A Good Corporate Citizen is a company/ organization that integrates Social, Environmental, Ethical and Economic values in the core decision making process and activities of their business. This award recognizes an Organization which has made an outstanding contribution in the field of Social Work/Community Development & Service/ Environment protection and conservation/ employee development. Corporate Citizenship is the way in which a company has shown an outstanding contribution to Social development & Community services, Environmental impacts and Economics in the design of its products and services and the management of its operations. a) Distinguished Entrepreneurship Award (Individual)

ELIGIBILITY

a) An Indian corporate / individual (above 18 years of age) can apply for award for one or more of

the following category of awards. The Nature of Business should not be socially undesirable.

b) Distinguished Entrepreneurship Award – MSME (Individual)

(*- decided on nomination basis)

Narration of each Award is appended at the end

b) Previous recipients of awards can apply for any category(s) other than the one in which they

were awarded in the last 5 years.

c) Applicant must ensure that their Annual Accounts and Report and Balance Sheet for the last 3

FYs (2018-19, 2017-18, 2016-17) are audited and furnished with the form.

APPLICATION PROCEDURE

e) Start Ups established after 01.01.2015 will only be considered.

d) Both Investment and turnover will be considered as the criteria for the MSME award category (as

per the revised definition of MSME,June 2020)

1. Application Forms can be downloaded from the website www.phdcci.in/awards/2020.

2. All Application forms should be duly filled in and submitted before the end date i.e., 1st July 2020.

Incomplete forms without annexures will be summarily REJECTED.

3. All Application forms must be accompanied by a Declaration on Company’s Letterhead and

signed by the applicant.

6. Copy of the prescribed Application form can be used for filing more than one nomination in

categories listed above.

4. All Application form must be accompanied by a Recommendation letter on the Recommending

company’s letterhead and duly signed and submitted in original. The recommending person/

company / must be a member of any national / state level chamber or industry association and

must quote their membership number on the recommendation letter.

7. PHD Chamber of Commerce and Industry holds the right to reject any application at any time

with no liability to give justification.

5. A separate form needs to be filled if applying for more than one category. Each form must be

accompanied by annexures (photocopies are permitted with self-attestation with additional

forms)
