1. The patient has the right to considerate &respectful care, irrespective of socio-
economic status.
2. The patient has the right to obtain fromhis physician complete current information
concerning his diagnosis, treatment andprognosis in terms the patient canreasonably be expected to understand.When it is not medically advisable to givesuch information to the patient. The
information should be made available toan appropriate person in his behalf. He hasthe right to know by name or in person, themedical team responsible in coordinatinghis care.

3. The patient has the right to receive fromhis physician information necessary to giveinformed consent prior to start of any
procedure and or treatment. Except inemergencies, such information for informedconsent should include but not necessarilylimited to the specific procedure and or
treatment, the medically significant risksinvolved, and the probable duration ofincapacitation. Where medically significantalternatives for care or treatment exist, or
when the patient requests informationconcerning medical alternatives, thepatient has the right for such information.The patient has also the right to know the
name of the person responsible for therocedure and/or treatment.

4. The patient has the right to refusetreatment / life-giving measures, to the
extent permitted by law and to beinformed of the medical consequenceof his action.
5. The patient has the right to everyconsideration of his privacy concerninghis own medical care program. Case
discussion, consultation, examinationand treatment are confidential andshould be conducted discreetly. Thosenot directly involved in his care musthave the permission of the patient to be

6. The patient has the right to expect thatall communication and records pertaining
to his care should be treated asconfidential.
7. The patient has the right that within its
capacity, a hospital must make reasonableresponse to the request of patient forservices. The hospital must provideevaluation, service and or referral asindicated by the urgency of care. When
medically permissible a patient may betransferred to another facility only after hehas received complete informationconcerning the needs and alternatives tosuch transfer. The institution to which the
patient is to be transferred must first have

information as to any relationship of the
hospital to other health care and to other
health care and educational institutions in
so far as his care is concerned. The patient
has the right to obtain as to the existence of
any professional relationship amongindividuals, by name who are treating him.
9. The patient has the right to be advised if
the hospital proposes to engage on orperform human experimentation affecting
his care or treatment. The patient has the
right to refuse or participate in such
research projects.

10. The patient has the right to expectreasonable continuity of care; he has theright to know in advance what
appointment times the physicians areavailable and where. The patient has theright to expect that the hospital will providea mechanism whereby he is informed by his
physician or a delegate of the physician ofthe patients continuing health carerequirements following discharge.

receive an explanation of his bill regardlessof source of payment.
12. The patient has the right to know whathospital rules and regulations apply to his
conduct as a patient.

Nurses have the right to practice in amanner that fulfills their obligations to
society and to those who receive nursingcare.
Nurses have the right to practice inenvironments that allow them to act in
accordance with professional standardsand legally authorized scopes ofpractice.
Nurses have the right to a workenvironment that supports and facilitatesethical practice, in accordance withthe Code of Ethics for NurseswithInterpretive Statements.

Nurses have the right to freely andopenly advocate for themselves andtheir patients, without fear of retribution.
Nurses have the right to faircompensation for their work, consistentwith their knowledge, experience and
professional responsibilities. Nurses have the right to a work
environment that is safe for themselvesand for their patients.
Nurses have the right to negotiate theconditions of their employment, either asindividuals or collectively, in all practicesettings.

I have the right to be treated as a livinghuman being until I die.

hopefulness, however, changing its focusmay be.

however changing this might be. I have the right to express my feelings and
emotions about my approaching death inmy own way.

death.
