Preschool Audiometer Hearing Screening Procedure Purpose This explains how to complete hearing screenings for preschool children using an audiometer. Guidance All team members who perform hearing screenings must attend training. Hearing screenings are done for all preschool children within 45 days following the first day of attendance. Procedure Preparation 1. Select a screening date 2. Notify parents 3. Identify a quiet area with: o electrical outlet if needed o table or desk o chairs for screener and child 4. Demonstrate in the classroom what the children will be asked to do during the screening o Turn audiometer on to 80 – 100db. o Demonstrate the sounds at 500, 1000, 2000, 4000Hz. o Have children raise their hands when they hear sound. o Turn audiometer down to 20db and show how sound can’t be heard without earphones. o Demonstrate how to respond with earphones and raising your hand. o Have one or two children put on earphones and respond to sound. Screening 1. Turn audiometer on to 20db (If it has been stored in a cool location, allow it to warm up for 10 minutes before using.) 2. Adjust the earphones on each child. 3. Give impulses at 20db for 1000, 2000, and 4000Hz. Use 25 db for 500Hz. 4. When the child responds two out of three times for each frequency in each ear, consider this as “pass”. If they do not, consider this a “rescreen”. Recording 1. Record the results on the Preschool Health Screening Results Form 2. Send the original Preschool Health Screening Results Form to parent and file a copy o Head Start: Record results on the vision/hearing data entry worksheet and fax to the Early Learning Health Information Line 888-979-5897 or Scan/Email to [email protected]. o ECEAP: record in ELMS (ELMS entry instructions follow). 3. Head Start: Save the data entry worksheet. As new children are screened, add their names and results to the worksheet and fax to PSESD Re-Screening 1. Notify Sr. Health Coordinators of any child who needs to be rescreened. Spell their name and give the date of birth and the date of the initial screening. 2. 3. The Health Coordinator will arrange to rescreen the child using the Otoacoustic Emissions (OAE) hearing screening equipment, within 2-4 weeks of the initial Audiometer screening date.

4. If a child is unable to pass the first OAE screening, the Health Coordinator may refer them to their Primary Health Care Provider (PHCP) for a middle ear evaluation.

5. Staff will inform the Health Coordinator when the child has been seen by their PHCP 6. A second OAE screening must occur after the child has seen their PHCP and has finished treatment, if

needed. If a child is unable to pass the second OAE screening, a referral is made by the Sr. Health Coordinator back to the PHCP for a follow-up evaluation by a Pediatric Audiologist.

7. Notify Health Coordinator of results of Audiologist visit. 8. Record and file all screening results, parent/guardian communications, and follow up notes.

Preschool Audiometer Hearing Screening Procedure For ECEAP – How to Enter Hearing Screening Results in ELMS for a Single Child

1. Log into ELMS and click on the Child tab. This will take you to the Child Search screen.

2. Type the child’s name in the search box.

3. Click the Search button. The results will appear underneath.

4. Scroll down and find the child’s name that you need, then select Child/Family Updates to the right.

5. You will see the child’s name at the top of the screen. Underneath is a row of tabs corresponding to different service areas.

6. Click on the Health Screening tab.

7. Scroll down to the Hearing sub-heading to enter the child’s screening results –

a. Select the provider who did the screening.

b. Enter the date of the screening.

c. Select the screening results from the dropdown box.

8. Click Save in the top or bottom right corners of the screen.

Preschool Audiometer Hearing Screening Procedure For ECEAP – How to Enter Hearing Screening Results for a Whole Class Using Bulk Update

Bulk Update is an ELMS feature that lets you enter screening results for a whole class at one time. You can use Bulk Update to enter vision, hearing, and growth/BMI data. To enter hearing screening results using Bulk Update -

1. Go to the Child tab in ELMS and select Bulk Update.

5. In the Hearing Screening column, enter each child’s screening results in their row.

a. Enter the date of the screening in the Screening Date column

b. Select the result of the hearing screening from the dropdown box in the Result column.

6. Click the Save button in the bottom left corner of the screen.

2. Select your class from the dropdown box.

3. Click Search. Children’s names will appear below in spreadsheet format.

4. Check the boxes to hide columns you don’t want to see. In the example below, the Vision and BMI columns are hidden.
