PRACTICE COORDINATOR: HEALTH & NDIS INTERFACE

PURPOSEThis position will maintain, strengthen and build relationships and skills within discharge planning teams across Victoria with the evolving NDIS environment. It will influence positive hospital discharge outcomes and increase the capacity of health professionals in working towards sustainable housing opportunities for people with disability and complex support needs.
WORKS WITHAll Summer Foundation teams, particularly with our Communications, Policy and Research teams in identifying individuals and families who are willing to participate in wider Summer Foundation activities.
Working within the Practice team and closely with the Practice team leader for health and aged care, in delivery of project and program goals. ABOUT THE TEAMThe practice team aims to improve the interface between healthcare, aged care and the NDIS by: Working to ensure people with
disability have the right supports in place so they can live well in the community
Prototyping practical solutions for younger people with complex support needs,
Creating opportunities for improvements in the NDIS through the development of information and resources based on practice evidence
Increasing workforce capacity across sectors working with younger people with complex
support needs who are eligible for or participants in the NDIS
Achieving better discharge outcomes for younger people with high support needs
ABOUT THE ROLEThis role will: Deliver training, mentoring and
consultancy advice to the discharge planning workforce across Victoria
Conduct training evaluation and contribute to project documentation and reporting deliverables
Contribution to the Leaving Hospital Well: Community of Practice
Assist and support the roll-out of the Collaborative Discharge Approach within an additional LHN in Victoria
Support the development of a secondary consultation framework and delivery for Health services across Victoria
Contribute to development of written information and resources for presentation and dissemination across health networks
QUALIFICATIONSAn undergraduate allied health degree such as speech pathology, neuropsychology, occupational therapy, psychology, social work or equivalent; or equivalent experience
ESSENTIAL CRITERIA Experience working directly with
people with disability who have complex and high support needs, particularly those who are in hospital, and/or at risk of entry to RAC
A working knowledge of the NDIS, its framework, structure and practice
Demonstrated capacity to engage with allied health and clinical care staff, particularly those involved in hospital discharge practice with the NDIS
Advanced problem-solving skills, including a pro-active approach to individual problems, through to systemic advocacy.
Demonstrated capacity in training and mentoring other professionals in the area of disability/health.
Evidence of ability to work collaboratively in a team environment.
Professional presentation and excellent communication skills.
OTHER REQUIREMENTS Willingness to travel interstate
and stay overnight where required
Current police check. Current drivers licence and own
vehicle, or equivalent capacity to travel.
