2017 Findings

• Data coverage: estimated 93% of PSS member workforce. 14974 direct employees and

2393 indirectly employed (16907 direct employees in 2016 and an unknown number of

indirect employees). Responses from 59 member companies covering approx. 161 ports.

• 1 fatality (3rd party/indirectly employed) reported in 2017 (1 in 2016 and 4 in 2015)

• 40 Specified Injuries have been reported (40 in 2016 and 36 in 2015)

• 206 Over-3-Day injuries (258 in 2016 and 246 in 2015) of which 147 were Over-7-Day

injuries (compared with 188 in 2016 and 178 in 2015)

• 71 One-to-3-Day injuries (70 in 2016 and 103 in 2015)

• 318 total injuries in the report (370 in 2016 and 388 in 2015)

• 1 Industrial Disease case reported (1 in 2016 and 2 in 2015)

• 1 Case workplace stress reported (4 cases reported by 1 member in 2016)

• Accident Incidence Rate per 100 members of workforce 1.44 (1.77 in 2016 and 1.65 in

2015) down 15.9%.

• Accident Incidence Rate per 100 direct employees 1.33 (1.38 in 2016 and 1.34 in 2015)

up 7.5% (N.B this is affected by reclassification of some persons as indirect employees in

2017 from direct employees in 2016)

• Lost Time Injury Frequency RateA for Direct Employees 9.40 (9.13 in 2016 and 9.12 in

2015) up 2.9% (Adjusted for some members who do not yet report non-reportable LTI)B

• 63 Lost Time Injuries have been reported to indirectly employed persons and 3rd parties

(79 in 2016 and 71 in 2015).

• Lost Time Injury Rate per million hours worked by Operation Type - Provision of Cargo

Handling Personnel 16.6 (13.1 in 2016 and 24.7 in 2015); RoRo 15.8 (18.6 in 2016 and

15.8 in 2015); Container Operations 15.3 (16.8 in 2016 and 16.9 in 2015) produced the

highest lost time rates

• In previous years we have reported that the top 3 accident locations are, in order: Ships;

Quayside and Cargo Handling Equipment - accounting for 62% of all injuries reported in

2016. For 2017 the location data was split furtherC and now shows more specific locations.

The top 5 locations in 2017 which also have a similar frequency are: Berths/Quays,

Ships/Craft, Roadways, Open Storage areas and on Cargo Handling Equipment.

Container ships remain the most likely vessels on which to have an accident.

• The top 6 reportable accident categories for LTIs in order were: Slips Trips and Falls on

same level; Hit by a Moving, Flying, Falling Object, Driving Related incidents; Lifting,

Carrying and Handling; Hit Something Fixed or Stationary and Fall from Height. These

accounted for 90% of all accidents.

Appendix 3 – Reportable injury rate by

Employee Type (+7day)

For the first time in 2014, injuries were separated into those occurring to Direct employees, Indirect

employees and 3rd Parties. Additionally, an estimate of total hours worked was recorded. As this

is a relatively new development, there are insufficient years’ data to establish a trend. What is clear

is that the substantial majority of lost time injuries that ports are aware of, are those happening to

their own employees.

Time Injuries

22 organisations declared zero Lost Time Injuries in 2017 a lower figure than previous years.

However, as this figure included one major employer, the percentage of the workforce working in

companies with zero LTI was 12.7% - higher than previously.

Appendix 5 - Long Term Trends (+3day)

It is reasonable to compare incident rates through from 2000 to 2017 because of the relatively

similar collection process employed by PSS throughout the period. The Accident Incidence Rate

per 100 employees has fallen by 63% over the last 17 years, going from 3.9 to 1.44.

Appendix 6 - Lost Time Injury Frequency

The HSE identify two formulas for injury rates: incidence and frequency.

• An INCIDENCE rate gives injuries per set number of employees, normally 100,000. In our

case we use ‘per 100 employees’ because of the relatively small population size.

• A FREQUENCY rate gives injuries per million hours worked; approximately the number of

hours an individual works in a 45 year working life, based on a 40-hour week.

Appendix 7 - Accident Locations

The top named locations for accidents are led by Berth/Quay (including open storage areas) 40%,

Ship and Craft 18% and Roadway 13%. Consistent with previous years, the significant majority of

all reportable accidents occurred in traditional port operational/cargo handling areas. Also

consistent with previous years, Container vessels are the most likely ship type on which to have an

accident.

Although it is important to protect employees and others in non-cargo areas the emphasis clearly

must remain on cargo handling areas.

The “top 6” categories for reportable accidents are the same as previous years, though the order

is slightly changed. Essentially ports still need to push down on slips, trips, and falls on level, hit

by falling/flying objects, driving, lifting and carrying; running into things and falls from height. These

Severity

The majority, (98%l) of respondents gave a return to work date for each injury to direct employees.

The number days lost gives a severity for each accident category. 2015 showed a significant

reduction over 2014 but since then the days lost per injury appears to be more or less static.

Slips, trips and falls on same level continue to be the biggest origin of days lost. Injured whilst

handling, lifting or carrying has dropped noticeably in 2017. Hit by moving, flying or falling object

and Driving related incidents continue to figure highly. However, if looking at average days lost

then Fall from height and Contact with moving machinery /material being machined gave rise to

longer absences before return to normal work.

Exposed to harmful substance was 1 incident and whilst it has the highest average days lost for

2017 it is not statistically significant at this stage.

Reportable diseases remain low. Only specific diseases are reportable under RIDDOR:

• Carpal Tunnel Syndrome: where the person’s work involves regular use of percussive or

vibrating tools

• Cramp of the hand or forearm: where the person’s work involves prolonged periods of

repetitive movement of the fingers, hand or arm

• Occupational dermatitis: where the person’s work involves significant or regular exposure

to a known skin sensitiser or irritant

• Hand Arm Vibration Syndrome: where the person’s work involves regular use of

percussive or vibrating tools, or holding materials subject to percussive processes, or

processes causing vibration

• Occupational asthma: where the person’s work involves significant or regular exposure to

a known respiratory sensitiser

• Tendonitis or tenosynovitis: in the hand or forearm, where the person’s work is physically

demanding and involves frequent, repetitive movements

The low numbers do naturally raise question of potential under reporting. However, reporting is a

statutory duty. The dutyholder would be in breach of the regulations if they failed to report. It is

clear from the appendices above, that dutyholders are reporting lost time injuries; it makes little

sense that they would not also do the same for diseases. It is the case that effective risk

assessment, hazard elimination or control, alongside well planned and executed health monitoring

and early intervention should prevent these diseases occurring in the modern workplace. The low

numbers are more likely to be the result of effective management than of under reporting.

It is worth noting that the methodology is not currently set up to capture any industrial diseases and

health impacts other than those specified in RIDDOR. Neither is it collecting data on mental health

(e.g. stress) effectively. There is a need to review the methodology for assessing the state of health

and mental health in the port sector. PSS has other work on-going in this area but it is at an early

stage. For the time being, this accident statistics process will continue to focus on

injuries/accidents.

Appendix 12 - Data collection

Background

This report has been produced by PSS on behalf of its members and is made available to

subscribing members and other stakeholders on a confidential basis.

The main purpose of these statistics is to draw lessons from the incidents that have occurred in

2017 to further improve health and safety in the UK port industry. This is a report of incidents from

PSS members that required a formal report to the HSE under RIDDOR regulations and other

injuries where more than 1 day or shift was lost by the injured party.

Source Population

The 2017 data is based on returns covering approximately 14974 direct employees. An attempt

has been made to quantify indirectly employed persons but to date the result does give a

statistically robust figure.

Method of Collation

• Figures are collated for January to December inclusive

• The report form, based all injuries where the injured party has lost a minimum of 1 day (or

shift) of work, plus other RIDDOR reportable incidents, is completed and returned to PSS

• The definition of “employee” includes any person for whom an accident report would be

completed and submitted to the HSE by the company if that person were to be injured at

work. This may include employees on a shorter number of hours, such as cleaners, and

those who are not permanently employed by the company

• “Accidents” include accidents to non-employees that the port employer has a responsibility

to report e.g. if a member of the public is killed or taken to hospital and accidents to

contractors under the management control of the port.

Comparison with data from other sources

The statistical data in this report relates to members of PSS. Whilst it cannot be assumed that reflect the situation in the whole UK ports industry. PSS membership includes around 95% of the UK port industry by tonnage handled as well as many fishing and leisure harbours.

Industry accident statistics are also collated by the Department for Transport (DfT) and Health and

Safety Executive (HSE), although the basis and definitions for inclusion in each of the schemes

vary. The important point is that, over recent years, all the industry figures have demonstrated a

marked downward trend.

Except where stated, PSS statistics are calculated on a ‘per 100 average employees’ basis. Where

injuries to Contractors have been included this is recognised as an inconsistent measure, as

different ports and operations will have different levels of non-permanent employee utilisation and

this is not directly comparable with other industries. Direct comparison with PSS statistics prior to

1997, or with the accident statistics produced for other industries, should be treated with caution,

not least due to any differences in collation and reporting practices of other accident statistics

providers.

Reporting Regulation Changes in 2012

Under the Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995 there

were three types of reportable accident: Fatal, Major and Over-3-Day. The Over-3-Day category

required reporting of instances where the person was unable to carry out their normal duties for a

period of more than three days as the result of an accident in the workplace.

With effect from 6 April 2012 the Over-3-Day category changed to Over-7-Day1.

For valid comparison with historical data, this report mainly shows figures based on the old criteria

‘Over-3-Day’ incidents for all of January to December 2013. However, on occasion, a table will

show figures under the new criteria Over-7-Days criteria

A A revised data collection system was implemented in 2014. The main change was to ask for data on all Lost Time Injuries (LTI) – i.e. all absences for 1 day or over. Previously this had been restricted to only RIDDOR Reportable injuries. The collection of LTI data, along with an estimate of hours worked allows the calculation of a Lost Time Injury Frequency Rate (LTIFR) for Direct Employees. Essentially this is “Lost Time Injuries per Million Hours worked” and it can be used by members for bench-marking. Please contact us if you want to have your LTIFR calculated. B It has been found that a few members have been including some “Agency/Indirect Workers” in their Directly Employed figure. This has resulted in a mis-match for injury rate calculations in that if one of these workers suffered a LTI the injury would be classified as “Indirectly Employed” but the employee count allocated to “Direct Employees”. This has resulted in an understatement of Directly Employed LTIFR for previous years (likely range 5-10% too low). C The question relating to location of accidents has also been re written in 2017 to improve our ability to interrogate accident location in more detail and to reduce risk of misclassification of locations. D This is a new piece of data for 2017 and is an estimate. Members were asked to estimate numbers of Indirect Employees in the workforce. Although not all members have been able to do this we do have some data which has allowed us to be more accurate with accident rates, which will eventually lead to better bench-marking with other industries. Note the Indirectly employed figure includes those who were previously wrongly classified plus others, as this is not a question we have asked before.
