As was discussed on XX, you have not been meeting expectations in your role as a XX, particularly in the areas of XX, XX and XX. This being the case, I have outlined the following PIP that sets forth objectives that you must accomplish in order to bring your performance up to minimal acceptable standards. We request that you take immediate steps to address the deficiencies outlined in the plan below.

You have the next [30/60/90] days from today to meet these objectives. I am here as a resource for you to help you meet the above expectations. It is your responsibility to seek assistance or guidance in meeting the above expectations. In addition to meeting the specific objectives outlined in this plan during the [30/60/90] day period, and then thereafter on an ongoing basis, you must demonstrate a commitment to your job and to the Company’s objectives. If you fail to make the required improvement in the areas identified, you will [outcome varies]. As a XX, your role is critical to the success of the Company. The concerns outlined above have impeded the success of the Company. If you fail to make the required improvement in the areas identified by the end of your plan, disciplinary action will be taken up to and including termination of your employment. However, the Company reserves the right to terminate your employment during the plan period if it becomes clear you are not making sufficient progress, or for business reasons unrelated to your performance (e.g., misconduct). If you make the required improvement, you must continue to perform at a satisfactory level after the performance plan ends. Finally, please note that you are an at-will employee and that the Company may terminate your employment or change the terms of your employment at any time, without prior notice, for any lawful reason or no reason. I acknowledge these performance issues have been reviewed with me and agree to participate in this Performance Improvement Plan.
