Water fl owing through long and narrow dental unit waterlines (DUWLs) can harbour dangerous bacteria. When untreated, or improperly maintained, these contaminated DUWLs can potentially harm your patients, your staff and your practice’s reputation.Effective dental unit waterline maintenance is a key component of an infection control program. Understanding the risk involved with biofi lms and contaminated aerosols compels us to remain compliant with treatment protocols.

Criteria for choosing a dental unit waterline treatment system includes ability to control microorganisms and biofi lm at required standards, product and labour costs, safety to equipment and the environment, and most importantly, compliance to provide safe, clean dental unit water during treatment.

RCDSO Standard of Practice1: Infection prevention and control in the dental offi ceThe potential risk of infection from dental unit waterline microorganisms can be effectively reduced to counts similar to those in potable water standards by following regular waterline maintenance procedures.

For offi ces using municipal or communal water supplies

• Waterline heaters must not be used, as the heat encourages the growth of microorganisms.

• All waterlines must be purged at the beginning of each workday by fl ushing them thoroughly with water for a minimum of 2 minutes. Before purging is carried out, handpieces, air/water syringe tips and ultrasonic tips must be removed from the waterlines.

• Handpieces using water coolant must be run for a minimum of 20 seconds after patient care in order to purge all potentially contaminated air and water. The handpiece is then removed and, following cleaning and disinfection of clinical contact surfaces, another sterilized handpiece may be attached for use with the next patient.

• Sterile water or sterile saline should be used when irrigating open surgical sites and whenever bone is cut during invasive surgical procedures. Appropriate devices, such as bulb syringes or single-use disposable products, should be used to deliver sterile irrigation solutions.

For offi ces using closed or other water delivery system

• The manufacturer’s instructions related to dental units and equipment must be followed for daily and weekly maintenance.

Devastating incidents in the U.S. bring on new legislationDon’t wait for a headline in Canada. Get ahead of the curve with best practices

In 2016, a major infection control breach in an Anaheim, CA pediatric dental clinic resulted in over 70 children being hospitalized and treated for Mycobacterium (NTM) infections following pulpotomies (baby root canals). Per the Orange County Department of Health, the cause was positive laboratory results for Mycobacterium in the dental clinic’s internal water system.1

Amended State of California Dental Practice Act Assembly Bill No. 1277 was developed and signed on October 2, 2017 in response to these major infection control breaches in Anaheim.

This bill is designed to reduce the risk of creating an opportunity for infection during procedures that expose dental pulp.

Effective January 1, 2019, the bill calls for an important change to the minimum standards for infection control for dental procedures.

“… would require the board to amend regulations […] to require water or other methods used for irrigation to be sterile or contain recognized disinfecting or antibacterial properties when performing dental procedures that expose dental pulp.”

• Typical procedures include root canals and pulpotomies (baby root canals)

• Sterile irrigation solutions, include: saline or sterile water4,5

• Sterile delivery, such as bulb syringe or sterile, single-use disposable products can be used to deliver sterile irrigation solutions4,5

• Irrigation methods containing recognized disinfecting or antibacterial properties include NaOCl, CHX, EDTA and MTAD

Multiple incidents of mycobacterial and respiratory infections, and at least one fatality from Legionnaire’s disease, have all been traced back to contaminated dentalunit waterlines.

If you perform root canals, pulpotomies, or other procedures that expose the dental pulp, making California’s legislation a best practice in your offi ce (whether required by your province or not) can help to avoid putting your patients, your staff and your practice at risk.

Every dental product and device has its own instructions for use (IFU). The role of the IFU is to state that the product has been developed, manufactured and subjected to final inspection according to legal regulations, quality norms and industry standards.

Office infection prevention education and training should include reviewing IFUs and storing them in an area accessible by all staff responsible for utilizing and maintaining these products. In addition, offices should “…develop a policy and procedure stating their facility will not evaluate, purchase or trial a product (device) until the manufacturer’s instructions have been provided. Then offices need to review them to see if they can comply with all of the instructions or if additional information is needed1.”

Compliance with IFUs is not optional.

Failure to follow manufacturer’s instruction for use puts patients, staff and the practice at risk.
