    1.0 INTRODUCTION

    The management of Qatar Petroleum (QP) requires that the health and safety of allemployees and other persons who may be affected directly or indirectly by theiractivities, and the protection of its assets is ensured.

    Dukhan Operations is committed to developing and enforcing safe workingpractices. This Permit to Work (PTW) System is a major element in complying withthese requirements, providing a mechanism for controlling workplace hazards, andprotecting employees, facilities and the public.

    To ensure the PTW System is effective, DO NOT DEVIATE FROM IT. The systemis designed to ensure personal and environmental safety, and the integrity of plantand equipment.

    This PTW Procedure is a formal written system that uses Work Permits to controlwork by means of potential hazard identification and risk assessment.

    These work permits are also means of communication between plant management,plant supervisors, technicians who prepare and carry out the work.

    THE ISSUE OF A WORK PERMIT DOES NOT, BY ITSELF, MAKE A JOB SAFE.SAFETY CAN ONLY BE ACHIEVED BY THOSE PREPARING THE WORK ANDTHOSE CARRYING OUT THE WORK.

    The work permit is not simply permission to carry out a job. It is the cornerstone of asafe system of work that determines how that job can be carried out safely. A workpermit must not be regarded as a means of avoiding the need to eliminate hazardsor reduce risks, nor just an exercise in form filling. It does not remove individualresponsibility, as your signature means you are responsible.

    IF YOU ARE UNSURE OF ANY INSTRUCTION ON A WORK PERMIT OR ARE INANY DOUBT REGARDING THE PTW PROCEDURE, STOP AND ASK YOURIMMEDIATE SUPERVISOR FOR GUIDANCE.

    This procedure is applicable to the entire Dukhan Concession Area of QP.

    Work Permits are not mandatory for aboveground civil infrastructure works insidethe Green Fields Areas of Dukhan Concession Area of QP.

    1.1 Management of the PTW System

    The Respective Asset Holder is responsible for ensuring that an effective PTWsystem is established and maintained. He shall also ensure that the system ismonitored, audited and reviewed as required. As a minimum the system shall bereviewed every three years and amended as necessary.

    The Asset Manager or his authorized representative has complete authority for theoperation and administration of the system, and shall appoint Permit Authorities asrequired.

    Permit Authorities are the area supervisor for Operations - Dukhan Fields on allmatters related to the PTW system.

    These supervisors are responsible for ensuring the workforce (with PTWresponsibility) under their authority, inclusive of contractors, are adequately trainedin the operation of the PTW system.

    Administration of the PTW system shall be conducted in English and all personnelwho have responsibilities within the system shall be able to read and write in Englishand fully understand the requirements applicable to their role.

    Training shall be provided via a QP approved and recognized training medium.

    2.0 OBJECTIVES OF THE PTW SYSTEM

    The Dukhan PTW system is designed to ensure:• a coordinated, communicated and controlled safe system of work• all work carried out is recorded• continuous improvement of the PTW system through audit and revision

    3.0 WORK COVERED BY WORK PERMIT

    Work shall not be carried out unless:• it has been approved by either Planned Maintenance Routine, Engineering

    Department, Field Support Department or Plant Change Request (PCR).All other work not defined above must be approved and agreed by thePermit Authority.

    • the work has been planned and agreed 24 hours in advance, or is of abreakdown nature or priority unplanned work requiring immediate action.

    • all work permits have been properly authorized. All work in any arearequires a work permit, with the exception of emergency safety work that iseither ‘loss of life actual’ or ‘loss of life potential’.

    In the event of a breakdown outside of normal working hours, priority unplannedwork or emergency safety work arising, the Permit Authority is authorized to verballyallow work to commence immediately. Work permits shall be prepared as soon aspractical to cover the work being carried out.

    Note: If the above emergency procedure is used, it shall be recorded in the PlantPermit Logbook and signed and dated by the Permit Controller. Theensuing Incident Report shall also show how this work was carried out.

    5.0 DEFINITIONS AND ABBREVIATIONS

    Air Contaminant - Any hazardous substance or chemical that ispresent or that may be present in theatmosphere within a confined space / vessel.Air contaminants include but are not limited to:- hydrocarbon vapours or gases- vapours or gases resulting from

    production operations or from materialsused within the confined space / vessel(e.g. treating chemicals)

    - H2S, CO, SO2- Naturally Occurring Radioactive

    Materials (NORM).Authorized ElectricalPerson (AEP)

    - A trained electrical person, appointed by theLead Electrical Engineer, who has technicalknowledge and competence of the working ofhigh and low voltage systems in his designatedareas of responsibility to carry out switchingoperations and to issue and cancel ElectricalWork Permits. The notice of appointment shalldefine the voltage on which the AEP can workand the areas in which he has responsibility.

    Authorized Gas Tester - Appointed in writing by his departmentmanagement after successfully completing thegas tester course and certified accordingly.

    ‘Blanket’ Work Permit - Any general work permit that has a workscopenot specific to one piece of equipment andinvolving more than one department ordiscipline.

    Certification - Having successfully passed the appropriatelevel of training.

    Cold Work Permit(Uniquely numbered andidentified with the letter ‘C’)

    Competent ElectricalPerson (CEP)

    Controlled Area

    Confined Space

    - A document issued for the performance of workwhere a source of ignition cannot be created.This includes but is not limited to: bolting andunbolting flanges on pipelines, vessels andequipment, work on electrical or instrumentequipment that is incapable of being energizedor activated, manual excavations to a depth ofless than one metre.

    - An electrical person, either from Senior Staff orELS cadre and appointed by the Lead ElectricalEngineer, who has sufficient technical expertiseand experience to recognize hazards and totake remedial action in his designated area ofresponsibility. The notice of appointment shalldefine the voltage on which the CEP can workand the areas in which he has responsibility.

    - A work place where radiation exposure couldexceed the three tenth of the annual limit ofradiation workers. (20Sv/year), or the doserate could exceed 7.5 Sv /hour. Only radiationworkers are allowed to enter a controlled area.

    Any enclosure having a limited means of entryand exit and not designed for continuousoccupancy. Examples include but are notlimited to the following:- process vessels, tanks, bins, stacks,

    large pipe, pits, vaults- any enclosure where the presence of air

    contaminants may be harmful to a workerand prevent his ability to escape unaided

    - Any excavation with a depth of more than1.25m where hazardous atmosphereexists or could reasonably be expected toexist, such as excavations in landfill areas,contaminated areas, areas wherehazardous substances are stored nearbyand within the production/live stations

    Electrical Isolation - The secure, proven disconnection andseparation of a circuit or item of equipment fromevery source of electrical energy.

    Firewatcher - Appointed by the Permit Applicant, he shall holda valid training card from a certified TrainingCenter and have basic fire fighting knowledge(use of portable extinguishers and fire hose).He shall be capable of interpreting portable gasdetection monitors and understand what to do inthe event of an alarm, and how to raise analarm.

    Green Fields Area - A clearly demarcated virgin and safe area nothaving any underground or abovegroundfacilities / services like hydrocarbon processing,handling or transportation facilities, cables,pipelines etc.

    Hot Work Permit(Uniquely numbered andidentified by the letter ‘H’)

    Isolation Register(specifically for Long TermIsolations)

    - A document issued for the performance of workwhere heat or sparks may be generated,making it possible to ignite flammable liquids,vapors, gases or other combustible materials.This includes but is not limited to: burning,welding, grinding, chipping, grit-blasting, use ofelectrical equipment or internal combustionengines.

    - A written record of the location and descriptionof all physical disconnections and mechanicalisolations (blinds / spades and blank flanges)installed in process piping, vessels orequipment.This information includes the type of blind, tagnumber, date installed, date removed,authorizing signatures, and other requiredinformation.

    Lock off - Placement of an approved locking device onequipment controls to prevent unintendedoperation or start-up, e.g. placement of a lockand chain on a valve handle to prevent themovement of the valve handle and theunintended operation of the valve.

    Long Term Isolation - Isolations that remain in place when all workpermits associated with the PRC are closed.

    Lower Explosive Limit - The lowest concentration of gas in theatmosphere that results in a combustiblemixture.

    Naturally OccurringRadioactive Materials(NORM)

    - Any material found in the natural environmentthat has radioactive properties. In oilexploration and production terms, this usuallyrefers to radioactive scale sometimes found onthe walls of vessels, pipes, valves, or entrainedin sludge. This material is formed by theprecipitation of salts brought to the surface inthe produced water.

    Oxygen Deficiency - A condition in which the atmosphere containsless than 19.5% Oxygen by volume.

    Permit Applicant - The Performing / Asset Holding Department’srepresentative initiating the work, appointed inwriting by the Department Head. He shall havethe technical competence to consider andguard against potential hazards associated withhis work activity. However, the personshould be authorized by his departmentmanagement and hold a valid PTW trainingcompletion card issued by HS(D) Department.

    Permit Authority - A Supervisor from the operating / asset holding/ sponsoring department with responsibility forthe operation of the plant and managerialauthority for safety-related matters over allpersonnel on site, irrespective of theirdepartmental reporting relationship. However,the person should be authorized by hisdepartment management and hold a validPTW training completion card issued by HS(D)Department.Subject to Department Managers approval,Permit Authority can be Permit Applicant or aPermit Controller.

    Permit Controller - A Supervisor or Senior Technician to beidentified by his department management andshall control, coordinate and execute the issueand closure of work permits. He shall have acomplete overview of all operational activitiesunderway or planned on site in order to avoidrisk caused by simultaneous activities.However, the person should be authorized byhis department management and hold a validPTW training completion card issued by HS(D)Department.

    Policy - A rule or directive that guides or restricts action.Procedure - A document that details the execution of an

    activity.Public / Non-RadiationWorker

    - A person who is not designated to beassociated with activities of IndustrialRadiography, is not permitted to be involved inwork or passage where ionizing radiation ispresent, and must be excluded from areaswhere ionizing radiation warning signs andbarriers are erected.

    Pyrophoric Iron Scale - This is produced when Iron oxidizes in thepresence of materials containing HydrogenSulphide. The resulting scale can ignitespontaneously when dry and exposed to air.

    Radiation Protection Officer(RPO)

    - A suitably trained and experienced person,qualified in Radiological work and appointed, inwriting by an authorized licensee, to be aRadiation Protection Officer. This person shallhold a valid personal license issued by theMinistry of Environment and shall be approvedby HS(D) for Radiography work permit. TheRPO shall undertake the responsibility of theworksite supervisor.

    Radiation Worker - A person designated and qualified to beassociated with activities of IndustrialRadiography.

    Radiography Work Permit(Uniquely numbered andidentified by the letter ‘R’)

    - A document issued for the performance of workinvolving radioactive sources and radiationgenerating equipment.

    Shall - A mandatory action or requirement.Should - A preferred course of action.Sponsoring Department - The department that schedules, plans and

    coordinates the work. It shall ensure that allpersonnel under its jurisdiction are adequatelytrained and competent to perform their dutiesas required by the PTW system.

    Standby Man - Appointed by the Permit Applicant, he shall beapproved in his duties by the HSE Department.He is stationed outside the confined space andshall maintain line of sight or verbal contact withpersonnel undertaking confined space entry.

    Suspended Work Permit - A work permit for which isolations, precautionsetc. are still in place, but the work is not beingprogressed.

    Worksite Supervisor - A person appointed in writing by the SponsoringDepartment’s representative. He is responsibleto the Permit Applicant for all aspects of safetyrelated to the work and work party. However,the person should be authorized by hissponsoring department / Permit Applicant andhold a valid PTW training completion cardissued by HSE Department.

    An essential requirement for safe and efficient administration of the PTW system iscomprehensive forward planning. A weekly work permit meeting shall be held todiscuss the proposed Hot Work permits for hazardous / operational areas.

    As a minimum the following points shall be discussed:• a description of the work to be performed and the ways in which the work will

    be carried out• tools / equipment required and any necessary safety procedures• the area and system / equipment to be worked on, and any hazards or

    potential hazards that may arise• whether the work may affect any other area, system, or equipment either

    adjacent or remote

    Attendees at these meetings shall include the Permit Authority or PermitController, Permit Applicant(s), Safety Officer and anyone else required.

    Note: Breakdown and priority unplanned work may necessitate immediate action.

    7.0 TYPES OF WORK PERMIT / CERTIFICATE

    In order to determine the type of work permit required, see Attachment B.

    The PTW system incorporates the use of 3 Primary Permits as follows:• Cold Work Permit Primary (see Attachment C)• Hot Work Permit Primary (see Attachment D)• Radiography Permit Primary (see Attachment F)

    In addition to the above permits, a number of other certificates are used as anintegral part of the PTW system. They are used when it is necessary to recordadditional information and special precautions to be taken to ensure safe preparationand implementation of the work. They are as follows:

    Job Safety Analysis (Ref: QPR-DKN-25)• Electrical Work Certificate (see Attachment G)• Preparation / Reinstatement Certificate (PRC) (see Attachment H)• Confined Space Entry Certificate (see Attachment I)• Excavation Certificate (see Attachment J)• 33KV Overhead Line Clearance Form (see Attachment K)

    Note: A Spading / Blinding List (see Attachment L) is mandatory for all vessel ortank entry activities, however the Spading / Blinding List is not classed as acertificate.

    8.0 OPERATION OF THE PTW SYSTEM

    A flowchart of the PTW system is included as Attachment A.

    8.1 Work Permit Validity

    Cold - Maximum duration of 14 consecutive working daysfrom the date / time of issue.

    Hot, Radiography - Maximum duration of 7 consecutive working daysfrom the date / time of issue.

    8.2 Work Permit Application

    All identified work shall be authorized and planned prior to work permit application.The Permit Applicant shall raise the appropriate work permit and should use the legchart as a means of deciding the type of permit required. Included in Box 1 of thepermit shall be a full description of:

    • location of work• applicant telephone number• work order / contract number• equipment to be worked on and identification number• exact nature and scope of the work• speciality tools and/or equipment that may impact on safety at the worksite• need for daily Permit endorsement by the Permit Applicant, by ticking the

    relevant box

    The Permit Applicant shall liaise with the Permit Controller to complete the relevantsections of the work permit to the best of his knowledge, and any additional workpermits and supporting certificates as required.

    The Permit Applicant or his representative shall present the work permit and anysupporting documentation to the Permit Authority for review and authorization.

    8.3 Work Permit Authorization

    The Permit Authority, together with the Permit Applicant, shall review the workpermit and any supporting documentation and, if satisfied, authorize the PermitController to process the work permit, ensuring that equipment preparation andsafety precautions are put in place.

    The Permit Authority shall specify the validity of the work permit after discussion withthe Permit Applicant. However it shall not exceed the limits stated in Section 8.1. Anew permit shall be processed for work extending beyond the validity of a permit.

    8.4 Work Permit Control

    On being authorized, the Permit Controller shall review the work permit and anysupporting documentation and shall complete the appropriate sections. He shallarrange for equipment preparation, e.g. depressurizing, draining, flushing, purging,and any process, mechanical, safety, or electrical isolations required.

    When he is satisfied that the worksite has been safely prepared (see Section 10 ofthis procedure) and lock-off keys for process / electrical isolations are secure in theLockout Box(es), the work permit details shall be recorded in the Work PermitRegister. Only when this is complete will the work permit and the relevantequipment be released to the Permit Applicant, after the Permit Controller hassigned.

    8.5 Work Permit Issue

    The Worksite Supervisor assigned by the Permit Applicant shall collect the preparedwork permit from the Permit Controller and sign the permit to acknowledge heunderstands it and accepts the associated worksite responsibilities.

    The Worksite Supervisor shall then obtain the first endorsement from the PermitController for the work to proceed. He shall then proceed to the worksite tocommence the work.

    No amendment to a work permit or certificate is permitted without the approval ofthe Permit Authority. Additional safety precautions may be added with the approvalof the Worksite Supervisor and the Permit Controller.Note: Overwriting or correction fluid on a work permit / certificate immediately

    invalidates it and shall cause it to be cancelled, except in case of extension ofpermit (see Section 8.6 of this procedure).

    If a work permit has all the endorsements complete but the isolations and work areto continue, a new work permit shall be raised. The new work permit shall then beapplied for and authorized using the normal procedure.

    Each work permit endorsement from the Permit Controller is only valid for amaximum of one shift. The oncoming Permit Controller shall endorse workextension into the next shift. If the work permit endorsement boxes have all beencompleted, a new work permit shall be raised.

    8.6 Extension of a Permit

    If work has not been completed within the validity of a permit, a new permit shall beissued for the remaining work. The Serial Number of the new permit shall bestruck-off and the Serial Number of the old permit shall be written in its place. Allwork permits and certificates associated with the same work shall be cross-referenced by inserting the certificate number in the box provided on eachdocument. Additional certification raised during the validity of an issued permit shallalso be cross-referenced on all copies of the ‘live’ work permit. There will not beany limit to the number of new permits issued for completion of ongoing work.

    8.7 Transfer of Responsibility

    Permit Authority - If the position holder changes, all work permits shallbe presented to the new Permit Authorityimmediately after the change occurs.

    Permit Controller - All work permits shall be presented for transfer ofauthority immediately to the oncoming PermitController as he arrives on site.

    Permit Applicant - All work permits shall be presented for transfer ofauthority immediately to the oncoming PermitApplicant as he arrives on site.

    Worksite Supervisor - Work shall stop if he leaves the site for shift change.The new Worksite Supervisor shall sign the ‘Transferof Responsibility’ part of the permit in the presenceof the Permit Controller.If the work is being performed on more than one shiftper day and the Worksite Supervisor changes duringthese shifts, the oncoming shift Worksite Supervisorshall endorse the work and permit declaration withhis name, timing of the shifts, and signatures on theattached sheet.

    Radiation Protection - Work shall stop if he leaves the site for shift change.Officer (RPO) The new RPO shall sign the “Transfer of Responsibility”

    part of the permit in the presence of Permit Controller.If the work being performed is more than one shift perday and the RPO changes during this shift, theincoming shift RPO shall endorse the work permitdeclaration with his name, timing of the shift andsignature on the attached sheet.

    8.8 Cross-referencing

    All work permits and certificates associated with the same work shall be cross-referenced by inserting the work permit / certificate number in the box provided oneach document. Additional certification raised during the validity of an issuedpermit shall also be cross-referenced on all copies of the ‘live’ primary work permit.

    8.9 ‘Blanket’ Work Permit

    The use of ‘Blanket’ work permits as defined is not permitted.

    8.10 Work Permit Suspension

    If a general alarm activates all work shall stop immediately. Work shall notrecommence until cleared to do so by the Permit Controller or by announcementfrom the Control Room, and the permits shall be stamped and initialed accordingly.

    The Permit Controller, Permit Applicant or Safety Officer are authorized to stop workin the event of work permit violation or unsafe acts.

    All other personnel may stop the work if they observe any hazardous or potentiallyhazardous condition. They shall immediately inform the Permit Controller, PermitApplicant or Safety Officer. If on investigation it is determined the work permit is tobe suspended, the Permit Authority shall be informed immediately.

    If Hot Work is suspended for operational reasons, e.g. due to sampling, the ControlRoom shall advise the Permit Applicant / Worksite Supervisor if other work permitsare to be suspended or withdrawn. The Permit Controller shall retrieve all relevantwork permits. Work shall not resume until the Permit Controller declares it is safe todo so.

    A work permit can only be suspended for a maximum of five days or until the expiryof its validity period (whichever is sooner), when it shall be cancelled and signed off.Thereafter a new work permit shall be raised or the system / equipment shall beplaced in long term isolation as appropriate.

    8.11 Closing a Work Permit

    On completion of the work, the Worksite Supervisor shall check the worksite andensure that all tools and equipment have been removed and the area cleaned.When all activities have ceased, he shall return the work permit to the PermitController for his endorsement.

    The Worksite Supervisor shall then inform the Permit Applicant, who shall sign thework permit signifying handing back of the equipment. Responsibility for thesatisfactory handing back of the worksite is the Permit Applicant’s.

    Note: For Confined Space Entry Certificates, it is mandatory that the PermitController visits the site and checks the confined space is clear prior toclosure of the certificate.

    The Permit Controller shall ensure the area is acceptable prior to signing foracceptance of handing back of the area / equipment. The Permit Controller canthen allow de-isolation of the equipment.

    The Permit Controller shall ensure that all isolations are removed before closing thework permit, and associated work permits and/or certificates as appropriate, andallowing the equipment to be reinstated.

    8.12 Work Permit Retention

    The original (top copy) of the work permit, and top copy of any associatedcertificates, shall be held on site for one month or an appropriate time as determinedby the Permit Authority, prior to being archived.

    If an incident occurs associated with a work permit, the original copy of the workpermit and the original copy of any associated certificate(s) shall be attached to theIncident Report. Copies of these originals shall be retained on file.

    9.0 PREPARATION OF THE WORKSITE

    It is the responsibility of the Permit Authority to ensure the safe preparation of theworksite. He may assign the application for process isolations to a nominatedperson, but he still retains overall responsibility.

    Electrical isolations shall be assigned via the AEP.

    When the Permit Authority decides it is necessary, the Permit Applicant shallprepare formal plant or equipment preparation procedures / method statement. Thiswill be completed prior to the work commencing and reference should be made toany Simultaneous Operations procedures when necessary.

    The Permit Controller shall provide copies of marked-up P&IDs, spade lists and/orisolation procedures to the Permit Applicant (mandatory for all vessel entry andprocess isolations).

    Based upon the detailed description of the work to be performed, hazards shall beassessed regarding the materials to be handled and the prevailing conditions.

    The possible impact of the work on the surrounding area shall be assessed and caretaken not to issue a work permit that could impact on the safety conditions of otherwork permits in the area.

    The Permit Controller can introduce additional precautions if necessary based on hisassessment. Based on the assessment of the hazards involved, the precautions tobe taken to enable the work to proceed safely shall be indicated by the PermitAuthority on the work permit. Based on the residual risks associated with the job,the permit/JSA to be escalated to higher management approval. Refer procedureQPR-DKN-25 on Job Safety Analysis for further details.

    The following are examples for process equipment:• isolations required and method of isolation, e.g. blanking, spading, double

    block and bleed etc.• shifting of mobile equipment• personal protection required• gas testing / monitoring and specified frequencies

    9.1 Cold Work

    The performance of cold work shall require, but is not limited to, the followingprecautions:a) The Permit Authority shall specify if the Permit Controller and Permit Applicant

    are required to make a site visit during the shift.b) If a gas test is required, it shall be carried out immediately prior to the

    commencement of work and, if the work party has left the worksite, immediatelyprior to resumption of work.

    persons performing the work shall ensure that the portable gas detectors remainfully charged and active.

    d) The Worksite Supervisor shall ensure that all personnel under his control strictlyadhere to the conditions of the work permit.

    e) The Worksite Supervisor shall remain at the worksite during the performance ofthe work.

    9.2 Hot Work

    Prior to any hot work being authorized, the Permit Authority shall consider thefollowing points:

    • could the work be removed to a safer location?• is it essential for safety or process reasons to perform the work during

    normal operations, or could the work be deferred until a total or partialshutdown?

    • has the workscope and potential hazards been discussed by the PermitApplicant, Safety Officer and Permit Controller?

    • another hot work permit will NOT be issued for an area where a confinedspace entry certificate for work permits associated with this activity are inforce

    • the performance of hot work is NOT permitted in the same area as workinvolving paint spraying or hand painting

    • cold work that is likely to release hydrocarbons near to the site of hot workshall be suspended

    • gas test is required; it shall be carried out immediately prior to thecommencement of work and, if the work party has left the worksite,immediately prior to resumption of work

    • the Worksite Supervisor shall remain at the worksite during the performanceof the work as defined by the Permit Authority on the permit

    • the number of hot work permits to be issued in any one area will bedetermined by the Permit Authority, based upon the availability of manpowerand equipment to control and make safe for the work to be carried out

    a) Preparation and Performance of Hot Work

    The Permit Controller and Permit Applicant shall make regular visits to theworksite during the work activity.

    Total isolation from flammable or toxic gas / liquids shall be achieved in one ofthe following ways:

    • physical disconnection• blinding using correctly rated blinds• spading using correctly rated spades• double block and bleed• any combination of the above as deemed necessary by the Permit

    Note: It may be necessary to raise a Cold Work permit for this preparatory work.The Permit Authority shall assess this after discussion with the PermitController.

    The Permit Controller shall ensure that the equipment is:

    • depressurized• drained• vented• purged and flushed• or any of the above as required to allow hot work to commence

    Caution: During the venting of hydrocarbons to atmosphere, all hotwork in the same area must be suspended.

    All hot work sites shall be free of hydrocarbons and combustible materials, e.g.sludge, scale, deposits etc.

    The worksite shall be gas tested by an Authorized Gas Tester and certified asbeing flammable / toxic gas free. A certified gas detector shall be active on sitethroughout the hot work. The maximum LEL permissible for authorized hot workis 0% LEL.

    During the work, all sparks and splatter shall be suitably contained. If the work isabove access or walkways, precautions shall be taken to ensure that no sparksor splatter falls below. The area beneath the worksite shall be barricaded offand warning notices posted in English and Arabic. All drains in the vicinity shallbe covered.

    Mobile plant or equipment shall not be sited in hazardous or non-hazardouslocations unless it meets with the safety requirements for operation in that area.

    9.3 Confined Space Entry

    Preparation for entry into confined spaces shall be performed in accordance with theapproved procedure IP-OPS-031, which will be provided by the Permit Controller.The precautions are mandatory and apply even when a person’s head is insertedinto the space for a quick inspection. The following requirements are applicable toall process and utility systems, irrespective of service:

    • the Permit Controller and Permit Applicant shall make regular visits to theworksite during the work activity

    • the confined space shall be positively isolated from all process and utilitysystems by disconnection, blinding and/or spading, using the correctly ratedspades / blinds (where disconnection is employed as the means of isolation,both sides of the disconnected lines will be blinded)

    • isolations shall be achieved as close to the confined space as possible (aCold Work permit will be required for the isolation work)

    • if the confined space is equipped with a source of electrical power orconductors, an electrical isolation shall be carried out (the AEP can alsoconfirm the adequacy of the electrical earthing)

    • the atmosphere within the confined space shall be initially tested by thePermit Controller for oxygen level, toxic gas / vapour and combustible gas,and subsequently tested every hour (a portable gas monitor shall be on sitefor continuous monitoring)

    • entry to any confined space shall only be authorized by the Permit Controller• when determining the criteria for confined space entry without breathing

    apparatus, residues that may contain and emit dangerous levels of vapoursshould be considered, e.g. sludge, scale and deposits

    • normally only 2 persons shall be allowed to work in a confined space at anytime (any change to this shall be approved by the Permit Authority)

    • a Standby Man, equipped with a hand-held radio, shall be stationed at theentry point to the confined space AT ALL TIMES when persons are inside

    • the amount of time allowed for personnel to work inside the confined space,and the appropriate rest periods, shall be stipulated on the permit (timeshould be decided based on site conditions)

    Where the Permit Controller is one of the persons entering the confined space, thePermit Authority shall endorse the certificate. Where the Permit Authority decides toenter, the Permit Controller shall endorse the certificate. Under no circumstancesshall the Permit Authority and the Permit Controller be permitted to enter theconfined space at the same time.

    9.4 Radiography

    The transportation, control and handling of radioactive isotopes shall be inaccordance with QP Radiation Safety Standards for Industrial Radiography.

    The following conditions shall apply:• the Permit Applicant shall raise a radiography work permit and, where

    possible, schedule the work to be carried out during meal breaks or onnightshift

    • a radiography work permit can be endorsed for a maximum period of 14days

    • if a radiography work permit is required for more than 24 hours, the PermitApplicant must also endorse it. This endorsement acknowledges the PermitApplicant’s acceptance of responsibility for the conditions specified in theradiography work permit.

    • a 3-dimensional controlled area shall be designated and marked by barrierswith warning signal and light.

    • prior to the commencement of radiography, all other work permits, eitherwithin the controlled area or as specified by the Permit Authority, shall besuspended

    • identified UV overrides shall be activated in accordance with the PRC

    • PA announcements, advising all public or non-radiation workers to makesafe their worksite and vacate the area, shall be made 30 minutes prior toradiography commencing. A further PA announcement, advising all public ornon-radiation workers to vacate the area immediately, shall be made 5minutes prior to radiography commencing.

    • the Radiation Workers shall check the controlled area to ensureall public or non-radiation workers have vacated.

    The contractor RPO shall ensure that all radiation safetyprecautions are in place as required.

    • a final PA announcement shall be made to announce the commencement ofradiography

    9.5 Electrical Work Certificates and Associated Certificates

    Engineering Standards – Electrical ES.E.50, Volume 6 shall be used to refer to theperformance of electrical work in Dukhan facilities.

    a) Electrical Work Certificate

    This is an electrical working document issued by an AEP, in conjunction with aprimary permit to work, for the purpose of specifying electrical equipment safe towork on (see Attachment F).

    At the discretion of the AEP, an Electrical Work Certificate may be issued forwork on a low voltage system, e.g. work on the busbars of a 415V switchboard.The safety rules for work on high voltage systems shall apply to this work. Thiscertificate can be transferred between AEPs.

    b) Electrical Section of the PRC

    After discussion with the Permit Applicant, the requirement for electrical isolationshall be specified by the Permit Controller at permit application.

    The PRC can be issued to a CEP for the isolation of low voltage equipment only,i.e. equipment not exceeding 1000V AC or 1500V DC. A PRC for high voltage,i.e. exceeding 1000V AC or 1500V DC, can only be issued to and performed byan AEP.

    c) 33 KV Overhead Line Clearance Certificate

    This is an electrical working document issued by an AEP, in conjunction with aprimary work permit, for the purpose of assessing clearance between temporaryconstruction equipment and 33 KV overhead lines (see Attachment K).

    At the discretion of the AEP, this certificate may be issued to allow work to becarried out in the vicinity of such overhead lines.

    9.6 Excavation Certificate

    Excavation work shall be carried out under the control of an ‘Excavation Certificate’

    (see Attachment J), which will be supplemented by a primary work permit forexcavation work only authorized by the Permit Authority and the Permit Controller.

    A drawing or sketch of the proposed excavation work and site shall be prepared bythe sponsoring department and submitted along with the certificate. This will enableany affected disciplines to identify hazards and specify precautions to be takenwhen signing for approval.

    Considering the criticality of excavation or its proximity to live facilities, if the PermitController stipulates that the presence of the Permit Applicant / QP Representativeis required, then the relevant box will be ticked on the Excavation Certificate.

    Excavation Certificates shall normally be presented for approval at least 24 hoursprior to the work commencing. The Permit Authority may authorize the immediateprocessing of a primary work permit and ‘Excavation Certificate’ for emergency workonly. Each Excavation Clearance Certificate shall be signed by the InstrumentationAuthority, Electrical Authority (Distribution), Electrical Authority (Operations),Telecommunication Authority, Civil Maintenance Authority, Mechanical MaintenanceAuthority, Water Authority (Potable and Fire) and Sewage Authority (Raw andTreated Sewage Effluent).

    A matrix for identifying the respective authority for each area is included asAttachment Q.

    Notes: I. The cancelled certificate shall be attached to the new certificate duringprocessing and retained by the Permit Controller until all work has beencompleted and the permit / certificate closed.

    II. As soon as the “excavation” work is finished, the ExcavationCertificate shall be closed and a new permit shall be issued tocontinue the work at the excavated area.

    The Worksite Supervisor shall examine the worksite each time the ‘ExcavationCertificate’ is endorsed to ensure it is safe to continue work. All excavationworksites shall be barricaded off, with warning notices written in English andArabic clearly visible. Flashing warning lights are mandatory during periods ofpoor visibility, e.g. during the hours of darkness, during foggy weather orsandstorms.

    If an unexpected underground cable or pipeline is discovered during excavation, thework shall be stopped and the Permit Controller informed. He shall attend at theworksite and liaise with the Permit Authority to decide on the action to be taken.

    10.0 ISOLATION, LOCK OFF AND TAG

    Isolation, lock off and tag is an integral part of the PTW System. If plant orequipment is specified as requiring isolation as a condition of the work permit, a‘Preparation / Reinstatement Certificate (PRC) shall be issued and becomes part ofthe work permit to which it is referenced.

    Personal isolations are only permitted to be carried out by a CEP or AEP (forelectrical, instrument and telecommunications) as defined in ES.E.50. When suchisolations are used, this shall be identified by the Permit Authority on theappropriate section of the work permit as a condition of the work to be performed.Personal isolations can only be used for a period of one shift.

    The method of isolation shall be detailed on the PRC and the isolation shall only becarried out by the nominated authorized person. If isolations require ‘locking off’,official QP padlocks, clearly stamped with an individual ID number, should be used.If padlocks cannot be installed, other approved immobilizing devices as specified bythe Permit Controller shall be used.

    Isolation padlock keys and, where other forms of immobilizing devices are used, the‘tear off’ section of the isolation tags shall be locked in a Lockout Box (seeAttachment M). Each isolation point and any vent / drain point that has beenopened or closed shall have an isolation tag attached, clearly noted and referencedto the PRC.

    10.1 Mechanical Isolation

    The type of isolation used shall depend on the level of risk associated with the workto be performed. For further information, refer to IP-OPS-029.

    10.2 Process / Mechanical Isolation Procedure

    It is the responsibility of the Permit Controller to ensure that marked-up P&IDs andan approved spade list are issued to the Permit Applicant for use.

    The physical application of isolations can be delegated by the Permit Controller toan appointed person (mechanical in the case of blind or blank installation). Allspades / blinds inserted, and ultimately removed again, shall be logged on a‘Spading / Blinding List (see Attachment L).

    The effectiveness of isolations shall be tested and proved. If an isolation valve ispassing, it may be necessary to shut additional valves or take further plant orequipment out of service. The possibility of the presence of wax, hydrates, sludgeor scale under a valve seat should be considered, as subsequent pressure drop,melting or movement of the obstruction may render the isolation ineffective.

    All isolation points shall be detailed on the PRC as follows:• all valves ‘locked closed’ and tagged• lock numbers shall be noted• vent and drain valves left in an abnormal position shall be tagged and noted

    • blind flanges, plugs, pipe joints or any other equipment removed or loosenedshall be tagged and noted

    When the lockout box finally contains all the relevant isolation information, thenumber of the lockout box shall be recorded on the PRC.

    The Permit Controller shall issue one lockout box key to the Worksite Supervisor,record the issue on the PRC, and obtain the relevant signature.

    If an isolation is in force under a work permit and an additional work permit is raisedwhich requires exactly the same isolation, no additional isolation needs to becarried out. To ensure isolation control, an additional key is removed from thelockout box holding the appropriate isolation records and is handed over to thesecond Worksite Supervisor under signature.

    The original and all copies of the PRC will be amended to reflect the issue of thesecond key, and both permits shall be cross-referenced to each other and the PRC.

    A control system (issue and return) for the lockout box keys shall be developed bythe Permit Authority and any local rules approved by management. This may reflecteither a ‘2-key’ or ‘3-key’ system, dependent on the requirements of the userdepartment.

    The ‘C’ (Control) key from a lockout box shall not be issued to a WorksiteSupervisor. This key can be cascaded from work permit to work permit by beingplaced in an unused lockout box, providing additional Worksite Supervisors withlockout keys. This allows multiple work permits to be issued without jeopardisingthe integrity of the original isolation.

    When the work associated with a work permit is completed, the Worksite Supervisorreturns the work permit, suitably signed off by the Permit Applicant, along with thelockout box key to the Permit Controller.

    The Permit Controller and the Worksite Supervisor shall sign the ‘lockout box key’section on the PRC to record the return of the key.

    If isolations need to remain in force, this shall be reflected on the work permit whenclosed, along with all associated work permits / certificates.

    The original PRC shall be stamped with the letters ‘LTI’ (Long Term Isolation), andthe padlock keys, tags, and PRC shall be placed in the Long Term Isolation LockoutBox and recorded in the LTI Register.

    10.3 Electrical Isolation

    The Permit Controller, in liaison with the Permit Applicant, shall specify therequirement for an electrical isolation.

    Following discussion with the Permit Controller and the Permit Applicant, anappropriate AEP or CEP shall complete the ‘electrical isolation’ section of the PRC.

    The isolation will be carried out using a padlock and key supplied by the PermitController. Once the isolation is effected, the AEP / CEP shall place the padlockkey(s) in the lockout box allocated by the Permit Controller and record thenumber(s) on the PRC.

    The lockout box Key ‘A’ will then be issued to the Worksite Supervisor in charge ofthe work.

    When the work requiring the electrical isolation is completed, the WorksiteSupervisor shall return his signed-off permit, with PRC attached, and lockout boxKey ‘A’ to the Permit Controller, who shall arrange for de-isolation by theAEP / CEP, provided it is safe to do so.

    If the isolation needs to remain in force, this shall be reflected on the work permitwhen closed, along with all associated work permits / certificates.

    The original PRC shall be stamped with the letters ‘LTI’ (Long Term Isolation), andthe padlock keys, tags, and PRC shall be placed in the Long Term Isolation LockoutBox and recorded in the LTI Register.

    10.4 Long Term Isolation

    If work is completed or suspended beyond the work permit’s period of validity, butthe worksite requires the continued protection of an electrical, mechanical, safety oremergency system isolation, the following shall apply:a) Sign off to close the original work permit.b) Stamp the original PRC with the letters ‘LTI’.c) Retain the original copy of the PRC, with the original work permit attached, in the

    Long Term Isolation file, and give a copy to the Permit Applicant for his record.The details of the LTI shall be recorded by the Permit Controller.

    d) The Permit Controller shall retain the lockout box control key and one other keyshall be retained by the Permit Authority.

    When the work is to re-commence, the following shall apply:a) The Permit Applicant requiring release of the equipment / system shall raise a

    work permit.b) The key held by the Permit Authority shall be returned to the Permit Controller,

    who shall ensure that the original isolations are verified as being in place andeffective.

    c) The Permit Controller shall then transpose the original details from the LTI PRConto a new PRC, noting the name of the person who verified the isolations as the‘isolator’.

    d) The Permit Controller shall draw 2 lines across the original LTI PRC, writecancelled and discard it. The new PRC shall be attached to the work permit.

    The work can then be completed under normal PTW conditions. When the work iscomplete, the Permit Controller shall cancel all work permits, associated certificates,and remove the isolations.

    11.0 SCAFFOLDING WORK UNDER PERMIT

    The erection / dismantling and inspection of scaffolding shall be in accordance withIP-OPS-030.

    All work to be carried out at a height above 2 metres shall be accomplished from acertified scaffold, secured ladder or purpose-built platform.

    12.0 PERMIT AUDITING

    Three audits per week shall be carried out on specific work permits as follows:• the Permit Authority shall audit 1 live work permit• the Permit Controller shall audit 1 completed work permit• a senior supervisor shall audit 1 completed work permit

    The Permit Authority may, at his discretion, request other supervisors to carry outaudits.

    LTIs shall be audited at least once per month.

    A ‘Permit Compliance Checklist’ shall be completed for each audited work permit(see Attachment O). The Permit Authority shall endorse the audit findings and, ifdeficiencies are found, may put corrective actions in place immediately or makerecommendation for amendment to the PTW System (see Attachment P).

    All completed audit reports shall be forwarded to the HSE Department forindependent review.

    In addition to the formal audit program a PTW – STOP/OBSERVATIONCARDS ,also introduced in the Dukhan fields to collect the informationfrom the work site and to report the management about the PTWcompliance. This to be utilized for stopping any unsafe act if observed atworksite. Refer Attachment “R” for the form.

    13.0 RESPONSIBLE PERSONS

    PTW position holders are required to have completed the appropriate training andbe certified accordingly. Certification shall be the responsibility of the respectiveDepartment Head and, where appropriate, the Permit Authority.

    13.2 Permit Controller

    The Permit Controller shall ensure that:• all hazards associated with the proposed work have been identified• the worksite has been examined prior to commencement of work, and all

    specified precautions have been taken• work permits that may interact or affect one another are clearly

    cross-referenced• the original of the issued work permit is displayed on site• a copy of the work permit is displayed in the Control Room. The other copy

    shall be distributed at the Permit Controller’s discretion.• the worksite is examined when work is suspended, and prior to the restarting

    of work leading to completion and return to normal operations, to ensure thatit is in a safe and acceptable condition.

    • the handover procedure is properly followed and endorsements or transfer ofresponsibilities are completed within the first hour of shift change

    • any precautions and isolations have been withdrawn and the systemsreturned to normal operation prior to closing / cancelling a work permit

    • the work permit and associated certificates have been signed off by thePermit Applicant before the work permit is closed and the equipment isreturned to service

    • the Permit Authority is notified as soon as practicable of the suspension orcompletion of work, or any other fact relevant to safety, or when equipmenthas been returned to service

    • the Work Permit Register is updated on a continuous basis, recording allnecessary details of work permit issued and closure / suspension

    13.3 Permit Applicant

    The Permit Applicant shall ensure that:• the work permit and supporting certificates are raised and presented for

    • nominated Firewatchers and Standby Men have been certified

    13.4 Worksite Supervisor

    The Worksite Supervisor shall ensure that:• he has detailed working knowledge of the procedures associated with his

    work activity• he does not start any work requiring a work permit until one has been

    authorized and issued• he fully understands the limitations and restrictions of the work permit in

    order that work may proceed safely• all members of the work party adhere to safe working practices, are fully

    conversant with the limitations / restrictions / hazards involved, and allprecautions specified by the work permit are taken

    • the work permit is prominently displayed at the worksite at all times duringthe work

    • he does not leave the worksite while work is ongoing• the work permit is endorsed as necessary and responsibilities are passed to

    the oncoming Worksite Supervisor at shift change handover• the work described on the work permit is completed where possible, and the

    worksite / equipment is left in a safe condition when work is not completed• he signs off the work permit on completion of the work and passes it to the

    Permit Applicant for completion

    13.5 Authorized Electrical Person (AEP)

    He is responsible for raising all work permits and supporting certification forelectrical equipment, and can isolate / de-isolate electrical equipment up to andincluding high voltage equipment.

    13.6 Competent Electrical Person (CEP)

    He shall be responsible for:• performing isolation and de-isolation duties on low voltage equipment when

    authorized by a work permit• accepting an electrical work certificate for work on low and high voltage

    equipment

    Note: A CEP cannot issue a work permit or associated certificate of any type. Hecan endorse the relevant section on a PRC. CEPs from the Instrumentsection can raise a work permit specific to their discipline.

    13.7 Radiation Protection Officer (RPO)

    The RPO shall understand the necessary precautions to be taken for the work to beperformed, and the extent to which these precautions restrict exposure.

    He shall monitor the work of the Radiation Workers and shall remains at theworksite during performance of the work. The RPO may either be an additionto the radiographic crew or be a part of the radiography team.

    More than one RPO may be appointed and the name(s) shall be posted.

    13.8 Radiation Worker

    All personnel engaged in radiography work shall wear a personal monitoring badge(Thermal Luminescence Dosimeter TLD Badge), and should carry a personalradiation (Dosimeter) at all times during the performance of work.

    Note: All personnel engaged in radiography shall carry their license with them allthe time during the performance of their work and produce it if requested bythe Permit Authority, Permit Controller, QP RPC or Safety Inspector.Personnel not in possession of valid license shall be prohibited from usingradio active material and/or entering the controlled area.

    13.9 Authorized Gas Tester

    He shall ensure that:• gas testing equipment has been calibrated and is in reliable working

    condition prior to use.• he carries out gas tests and records the results as per the specified

    conditions of the work permit.• the Permit Controller and the Worksite Supervisor are informed immediately

    in the case of abnormal gas test results, and the work shall be stopped.

    13.10 Firewatcher

    He shall monitor the work area while Fire and Gas Detection systems are inhibited,or as required by the Permit Controller during Hot Work.

    He shall ensure that:• all flammable materials are removed from the worksite• drains remain covered and water sealed• sparks / welding spatter are contained (by use of fire blankets, water spray

    etc.)

    13.12 Work Party (Permit Users)

    Work Party members shall understand the principles of the PTW system and betechnically competent to carry out the tasks assigned to them by the WorksiteSupervisor.
