POLICY AND PLANNING ROTATION OBJECTIVES FORM Purpose of Rotation: In the course of the rotation residents will gain experience in assessing health needs of a population; investigating health issues, assessing sociopolitical realities and advocating the appropriate action to improve health; plan implement and evaluate health programs and/or strategies; contribute to the formulation of public policy; collaborate and build relationships; contribute to the body of knowledge of community medicine. Instructions: The form is to be completed, reviewed and signed off with your rotation supervisor before the start of each rotation. A signed copy is to be submitted to the Program Director no later than the first day of the rotation. Keep a copy for review at mid-unit evaluation and at final evaluation. At the end of each rotation, please complete the “Evidence of Achievement of Objectives” column of the form and review with your supervisor. Consider this information in the resident, faculty and site evaluations. Then, sign the form along with your supervisor and resubmit to your Program Director. Purpose of Rotation:

In the course of the rotation residents will gain experience in assessing health needs of a population; investigating health issues, assessing sociopolitical realities and advocating the appropriate action to improve health; plan implement and evaluate health programs and/or strategies; contribute to the formulation of public policy; collaborate and build relationships; contribute to the body of knowledge of community medicine.
