Blood tracking to ensureright patient, right blood

Case Study: Mayday HealthcareNHS Trust


Requirements

Mayday are running a ‘Right Patient, Right Blood’ project;the purpose of this is to provide equipment to helpensure that patients are tracked through the process ofblood transfusion and that the right patient is given theright blood. The tracking process uses patient wristbandsto hold patient information in Human Readable form, in1D (linear) and 2D barcodes, as well as containing HighFrequency programmable RFID tags. At any point oftreatment, the patient’s wristband will be scanned toconfirm their identity.

Existing wristband printers would be replaced with newprinters which are capable of printing the wristbands to ahigh quality and at the same time writing information tothe RFID chip on the wristband.

In terms of RFID equipment, this would be a relativelylarge deployment and an important project from theperspective of equipment suppliers.

Existing printers receive information from the PatientAdministration System via the network, and the newprinters must plug-in to the network and receive similardata without significant change to the PAS system. Eachwristband must contain unique identification for thepatient, and this information is used to link in with otherblood-tracking systems. The 2D barcode must replicatethe information contained in the RFID chip, and in thisway there will be redundancy to the data on thewristband : it could sustain damage to the 2D codeprinted or to the RFID chip, and the information couldstill be extracted.

A 1D barcode was required for use by consultants aroundthe site to link into other information systems and, of

course, normal printed (‘Human Readable’) information isalso placed onto the label according to the agreed layout.Depending upon the information transmitted by PAS, theinformation on the wristbands would vary.

There were 2 types of wristbands, Adult and Child,utilising different materials and of different dimensions;both as agreed by NHS Connecting for Health (arequirement primarily driven by the National PatientSafety Agency).

Where patients have to undergo MRI treatment, the RFIDwristband needed to be removed and a ‘Skin Label’printed and produced. The RFID wristband could not beworn into such an environment so a paper-only label hadto be produced temporarily.

There was also a requirement that the information storedwithin the RFID tag should confirm to GDTI-113, astandard defined by GS1 UK. This determined a datastructure in which documents (of which the MaydayPatient Wristband was one kind) identified globallymerely by reading the GDTI-113 number.

The Solution

The approach was to utilise ZebraR2844-Z printers whichcontain an embeddednetwork interface card andRFID transponder alongwith the Zebra BasicInterpreter.



By incorporating all these components, a single printerdevice could contain and run a custom-writtenapplication to listen for incoming network data fromPAS, decipher the information, make variouscomputations, then write (with write-protect followedby read-back validation) the RFID chip before printingand advancing the wristband.

This write and read-back method guaranteed that theRFID chip on a wristband being printed did indeedcontain precisely the information which had beenwritten and printed. Any faulty RFID wristbands wouldbe printed as VOID before limited re-prints werepermitted.

Each HF RFID chip contained a unique 64 bit TagIdentity (MAC, or Media Access Control address) andthis would be used as the basis for uniqueness, passingthe 64 bit value into some computational logic toproduce a GDTI-113 compliant number.

For the Skin Label printers, a variation of theapplication was deployed to Zebra LP-2844 printerstogether with a 2D image scanner. By scanning awristband, the imager sent the 2D barcode data to theLP-2844 printer, and a new skin label was produced toreplicate precisely the patient and barcode informationfrom the wristband.

A number of technical challenges were encounteredduring the project. Firstly, the timescales wereextremely tight and the detailed requirements requiredsome refinement before implementation couldcommence.

Initially there did not appear to be sufficient spacewithin the 896 bit chip, and critically there was noformally ratified standard for GDTI-113 within HF RFIDtags. Also, the 1D barcode would not readily fit across

the width of the wristband in all cases, whilst at thesame time being readable to the equipment.

Issues were encountered with sensors in the printersnot reliably seeing the markings on the wristbands, andthis was especially problematic on the ChildWristbands, being of a different type of material and awith calibration marks printed differently.

By steadily analysing the issue and working with anumber of relevant teams, the cause of each of theseproblems was identified and a solution wasimplemented. The GDTI-113 requirements were clarifiedin great detail and the codes computed and written tological positions within the tag. The 1D barcode could bereduced by several redundant characters withoutimpact in function and could therefore beaccommodated across the label. Modifications to theR2844-Z printer sensor location allowed the childwristbands to be handled by the printer satisfactorily.

Benefits

Overall, The Barcode Warehouse was able tounderstand the requirement in great detail, to see the“big picture” of the technical challenges, towork with the client and client’spartners and suppliers, toultimately ensure that a workablesolution was provided in good time.

The SolutionThe development team at The Barcode Warehousedeveloped an Electronic Casenote Tracking system,which it named ‘pinpoint’.

Pinpoint allows casenotes to be accurately trackedwithin the library and around the hospital complex. Aunique barcode label is attached to each set ofcasenotes. The Casenote tracking system iselectronically linked to the PAS (Patient AdministrationSystem) clinic diary.

Casenotes are pulled for clinics with the aid of handheldcomputer scanners. The barcode on the casenotes arescanned providing the casenote tracking system withthe location. Using the tracking system, the location ofcasenotes can be quickly identified at the touch of abutton whenever necessary.
