The scheme works in partnership with health professionals in Swansea that include local GPs, practice nurses, dieticians, physiotherapists, diabetic nurses, psychologists and occupational therapists among others. The scheme is aimed at people who: Are aged 16 years and above Are currently inactive Are overweight or clinically obese Have specific health conditions The referral instructors: Offer one to one assessments Are highly qualified professional staff Provide community based sessions Give advice and guidance on increasing participation in physical activity The referral scheme lasts for a minimum of 4 weeks up to maximum of 16 weeks and is delivered across our Active Swansea Leisure Centres and a variety of community buildings throughout Swansea. How do I join the scheme?Firstly, you must be referred by a healthprofessional.

Members of the exercise referral team willthen assess each individual and establishwhether you are suitable for thisparticular scheme.

What does the programmeconsist of?Participants who are referred ontothe scheme will get a chance to takepart in a range of activities whichinclude chair based sessions, fitnessclasses, Nordic walking and gymsessions.

These classes last up to an hour(approximately) and are delivered in small groups of people.

The long term aim of this programme is to ensure youcontinue to be physically active for the rest of your life.

What is the Cardiac Rehabilitation Scheme?The Cardiac Rehabilitation Scheme is aimed at people who have

experienced a cardiac event within the last 6 months orthose who have been referred from the cardiac team

in Morriston or Singleton Hospital.

Phase 4 of the Cardiac Rehabilitation Schemeprovides participants with specific exerciseclasses, gym sessions and/or walks as wellas general advice on health and well-being.All sessions are run from a range ofcommunity buildings across Swansea.

How do I join the scheme?You must be referred by the cardiac team

either in Morriston or Singleton Hospital.

Members of the exercise referralteam will then assess you andestablish whether you are suitablefor this particular scheme.

What does the programmeconsist of?A tailored exercise programme based onyour medical condition and general healthand fitness will be put together, which buildson the work completed in Phase 3.

The scheme lasts a minimum of 4weeks up to a maximum of 12 weeks.After those 12 weeks you can go onto the National Exercise ReferralScheme to continue the programmefor a further 16 weeks.

The long term aim of this programmeis to ensure you continue to bephysically active for the rest of your life.

What is the Falls Prevention Scheme?The Falls Prevention Scheme is aimed at those who have been referred because of:

� a fall � difficulty with balance and general mobility � a risk of falling or� anxiety about falling

What does the programme consist of?A tailored exercise programme based on theindividual’s medical condition and generalhealth and fitness will be put together.

The scheme lasts for 12 weeks and aims toassist individuals through a dedicatedprogramme of activities, mainly chairbased, to assist those who have fallenand to prevent any further falls.

After those 12 weeks participants willthen have the opportunity to take partin an exit class. The exit class will buildon skills and practices developed in theprevious 12 weeks and will help theindividual improve their functional abilityand quality of life.
