4 Year Cohort Graduation Rate is the percentage of students who begin at PHS as 9th grade students and graduate in four years.
Definition of term:

Currently 297 (47.1%) students still at PHS.

Step 2: Problem Analysis CLASS of 2012 Tier 2 - 9.7% of current 4-year cohort
students have not passed English CAHSEE (excluding students with I.E.P.s)
Tier 2 - 9.0% of current 4-year cohort students have not passed Math CAHSEE (excluding students with I.E.P.s)
Tier 3 - 15.1% of current 4-year cohort students need more than 70 credits to graduate in June

Step 3: Intervention Design and Implementation
Tier 1 – Teachers of these students continue to support students in current classes. Counselors will continue to monitor students grades in classes needed to graduate in June 2012.
Tier 2 –
Identify (39) students who need support to pass one or both parts of CAHSEE and offer CAHSEE Boot Camp
Identify (33) students who need will lack 5-10 credits in order to graduate in June 2012 and get them enrolled in Adult School
Tier 3 – Counselors identify (45) students who need more than 2 classes (10 credits) and find other alternatives programs for graduation.

Evaluation
Step 4: Evaluation In May 2012, look at number of
students who passed CAHSEE in February and/or March in order to graduate in June 2012.
In June 2012, determine how many of the students in Tier 2 (students needing 1 or 2 additional classes) were able to receive the necessary credits to receive their diploma.
In June 2012, determine how many students that were transferred to other programs met the requirements for a diploma.
Evaluation
