Dr Chopra is Professor of Medicine and Faculty Dean for Continuing Education at Harvard Medical School and Senior Consultant in Hepatology at the Beth Israel Deaconess Medical Center, Boston. He is Editor-in-Chief of the Hepatology Section of UpToDate and has more than 120 publications and six books to his credit. The recipient of numerous awards in the past 25 years, including the Excellence in Teaching Award and the Distinguished Educator Award, Dr Chopra was elected in 2009 as a Master of the American College of Physicians, an honor bestowed upon “citizen physicians, educational innovators, scientific thinkers, and humanists who inspire and set the standards for quality in medicine.” Dr Chopra serves as Course Director for several CME courses, including seven annual Current Clinical Issues in Primary Care. His most recent books include Dr. Chopra Says: Medical Facts and Myths Everyone Should Know, which has been translated into multiple languages, Live Better, Live Longer. The New Studies That Reveal What’s Really Good and Bad for Your Health, and Leadership by Example: The Ten Key Principles of All Great Leaders.

Dr Barker has devoted his career to the training of general internists in the residency program at the Johns Hopkins Bayview Medical Center. He and his colleagues edited the comprehensive textbook Principles of Ambulatory Medicine, now in its seventh edition (2007).

Drs Chopra and Aronson will cover the greatest discoveries and scientific advances in the last 100 years. All scientific breakthroughs come in fits and starts. The stories behind the discovery of drugs like penicillin and statins; the discovery of vaccines for polio and hepatitis B; the control of HIV, the greatest of all plagues, with HAART therapy; the cure of diseases such as hepatitis C; the history of how the birth control pill came about; and advances in cardiology such as the development of the pacemaker for complete heart block have not only changed medicine forever but make for fascinating tales that combine serendipity with brilliant science.

at the intersection of Jazz and Medicine: improvisation in the Clinical Encounter

Paul M. Haidet, MD, MPH General Internist Hershey, Pennsylvania

Dr Haidet is a general internist in Hershey, Pennsylvania. He completed residency at the Penn State Milton S. Hershey Medical Center and fellowship at the Beth Israel Hospital and Harvard School of Public Health. He was the 2011 president of the American Academy on Communication in Healthcare and has published widely on topics related to relationships in health care. He has been incorporating his lifelong love of jazz into his research and teaching work, exploring the improvisational aspects of medical communication.

Physicians encounter uncertainty, complexity, and change, particularly in the medical encounter. Strictly following communication algorithms (e.g., course, onset, duration, etc) does not provide physicians with the latitude to explore critical aspects of patients’ unique illness narratives and perspectives. On the other hand, conducting the medical interview in a completely unstructured fashion is inefficient and chaotic. Physicians need to improvise, effectively managing this tension between structure and freedom. In this session, Dr Haidet will use examples from the world of jazz to illustrate several improvisational concepts that may translate to the world of doctors and patients.

warfarin, aspirin, Dabigatran, and others: How are you to Decide in the Patient with atrial Fibrillation? This will be a discussion of different agents that can be used to reduce the risk of stroke in patients with atrial fibrillation. An attempt will be made to determine how information from clinical trials can be used in real world practice.

The Phone: Clinicians’ Friend or Foe? Top 5 advancesTelephone medicine, a major component of Telehealth, has transformed clinical primary care. This talk will discuss 5 advances that can most potentially impact clinical practice for both you and your patients. These include 1) the dramatic growth in After Hours Call Centers, 2) payment for telephone time, 3) quality Improvement-Risk Management efforts, 4) Telemedicine, and 5) the recent American Academy’s support of telephone medicine through the Section on Telephone Care. The goal of this talk is to encourage attendees to identify which of these advances can be incorporated into practice.

innovations in shared Medical Decision MakingShared decision making (SDM) is a collaborative process that allows you and your patients to make health care treatment decisions together, taking into account the best scientific evidence available as well as the patient’s values and preferences. This lecture explores how to engage in SDM, using specific clinical examples, and how to incorporate the process into a busy practice setting.

understanding the Medicare annual wellness visitsAs we enter the era of primary care redesign, we need to understand the building blocks used for the determination of productivity and compensation. In this presentation, Dr Goodson will discuss the health promotion and illness prevention services that we in primary care can employ with our patients, specifically focusing on the Medicare Annual Wellness service codes, the long-standing Welcome to Medicare service code, and the new post-discharge care reconciliation service code that will be available for Medicare patients in 2013.

bad air, yellow Jack, and Montezuma’s revenge: a Trip Through Travel MedicineChildren are traveling to foreign destinations with increasing frequency. In this talk, and based on over 15 years of experience as director of the Travel Clinic at Boston Children’s Hospital, Dr Malley will review the most frequent medical problems, up-to-date recommendations for vaccines and chemoprophylaxis, and commonsense preventative measures for pediatric travelers and their families.
