Student Forms

Please complete & return the Medical Declaration Form to the Form Teachers on the first day of school (2 Jan 2018, Tue).

Please update the school of any changes when they occur.

For NEW Applicants

Please collect a FAS application form from the Form Teacher at the registration counter.

Complete the form (with supporting documents) and submit it to the General Office when ready.

You will be contacted once the outcome is ready.

Please have your approval letter ready and proceed to the FAS counter after registration with the Form Teachers.

Submit approval letter from primary school to receive collection slip for books and uniform.

Students who are unable to offer CL/ML/TL (official MTL)- apply to offer an approved foreign language in lieu of the MTL (French/German/ Japanese/Burmese/Thai)

Lessons will be conducted at Ministry of Education Language Centre (MOELC).

Marks will be entered into school report slips and taken into consideration for promotion.
