ADDENDUM #1, 02/17/17 -- ITB 17-01 2017 Street Resurfacing:

1) In the proposal the information on page 3 states that the 12.5mm Superpave will be placed at 151.25 lbs/sy. Would the City of Dunwoody consider changing the depth of the 12.5mm Superpave to 165 lbs/sy? Increasing the depth makes the mix more workable and decreases any dragging or tearing of the mix. RESPONSE: for Ashford Dunwoody Road, Change the 12.5 mm Superpave to 165 lbs/SYD.

2) On page 4 of the proposal it states that the streets in the Dunwoody Green Subdivision will be Edge Milled. These streets have only 19mm or B-Binder on them and no Topping. These streets will not need any Edge Milling. Please clarify. RESPONSE: Correct. No edge milling will be required in this subdivision (10 streets in Dunwoody Green S/D).

3) Will the contractor be required to use a MTV (Shuttle Buggy) when placing the 12.5mm

Superpave? RESPONSE: General Notes Item #11 (page 9) shall apply.

4) There is a 48 hour cover up on the milled surfaces, so will the contractor be required to

temporary stripe (paint) the milled surfaces? RESPONSE: In cases where milled roads (arterial and collector) are to remain open to traffic, temporary lane markings will be required. Tape may be used in these circumstances.

5) Chamblee Dunwoody Rd. has an existing asphalt ditchline, which will receive 2” milling.

Is the asphalt in the ditchline thick enough to receive the 2” of milling? RESPONSE: Core samples taken along Chamblee Dunwoody Road indicate pavement thicknesses in excess of 7 inches; therefore, we do not anticipate that milling 2 inches will present any problems.

6) Will the contractor be required to post mount the Advanced Warning Signs or can these

signs be placed on tripods? RESPONSE: tripods are acceptable.

7) Is was stated in the Pre Bid Meeting that all of the Residential Work needed to be

completed by the end of July 2017. Is this correct?

RESPONSE: Residential roads on the Primary Scope list shall be completed before School is back in session (August 7, 2017). If supplemental streets are added to contractor’s scope, a time extension will be negotiated based on the quantity of roads added.

8) On page 19 of the proposal there is a Bid Schedule for Unit Pricing – Add Alternates.

Would it be possible for Dunwoody to supply the contractors with quantities for each of the items on this Bid Schedule? This will allow all of the bidding contractors to price the same quantities. RESPONSE: Yes. A revised Bid Schedule is attached.

9) Would Dunwoody consider changing the items for the Striping and Pavement Marking

to unit prices instead of lump sum? When the striping subcontractors price striping at lump sum they have to cover for worst case, which makes the striping at a higher cost. RESPONSE: Yes. A revised Bid Schedule is attached.

10) Will the finished asphalt need to be flush with the curb?

RESPONSE: Ideally yes. However, on residential streets where existing pavement thickness are less than 2-1/2 inches, the contractor would be instructed to reduce the edge milling (say to ¾ inches below curb line) so the finished asphalt would extend up to ¾ inches above the curb in this instance.

11) The existing asphalt on several of these roads is above the curb, so if edge milling is required the contractor will need to make several passes with the milling machine to correct the slope of the road if the finished asphalt is to be flush with the curb. Will the contractor be paid for this additional milling?

RESPONSE: In these isolated instances where this condition occurs, any additional milling deemed necessary by the engineer will be paid at the contract unit price for variable depth milling.

12) Would Dunwoody consider giving the contractors a spread sheet with the tons of

patching, leveling, 9.5mm TP2, 12.5mm and square yards of milling on each road?

RESPONSE: This information will be provided to the successful bidder once the contract is awarded.

13) Will the contractor be required to mill/pave the turn lanes on the Arterials and Collectors? It looks like the quantities do not include the turn lanes. Please clarify.

RESPONSE: Yes. The quantities indicated on the Bid Schedule include the turn lanes. Prior to beginning construction on the arterials and collectors, the engineer / City will walk the site with the contractor to identify specific milling / paving limits.

14) Please clarify the quantities for the 9.5mm and 12.5mm. Based on the square yards and depths given in the proposal the 9.5mm should be +/- 6,767 tons and the 12.5mm should be +/- 10,586 tons. RESPONSE: The corrected quantities are reflected on the attached Bid Schedule. Provide unit pricing accordingly.

15) The quantity stated on the Bid Schedule for the 1.00” milling looks to be for the .75” milling and the quantity for the .75” milling looks to be for the 1.00” milling. Please clarify. RESPONSE: The corrected quantities are reflected on the attached Bid Schedule. Provide unit pricing accordingly.

16) The quantity stated in the Bid Schedule for the Variable Depth Milling looks to be for 100% of the square yards on the roads and not for the 6’ to 7’ wide Edge Milling. Please clarify. RESPONSE: Yes, we have corrected this error and the current quantities noted on the attached Bid Schedule will be used.

17) On page 3 in the proposal (Scope of Work) it calls for North Peachtree Rd. to be milled 1.5” minimum, but the quantity in the Bid Schedule does not look like it includes the North Peachtree Rd. quantity. Please clarify. RESPONSE: Yes, we have corrected this error and the current quantities noted on the attached Bid Schedule will be used.

18) It was stated in the Pre Bid meeting that the 12.5mm depth for Ashford Dunwoody Rd. should be 165 lbs/sy. Will the depth of the milling also be changed to 165 lbs/sy? RESPONSE: The depth of milling for Ashford Dunwoody Road shall be 1.5 inches as noted in the scope narrative.

19) Will the City be responsible for Mix Designs and Testing for the FDR? RESPONSE: Construction materials testing will be provided by the City. Contractor to submit Mix Designs for review and approval.

20) On the pay item it states FDR at 8” and Portland Cement at 10” but in the scope of work it states FDR at 10” and Portland Cement at 8”. We are assuming the city is wanting the roadway mixed 10” and then 2” milled and hauled off while stabilizing 8” depth. Will the City please clarify if this is the intent RESPONSE: Yes, soil base to be amended shall be 8 inches deep, which corresponds to 10 inches below the gutter line.

21) Will the City change the lump sum striping pay items into unit price pay items to ensure the City gets the best pricing. RESPONSE: Yes. See attached revised Bid Schedule

22) Are risers an acceptable adjustment for the manholes? RESPONSE: Risers are acceptable on Ashford Dunwoody Road, Meadow Lane and Chamblee Dunwoody Road. All other streets will require concrete collars.

23) On the pay item for FDR it states that Macbain Lane, Spalding Club Court and Wickford Way (Partial) will receive FDR but in the scope of work it states only Spalding Club Court and Wickford Way (Partial) receive FDR. Will the City please clarify. RESPONSE: Only Spalding Club Court and Wickford Way (partial) will receive FDR; Macbain Lane will be edge milled and overlaid as noted in the scope narrative. The Bid Schedule has been revised to reflect this revision.

24) Is tape acceptable for temp. striping on milled surfaces. RESPONSE: Yes.

For all streets the existing roadway materials should be mixed into the underlying subgrade using a pulvimixer. Access to the residences and businesses must be maintained at all times. The subgrade will be moisture cured for 7 days prior to paving.

General Notes: 1. Work is expected to begin in April 2017 and be completed within 150 calendar days.

The contractor and the City will conduct one (1) contract and pre-construction meeting shortly after award of the contract. The contractor and the City will discuss the terms of the contract as well as the schedule for the Work. The contractor shall submit a construction schedule to the city within 14 days following the meeting. The work is to be assigned to the contractor by geographical areas unless scheduling and coordination dictates otherwise.

2. The contractor is responsible for calling for utility locations prior to the start of each work. It shall be the contractor’s responsibility to coordinate his work with any utility owner whom may be in conflict with his work. No claims will be considered for extra compensation. Note: For full depth reclamation the bid form includes a per street unit cost to contract with a private utility locator.

3. Any item which must be removed during the construction work and is not specifically called for shall be removed by the contractor. The cost shall be included in other unit price bid. No claims will be considered for extra compensation.

4. The general descriptions of each item of work are as described in the Georgia

Department of Transportation’s standards and specifications (2013 Edition, and the applicable Special Provisions and Supplemental Specifications modifying the 2013 standard specifications), complete and accepted. Any conflicts which might occur during the course of the construction work or any conflicts which might be related to the compensation of any work shall be decided based on the Georgia DOT’s standards and specifications. No claims will be considered for extra compensation.

5. Upon the completion of each street, any excess items which might be left over from the

construction-related work, including asphalt pieces / excess aggregate, shall be removed or broomed off and disposed of properly by the contractor within 24 hours. Upon completion of each geographical area, the contractor shall use a street sweeper on all streets. The cost for such removal and disposal of such items will be included in other unit price bid. No claims will be considered for extra compensation.

6. Under this scope, “Department”, “Engineer”, and “Resident Engineer” shall mean the

“City of Dunwoody, Georgia, Public Works Director or City of Dunwoody, Georgia or Public Works Designee”.

7. Traffic Control- On Residential streets, working hours are limited to Monday to Friday,

7:30 AM to 8:30 PM unless prior approval is granted by the Department of Public Works. Saturday work may be allowed with prior approval by Public Works. The City requires that the following Arterial / Collector Roads be completed between the hours of 9 PM and 6 AM, Sunday to Thursday (5 days per week):

Meadow Lane Road Lane closures on other non-neighborhood streets will only be permissible between the hours of 9 a.m. and 4 p.m. All traffic control shall be provided by the contractor in accordance with GDOT’s “shelf” Special Provision 150. Assistance from police officers will not be provided by the City. Post-mounted traffic control signs are not required. Liquidated Damages: For each hour in excess of those specified above that the lane closure is in place, Liquidated Damages of $500/Hour shall be assessed.

8. Undercut Excavation- If unsuitable material is encountered below the patch depth, the contractor shall remove the unsuitable material and replace it with additional asphalt patching material as directed by the City’s engineer. The pay item unit price shall include the excavation and disposal of the unsuitable material.

9. Milling- The maximum allowable time between milling and placing new pavement shall

be 48 hours. The standard edge mill width will be 6 to 7 feet. On Arterial / Collector roads, milling may extend around the ends of the radius on side streets, as directed by the City. The depth at the edge shall be determined by the City on a case by case basis; however, the depth is not to extend into the sub-base. Milling shall occur prior to patching. (Add #1, 02/17/17)

10. Manhole and Water Valve Box Adjustments-It shall be the Contractor’s responsibility to note and mark the location of each water valve box and manhole cover on the streets prior to resurfacing, then locate and adjust each of these after resurfacing. The contractor is required to paint a small green mark on the curb opposite sanitary manholes and blue paint for water valves. The contractor shall also be responsible for adjusting all (storm, sanitary, telecom and electrical) manholes to grade as applicable. Water valve adjustments may be made with adjustable rings to match the new resurfacing elevation. Manhole adjustments shall be made in accordance with the provided detail. Adjustments shall be completed within 30 days of paving. Concrete shall be protected from traffic for a minimum of 3 days with steel plates, or other measures. In the event an existing casting or structure is found to be structurally deficient, it shall be reported to the engineer for evaluation.

11. A material transfer vehicle is not required unless the plant and mix used require one as noted on GDOT’s QPL 45.

12. Pavement Marking- Marking tape is permissible for temporary pavement markings on milled surfaces and freshly paved asphalt. The pre-marking layout shall be approved by the City prior to temporary or permanent pavement marking. For bidding purposes, the proposers are to bid on all existing traffic striping to be replaced in the same locations. On Meadow Lane additional marking will be added for bicycle lanes in accordance with the provided detail. The City’s standard bike lane marking is a preformed, skid resistant thermoplastic marking, Pre-Mark PM602006 with arrow or equivalent product on the GDOT Qualified Products List, QPL-74 list. Peek Pavement Marking is not a qualified contractor with the City of Dunwoody.

Permanent thermoplastic pavement markings must be installed no sooner than 30 days after paving but no later than 45 days after paving. Should the contractor fail to meet these requirements, the City will remove these items from the contract and deduct from the contract any additional costs incurred by the City to complete this work. Ashford Dunwoody Road: The existing pedestrian crosswalks will be milled, resurfaced and temporarily painted; the remainder of the striping will be thermoplastic. The contractor shall be responsible for protecting existing stamped or decorative paver crosswalks. PROTECT THE WORK.

13. Asphalt-Rubber Crack Seal- Cracks greater than ¼ inch in width shall be blown free of

debris with an air compressor and filled as directed by the engineer. If necessary to prevent the Asphalt-Rubber Fill from being picked up; clean dry sand shall be spread by hand or mechanically over the filled area. Sand shall only be applied to those areas that are tacky, and the amount shall be the least needed to prevent picking up. The contractor, or its subcontractor, shall notify the City (24 hours advance Notice) as to when and where the Work is to be performed.

14. Contractor shall have all vehicles marked with their company name. 15. The City of Dunwoody will not provide restroom facilities. The contractor shall

arrange for a portable restroom facility at each geographical location for the duration of the time that crews are deployed to the area.

16. The Contract shall perform project housekeeping/clean-up on a daily basis. A 24-hour

contact must be provided to the City of Dunwoody for all issues as needed in regards to the project for any safety, signage, or other emergency as needed.

17. The Contractor’s performance will be measured based on the following:

• Compliance with the Scope of Work and GDOT Specifications • Meeting agreed upon schedule dates • Submission of all deliverables as specified

18. The contractor shall obtain permission from any private property owner on whose

property construction equipment may be parked. Failure to obtain permission from property owners may result in citations.

19. The City of Dunwoody reserves the right to extend this contract by mutual consent for

a limited period of time, not to exceed twelve (12) months, and extend the contract for

additional work to the awarded vendor. This action should be taken in writing prior to the expiration of the current contract.

20. Proof of Commercial general liability insurance with coverage of not less than ONE

MILLION DOLLARS ($1,000,000.00) combined single limit per occurrence, and with contractual liability coverage for Contractor’s covenants to and indemnification of the City under the Contract.

 

21. All Bidders are required to submit a Bid Bond or a certified check made payable to the City of Dunwoody in the amount of five percent (5%) of the total amount bid. The Bid Bond or certified check must be enclosed in the envelope with the sealed bid.

22. The awarded bidder will be required to furnish a contract Performance Bond and

Payment Bond, each in the sum of 100% of the total amount bid and provide insurance coverage as required by the contract documents. Bonding Company must be licensed to do business in Georgia, licensed to do business by the Georgia Secretary of State, authorized to do business in Georgia by the Georgia Insurance Department, listed in the Department of the Treasury's Publication of Companies Holding Certificates of Authority as Acceptable Surety on Federal Bonds and as Acceptable Reinsuring Companies and have an A.M. Best rating of A-V or higher.

23. The awarded bidder will be required to submit Maintenance Bond valid for 12 months

in the amount of the total bid.

24. The contractor shall be responsible for any damage to public or private property caused by the Work such as street signs, mailboxes, fences, curb damage, driveway damage, hardscaping damage, etc.

a. In the event the contractor (or subcontractors working for them) damage private property in the course of performing the Work, the contractor shall be responsible for making restitution.

b. Contractor shall be responsible for communication with property owner(s)

directly with regard to all property damage claims. Contractor shall copy City of Dunwoody on status of each claim until resolved.

25. The contractor shall be responsible for ensuring all cross-slopes are within tolerance per

GDOT specifications and proper drainage flow is achieved. In particular, the contractor shall restore proper drainage flow at intersections to prevent ponding along the radius.

26. Speed Tables are to be marked on the curb opposite each existing location prior to removal. New speed tables are to be installed in place of the old ones per the attached detail.

27. Contractor shall be responsible for notifying the geotechnical consultant when their

input / direction is required and prior to start of work on each street. Copy City of Dunwoody engineer on all correspondence.
