Registration: To reserve a space for your child, please complete admission information, read and sign operational policies and return. A copy of your child's immunization records must be turned in before your child's first day of attendance. Also, a Health Care Statement from your child's pediatrician must be turned in after their next well child visit. Tuition: Tuition is due each week on your child's first day of attendance and is considered past due on their 2 nd day of attendance. Once past due I will be unable to care for your child until full payment is made. Full tuition is due even if your child is absent. If child is absent the whole week (and a pre-arranged vacation week is not being used) tuition from missed and current week will be due upon return. The weekly tuition for your child(ren) is . Payment can be made by check, cash, or by electronic payment. Checks should be made payable to: Jennifer James, and a charge of$30 will be added to any returned check. Please let me know if you are interested in signing up for electronic payments. Hours & Days of Operation Child care hours are from 6:30am to 5:45pm Monday-Friday. Parents are charged a fee of$1.00 each minute after closing. Please do not drop off your child before opening.

Registration:To reserve a space for your child, please complete admissioninformation, read and sign operational policies and return. VacationsI will give you at least 2 months notice of my vacation plans. Ischedule myself 10 vacation days a year, starting over each January1st. Vacation days are at no charge to you. You will not be charged atall for week long vacations and your tuition will be prorated for partialweek vacations.

You are given 2 weeks of your choice a year for your vacations free ofcharge. In order to receive week free of charge I must have at leasttwo weeks notice. Your vacation weeks start over each year on theanniversary of your child's first day of attendance. Unused vacationweeks can not be carried over to the next year.

Paid HolidaysThe following days are considered my paid holidays and I will beclosed: New Year's Day, Memorial Day, Independence Day, LaborDay, Thanksgiving Day and the Friday after, Christmas Eve Day, andChristmas Day. If the holiday falls on Saturday, I will be closed theFriday before. If the holiday falls on Sunday, I will be closed theMondayafter. Both full and part-time parents pay their regular tuitionon weeks that include a paid holiday. The only exception is if you areusing one of your free vacation weeks on the week of the paidholiday.

Other ClosuresAny days that I am sick and! or unable to care for your child will bededucted from the following week's fee. I will contact you as early aspossible if I am unable to care for your child, in order for you to findalternate care.

Part-Time CarePart- Time Care is only available with a fixed weekly schedule.(Example: child attends every Monday, Wednesday, Friday) Yourweekly tuition is based on these fixed days. Any extra days of care (ifspace is available), are billed as additional days and can not beexchanged for fixed days. Extra days outside your normal schedulemust be pre-arranged.

WithdrawalShould you decide to withdrawal your child, a two weeks notice mustbe given.

IllnessChildren who become ill may not remain in care, nor will an ill childbe admitted. It is the responsibility of the parent to have the childpicked up as soon as possible after being notified of illness. Yourchild will need to be picked up and! or will not be admitted into care ifone or more of the following exists.

*Child can not participate in child care activities, including outdoorplay.*The illness results in greater need for care than I can provide withoutcompromising other children in care.*The child has underarm temperature of 99.4 or greater, with othersymptoms.*Symptoms such as lethargy, abnormal breathing, 2 or more vomitingor diarrhea episodes in 24 hrs, rash with fever, mouth sores withdrooling, wheezing, behavior changes, pink tint to eye, etc.*The child has a communicable disease and does not have medicaldocumentation to indicate child is no longer contagious.

Children must remain naturally fever and symptom free (without thehelp of medication) for 24 hours before returning.

ImmunizationsYou must turn in a copy of your most recent immunization record,showing your child has had all required vaccinations before yourchild's first day of attendance. Please be sure to keep your child up todate on immunizations, and provide me with an updated copy eachtime your child receives new immunizations.

Handling of Serious Illness or Medical EmergenciesIf your child is seriously injured or becomes seriously ill, I willadminister CPR and! or appropriate first aid until EMTs arrive. Iwill call 911 immediately for assistance. I will contact parents assoon as possible.

MedicationIf your child needs any medication administered, you must sign anauthorization and include times and amount of medicine to begiven. All medicine must be in original container and labeled withchild's full name and date brought to me. I can only administermedication in amounts according to label directions or as directedby a physician. I can not administer medication after expiration date.

AnimalsWe have a very good natured beagle, Joey, that will be present in ourhome during child care hours. He is current on all vaccinations andhas a statement of health on file.

Release of ChildYour child will only be allowed to leave with a parent or a persondesignated by the parent. Please provide written notice or a telephonecall stating the name of the person to pick up child. Identificationmust be shown before your child will be allowed to leave with them.This is strictly for your child's safety and your cooperation IS greatlyappreciated.

VisitingParents are always welcome to visit their children anytime during theday.

Child Care LicensingYou may contact the local Child Care Licensing office located at 8700N. Stemmons Freeway, Ste. 104 Dallas, TX 75247. The telephonenumber is 1-800-582-6036. You may ask for my most recentlicensing inspection report and! or the Minimum Standard Rules forRegistered and Licensed Child Care Homes. What I Expect of You:-Respect my hours of operation.-If your child is going to be absent or late, please call in advance.-Notify me in case of illness and keep a sick child at home.

-Please refrain from sending food, candy, gum, and toys with your child,unless it's a special occasion.-Please make payments on time.

What You Can Expect of Me:-Lots of tender, loving care!-Nutritionally balanced breakfast, lunch, and snack. (If dropping off after8am, it is your responsibility to feed your child breakfast before drop off.)-Developmentally appropriate activities planned daily including art, music,circle time activities, etc.-Parents will receive a Daily Activity Sheet to help communicate child'seating, sleeping, diapering or toileting activities each day.-To provide a safe place for your child to learn, play, and make friends!

What to Bring:-If child is in diapers, parents must supply diapers and wipes.-Each child needs a complete change of clothes in case of accidents or spills.-Each child needs a child sized blanket for naptime.-If you would like sunscreen applied to your child before outdoor playplease provide a bottle to keep in my home.-Must bring completed paperwork, immunization records and first week'spayment on your child's first day of attendance, if you have not alreadyturned them in.

NoticesA two weeks notice is required for the following:*Termination of agreement by either party. (after 10 day trial)Increases in child care feesVacations

*The first 10 days of care will be considered a trial run (with pay) for allparties. During these 10 days agreement can be terminated by either partywithout two weeks notice.
