Getting ready

Develop the team

Check that your vaccination,anaphylaxis, basic life support and

infection control training is up-to-datefor all pharmacists, including regularlocums, and that their Declaration of

Competence has been completed.

Develop all pharmacy team membersto understand what flu is, how to treat

it, who is eligible for the NHS serviceand infection prevention and control.

Understand the service

As soon as the NHS ServiceSpecification and Patient GroupDirection (PGD) are published, ensurethat you and all the team know whatis involved and what their role is indelivering the service.

If you have a private service,understand the requirements andlimitations of the PGD.

You must have a Standard OperatingProcedure (SOP) for delivery of bothservices.

Make a plan

Have a team meeting to decide whatneeds to be done to get your pharmacyready for your flu vaccination service.

Create a list of critical steps and actionsthat you must do to implement asuccessful service and agree who doeswhat and by when.

Review progress, evaluate what's goingwell and who may need support.

Risk management

Social distancing during a vaccinationwill not be possible.

Hygiene procedures and SOPs, thequality and safe environment of your

consultation room, appropriatepersonal protective equipment for thepharmacist and the patient must be in

place to mitigate risk for all.

Patient education and expectationsmust be carefully managed to ensure

a good experience.

Promote the service

Use social media to target eligiblegroups in your community. Feature on

your pharmacy website and promotein the pharmacy using posters,

leaflets, prescription bag stickers anddigital media.

Contact people in your PMR database

who are at risk and those you havevaccinated before.

Use your team to ask people visitingthe pharmacy whether they have had

their flu vaccination this year.

However good your service may be, ifpeople don't know about it you will

never achieve your goals.

Build on their flu vaccination byensuring that they are aware of your

other NHS and private services andbuild a healthier future for them and

for your pharmacy.

COVID-19 has changed the mindset ofconsumers of healthcare services.

Community pharmacy hasdemonstrated the benefits of

accessing a range of services from youso now is the time to leverage that.

Who does what

Review progress at an individual andteam level, support those who need itand recognise their contribution.

From your action plan, delegate withclear expectations, and empowerpeople in your team to implement andmanage their specific responsibilitiestowards a collective goal.

Collaborate

If you are delivering a private service,contact local businesses andorganisations to see if you cansupport their occupational health.

Work with local pharmacies and GPpractices in your PCN to ensure thatas many at-risk people as possiblereceive the vaccine.

Why this is important

The quality and safety of the environment of yourconsultation room is always important.

For a clinical service like flu vaccination this isparticularly crucial especially during the COVID-19pandemic.

Your room must meet the highest standards of patient,professional and regulatory expectations.

Technology

Having access to IT connectivity in a consultation room isan essential requirement. It also makes for a moreefficient and professional service delivery and facilitatescompletion of all required service administration at thetime.

Some private PGD systems come with an e-tool so thatyou can capture required patient details and eligibilityand book an appointment online. This improvesefficiency but also reduces the time the patient has tospend in the room and risks associated with spread ofCOVID-19 or other infections.

Your consultation room

Organisation

Rooms used for vaccination should be kept clutterfree with equipment kept in closed cupboards tominimise potential for contamination.

Keep any surfaces clean and tidy with the minimumequipment available for the vaccination.

All non-essential items including books andmagazines should be removed from the room or keptin a cupboard.

Consider replacing any carpet floor covering with anappropriate hard floor surface that can bedisinfected. Soft furnishings should be avoided.

Hygiene

Before and after each vaccination, any surfaces,keyboards and door handles (including fridge) should becleaned with disinfectant wipes on the assumption thateither you or the individual has potentially contaminatedthem.

PPE should be readily available and accessible, correctlyput on, used and taken off.

At the end of a session all PPE should be disposed of asclinical waste.

The safety and wellbeing of all people working in and visiting a communitypharmacy is paramount and during the Coronavirus pandemic this is critical.The implications on the pharmacy of the test and trace system also hassignificant implications for business continuity.Self-isolation, the management of social distancing through effective signageand floor markings, good hand hygiene, and the use of protective screens areall key elements of reducing risk. However, the use of masks, visors, apronsand gloves also has a place in certain situations.Every pharmacy should complete a risk assessment for their premises, theirteam and their patients.

FACE MASKS

Public Health England's guidance recommends that a Type l or Type ll face maskis worn by pharmacy personnel to prevent the spread of infection from thewearer. If Type IIR (FRSM) face masks are more readily available, and there areno supply issues for their use as personal protective equipment, then these canbe used as an alternative to Type I or Type II masks1.See separate guidance on the correct use of face masks.

FLU VACCINATION SERVICE

Managing infection control risks when providing clinical patient-facing servicessuch as flu vaccination requires enhanced levels of protection.Public HealthEngland’s guidance for the delivery of flu vaccination in a clinical setting wherepatient contact is minimal recommends the single or sessional use of a Type IIRFRSM mask and hand hygiene between patients. The use of single-usedisposable gloves and apron is not deemed necessary unless there isanticipated exposure to blood, body fluids or non-intact skin.2.A strict hygiene protocol for the cleaning of the consultation room should alsobe developed and implemented.

Dispose of immediately as clinical waste

A GUIDE TO GLOVES USE

Before putting on and after taking offgloves, clean hands with alcohol-

based hand rub or wash with soapand water for 20 seconds.

Take out first glove from boxtouching only at the wrist and pull

on. Take out second glove with barehand and pull on with gloved handtaking care not to touch forearm.

To remove the gloves, grasp theoutside of the glove with the opposite

gloved hand and peel off. Hold theremoved glove in the remaining

gloved hand. Slide the fingers of theungloved hand under the remaining

glove at the wrist.

NO GAPS!

To remove the mask, take it off frombehind, do not touch the front of mask.Discard immediately as clinical waste.Clean hands with alcohol-based hand

rub or soap and water.

A GUIDE TO MASK USE

Before putting on a mask, clean handswith alcohol-based hand rub or washwith soap and water for 20 seconds.

Cover mouth and nose with mask andmake sure there are no gaps betweenyour face and the mask. Do not touch

the front or pull down mask during use.

Replace the mask with a new one assoon as it is damp or every

3-4 hours. Do not re-use single-use masks.

Booking your appointmentWe want to make sure that your flu vaccination is given in a way that keepsus all safe.

Please call us to confirm your eligibility for a free NHS vaccination. We willask you a few questions and then arrange a convenient day and time for yourappointment. Please keep a record to ensure you attend on the day and let usknow if anything changes.

Before leaving for your appointmentPlease ensure you feel well before attending your appointment and call us tore-arrange if you have a new and persistent cough, high fever or have lostyour sense of taste or smell. 

Please come on your own, if you need to bring someone with you then let usknow in advance. Please wear a face covering when you enter the pharmacyand keep this on throughout the procedure.

At the pharmacyPlease arrive about 10 minutes before your appointment time.

The pharmacy team will look after you and advise you on what will behappening. Please observe social distancing guidelines.

Your vaccinationYour vaccination will be administered by a pharmacist in a suitablelocation which may be our consultation room which will have beenhygienically cleaned before the appointment. They will be wearingpersonal protective equipment which may include a face mask, eyeprotection, gloves and apron.

They will ensure that you are cared for before, during and afteradministration of the vaccine.The pharmacy will let your doctor knowthat you have had the vaccination.

BEFORE YOU ENTER THISPHARMACY

If you have a fever, a new andpersistent cough, difficulty breathingor loss of taste and smell do not enter

the pharmacy. Go home and seekadvice from NHS 111.

DO YOU HAVE ANY SYMPTOMS?

Regularly and thoroughly clean yourhands with an alcohol-based hand rubor wash them with soap and water for

20 seconds.

Follow any one-way route and keep atleast 1 metre distance, preferably 2
