSB envision to create a directory as compilation of all ourcolleagues practicing for more than 50 years in the community anddoing magnificent services. We will honour all of them with a unique IMAplague and will also talk to the Government to ensure they adequatelyhonor these prestigious, celebrated altruistic doctors.

The clinical establishment bill 2018 enacted by the state governmentis much levered by all and also from other states for its easy applicabilityand doctor friendly contents and vision to eradicate the quackery. Thedistrict committee will be constituted with JDHS as competent authorityand the Deputy director of medical services will be the chair person. Wewill have a nominee in each district and also a member from TN Medicalcouncil and Dean. I amsure this is a golden opportunity for us toempower and enroll all our member hospitals are registered. The mainresponsibility is to identify and eradicate the quackery. Please ensureno quack are getting registration.

I am privileged to inform we had a fabulous 295th State CouncilMeeting held by IMA TNSB and hosted effectively by IMA Thiruchengodu.Many positive decisions were taken.

Dear members of IMA Tamil Nadu

Greetings from IMA TNSB.

It gives me great pleasure in

writing to you again as your secretary

about the various activities that have

taken place in the last one month

On 9th and 10th of June, we had

our management committee meetings

in FSS and PPLSS, followed by the

FSC meeting in Yercaud. We had lot

of useful discussion. The idea of

starting FSS FBS was presented to

the FSS trust for the first time by our

state President and the members

under the chairmanship of

Dr. K. Vijayakumar discussed the

possibility in a positive manner.

On the 11th of the same month

the state office bearers of IMA Tamil

Nadu met the members of TNMC and

had a fruitful discussion on the various

modalities of Accreditation by TNMC

and a mutually agreed decision was

arrived.

On the 17th of June we had our

NHB meeting for north zone in Green

Park hotel and lot of useful information

was shared with the members of NHB.

The meeting became historic for the

fact that the much awaited rules of

CEA act was received by the state

office and presented to the state

President in the presence of various

office bearers who have negotiated for

what is the most doctor and

community friendly act in India today.

We take this opportunity to thank our

chief minister, health minister, and

health secretary for the same.

On the 19th, the Project

Committee met in Kanyakumari to

discuss purchase of property by IMA

TNSB and FSS in Kanyakumari. It was

decided to go through the legal

documents and present the issue to

our state council.

On 20th the state office bearers

met with the state health minister and

health secretary to invite him to our

State Doctors' Day Ceremony on July

first, which they readily agreed to.

On the 23rd, the FSS trust and

management committee met in the

evening in THIRUCHENGODE to

discuss the launch of FSS FBS and it

was decided to thoroughly analyse the

pros and cons and implement the

same in the nearest future.

On the 24th, the 295th state

council meeting took place in a very

nice and ambient atmosphere and

various issues were discussed in a

healthy manner. We congratulate the

office bearers and other members of

IMA THIRUCHENGODE for the great

show.

On the 30th, the first state level

meeting of the Medical students

network took place in a grand manner

in the presence of the vice chancellor

of Dr MGR Medical university Dr. Mrs.

Geetha Lakshmi. The organising

committee led by Dr. T. N. Ravi

Shankar and Dr. K.M. Abul Hasan did

a wonderful job.

On the first of July we celebrated

our State Doctors' Day in the presence

of our state health minister and health

secretary. Many senior leaders were

honoured for their great service to the

society and IMA. We thank all our

members for their cooperation and

participation.

On the 7th your secretary

attended the Doctors' Day ceremony

of IMA Hosur. The next day he had

the great pleasure of inaugurating the

West zone sports meet in the presence

of state vice president Dr. Senguttuvan

and President-elect Dr. C. N. Raja.

On the same after noon your

secretary participated in the Doctors'

Day ceremony and inauguration of the

MPH scheme in Vellore in the presence

of state vice president Dr. L. Yesodha

and three former state Presidents Drs.

Sadagopan, Damodharan and

Kesavan sirs.

In the late evening your secretary

managed to reach Cuddalore to

participate in the Doctors' Day

ceremony of IMA Cuddalore in the

presence of the district collector and

DDHS of Cuddalore district.

I take this opportunity to thank

all the above mentioned branches for

their love and affection showered on

me and in turn I promise to continue

to serve you better and better.

HbsAg carrier:

� HBV will be an in-active stage (i.e:HBV DNA viral load will be lessthan 2000 i.u/mL with normalSGPT (ALT)).

� In this stage, liver biopsy willshow no inflammation. As thereis no hepatic inflammation, it willnot lead to hepatic fibrosis orcirrhosis.

� Hence, these group of patientsdose not require oral anti-viraldrugs but needs to be on life-longregular surveillance (i.e: Once ayear – HBV DNA quantificationassay, USG abdomen, Alfa-fetoprotein).

Chronic Hepatitis B –

Active stage:

� HBV will be an active stage (i.e:HBV DNA viral load will be morethan 2000 i.u/mL with elevatedSGPT (ALT).

� In this stage, liver biopsy willshow inflammation. As there is anactive hepatic inflammation, it willlead to hepatic fibrosis orcirrhosisover the period of years.

Definition of viral hepatitis:

Inflammation in liver (hepatocytes) ifit is caused by viruses are known asViral hepatitis.

Types of viruses:

1. Hepatotrophic virus: It will affect primarily liver

Ex: Hepatitis A, Hepatitis B, Hepatitis C, Hepatitis D,Hepatitis E.

2. Non-hepatotrophic viruses: It will cause systemicdiseases including liver

Ex: Herpes virus, CMV, EBV

Among these viral hepatitis, only hepatitis B virus andhepatitis C virus will cause chronic hepatitis (i.e: both theviruses will persist in our body for more than 6 months)and will lead to liver fibrosis, cirrhosis and liver cancer.

Hepatitis B virus:

WHO data shows that in India, 40 million people areHbsAg carrier (i.e: hepatitis B virus in inactive stage) andpotential source of HBV infection. Approximately 1,00,000death annualy due to HBV related liver disease.

� HBV is a DNA virus. It transmits throughcontaminated needle, blood & blood products, unsafesex and from mother to baby.

� Incubation period: 30 to 180 days.

� In newborn or children, it will be as non-icteric stageas body immune system is not well developed, henceno liver cell inflammation irrespective of viremia(immune-tolerant stage).

� In adults, it will cause icterus because of hepaticinflammation due to well developed body immune-system.

� These group of patients requires oral anti-viral drugs for life-long and needs regularsurveillance (i.e: Once a year – HBV DNAquantification assay, USG abdomen, Alfa-feto protein) to rule out anti-viral drugresistance as well as hepatocellularcarcinoma.

HBV related cirrhosis:

� If it is compensated cirrhosis (i.e: noperipheral signs of liver cell failure such asjaundice, pedal edema, ascites &encephalopathy), needs to continue oral anti-viral drugs. More over, needs endoscopyscreening for esophageal varices and CTabdomen triphasic study for hepatocellularcarcinoma screening.

� If it is decompensated cirrhosis, apart fromoral anti-viral drug needs screening forvarices and HCC. More over, needsendoscopy screening for esophageal varicesand CT abdomen triphasic study forhepatocellular carcinoma screening.

Oral anti-viral drugs:

� First line drugs: Tenofovir 300mg once a day

Entecavir 0.5mg once a day

� During pregnancy & lactating period:

Tenofovir 300mg once a day

� In renal failure or on hemodialysis:

Entecavir 0.5mg twice a week or afterhemodialysis

Hepatitis C virus:

Prevalence of HCV infection in India isestimated at between 0.5% to 1.5% (15 to 18million people).

� HCV is a RNA virus. Its transmission is alsosimilar to HBV transmission such ascontaminated needle, blood & bloodproducts, unsafe sex and from mother tobaby.

� Incubation period: 15 to 180 days.

Acute hepatitis C & chronic hepatitis C:

� Both acute and chronic infection mostly willbe as non-icteric phase.

� Unlike HBV, 85% of acute infection(hepatitis) will progress to chronic infection(chronic hepatitis) and will progress slowlyto fibrosis, cirrhosis & liver cancer.

� When anti-HCV comes positive, the requiredtests are liver function test, HCV RNAquantification, USG abdomen and alfa-fetoprotein.

� Irrespective of the HCV RNA viral load,patient should be treated by oral anti-viraldrugs.

HCV related cirrhosis:

� Velpatasvir 100mg or daclatasvir 60mg withSofusbuvir 400mg once a day with Ribavirin400mg twice a day for 3 months in cirrhoticpatients.

For esophageal varices:

If it is large esophageal varices, needs repeatedendoscopic variceal band ligation (EVL) toeradicate esophageal varices with beta blocker(Ex: Carvedilol).

We had our NHB District Coordinators meetingon 16 June 2018 at JVL Plaza and on 17 june2018 wehad our 116th Nursing Home Board North Zone Meetat Hyatt Regency, Teyanampet, Chennai. There werelot of discussions and Team IMA 2018 took lot ofresolutions based on Brain Storming discussions.

Dr.V.Saroja,Honorable Minister of Social Welfarepresided as the Chief Guest and assured her supportin finding suitable solutions. President Dr.J.A.Jayalallaunched our NHB App and is now available both inAndroid and Apple in the name of (TN NHB) whichwill be usable for member doctors who have registeredtheir mobile numbers.

It was decided to lobby for revision committeefor the minimum wages till July 15th and havesubsequent discussions with our office bearers andcoordinators regarding plan of action.

The new Clinical Establishment Act copy washanded over by Dr.Muthurajan to our President andoffice bearers, and IMA profusely thanked ourGovernment especially Honourable Health Ministerand Health Secretary for their support.

There was an interactive session on insuranceand Dr.A.K.Ravikumar presented the methodologyof costing analysis and the arrived costing, whichwas well appreciated.

Dr.Prakash (CEO Star health insurance) assuredthat in future handling insurance in small & mediumhospitals will be promoted in a big way and they

need not worry or close their hospitals and insteadthey can upgrade their standards to effectively handlethe insurances.Dr.Selvanayagam( CMCHIS) alsoagreed upon to look into tariff revision based oncostings.

It was also decided by the house that after thecosting analysis report prepared byDr.A.K.Ravikumar is passed by IMA,all memberhospitals have to quote that tariff as minimum chargesand hospitals failing to do so will not be eligible forthe benefits of membership of IMA and it’s scheme’s.

On discussion on Fire Safety it was decided thathospitals should go for adequate fire fightinginfrastructure wherever possible. Mere lack of rampwill not be accepted as criteria for closure of a hospital,provided they have fire fighting infrastructure.

IMA is in the process of doing barcoding forbiomedical waste and details will be circulated shortly.

More than 100 hospitals have applied for NABHby NHB and is progressing well.

We thank all the participating coordinators andmember hospitals which makes Team IMA 2018 actdemocratically hearing voices of members.

I request those who are not attending to kindlycome, give suggestions and take part in crusadingsolutions for our crisis situation.

Greetings from PPLSSS.

The Management Committee Meeting was held at Yercaud atShevaroys Hotel. Majority of the members have attended and someof the District Coordinators who have not attended the two meetingsalso have attended. Dear members, the name change of the document

of Javial Plaza Guest House, which was a long bending issue, has almost beensettled with the corporation office and the missing document was also obtained.Some complaints regarding the Guest Houses were also settled and also thewaste materials were disposed.

I request all the members to use the swiping machine in the Guest Housesto avoid malpractice in dealing with direct cash transaction. The Family BenefitScheme is going well and some members were confused with the proposedFamily Benefit Scheme of FSS. Both are different and there is no age limit inour scheme.

Our State President Dr.J.A.Jayalal has planned to have the next managementcommittee meeting at Andaman and requests all the District Coordinatorspositively to attend the meeting. Feedback from our members are welcome forimproving the facilities and for the growth of the scheme.

Some of the cases which we are very sure that we will not win, its betternot to go for the appeal as there may be more compensation. Renewal ofMembers should be encouraged by the District Coordinators and branch officers.

The answer to the question may be ‘yes’ and‘no’ as will be seen from the following case laws.

Even if a doctor or a surgeon does not havethe specialist’s / superspecialist’s qualifications,he is considered competent to treat patients orconduct specialist surgeries on the basis of histraining and experience. It will bepertinent to quote a few examples:

The Tamil Nadu State

(Gen. Surgery) doctor performing cardiac surgeryon the basis of his experience and frequent studyof latest techniques by going abroad.

An M.B.B.S doctor having obtained degreefrom the University was competent to practicemedicine, surgery and obstetrics. Caesareanoperation is part of the surgery. It may be that thepersons obtaining diploma like D.G.O may be morequalified to conduct caesarean operation, but it

qualified as well as eligible to conduct caesareanoperation, on the basis of her experience also.
