A joint project between NHS Connecting for Health, Prison

Health and HMPS

Strategic Context

• Transfer of budgetary responsibility to DoH from Home Office

• From April 2006 PCTs will be responsible for commissioning healthcare for prisoners

• Reorganisation of Prison and Probation Service- NOMS

• National Service Frameworks and MHMDS• NHS Connecting for Health

Present healthcare IT information

• Paper based system • HMPS has a contract with EDS to provide a

prison intranet and basic desktop software• Does not include healthcare• Runs over NT3 due to upgrade to XP next year• Few NHS net connections• A number of prisons have stand alone primary

care systems as used in the NHS

Facts

• 138 Prisons in England and Wales• 12 Private Prisons + 1 new one due to open

next spring• 400,000 movements a year• 74 000 Prisoners• 68 500 male, 5 500 female• 4 Categories of prison• Separate Women's estate• Ages 16 – 85+ (HMP Kingston)

Health Care Activity

• £100 million approximately• Throughput 250 000 persons per year• Male prisoners consult doctors10 times

per year (685 000), other health staff 20 times per year (1,370,000)

• Female prisoners consult doctors 20 times per year (90 000), other health staff 60 times per year (270 000)

• 9 600 inpatients admitted per year

Health Problems of Prisoners

• Mainly healthy young males

• High incidence of drug and alcohol misuse

• A number of studies have shown high incidence of mental health problems

• Problems of cervical screen follow up

• Blood borne diseases Hep B and Aids

• Usual incidence of diabetes and asthma

Prison Health-where are we now

• Prison health IT is part of NHS Connecting for Health, funding has been agreed

• An agreed strategic statement has been circulated to both prisons and PCT’s

• NHSnet issues resolved now prison health is an agreed part of the NHS

• Authorised Service Recipient to use the NHS Care Record Service

• An agreed OBC• Completed training needs assessment

Prison Health-IT next steps

• Update the OBC to include Welsh and contracted out prisons, and to clarify delivery alternatives

• Set up an Specialist Advisory and Information Governance groups to support and advise the Board

• NHS Connecting for Health team to be in place in May

• Agree a project plan• Set up a PRINCE board• Agree system architecture

Prison Health-IT timetable so far

• Update OBC by end of February 05

• Release management summary of present position in May 05

• Consult with the Professions on User Requirements-March/April

• Longer term-NOMS IT integration

Current position• The Department of Health, HM Prison Service and NHS Connecting

for Health have developed a joint work programme to deliver a first class information system for all healthcare professionals working within the prison service.

• A statement of user requirements has been developed and is currently being consulted upon. Whilst a delivery mechanism for the new systems has not been formally agreed, it is intended that this statement will form the basis of a national specification against which clinical systems will be procured

• Any local procurements can only be considered where clinical care is being compromised because of the lack of an IT system. All purchases should be with the consent of the relevant Strategic Health Authority’s Chief Information Officer and will be subject to future conformance testing
