FORM R1a APPLICATION FOR A SCIENTIFIC PROCEDURES PREMISES LICENCE (SCHOOLS) This form is to be used to apply for a Scientific Procedures Premises Licence to renew a licence expiring during the 2018-19 financial year. Premises details – licensed premises variations will not be processed at renewal. If variations to premises are planned, a variation form must be submitted separately. Further information - is provided in the Explanatory Notes on page 7 and 8 of this application. The licence renewal form includes a new (optional) section: Administrative functions authorisation – a table has been added to the form (Part G) to allow the licence holder signatory the ability to delegate specific administrative functions to specific positions. This is aimed at reducing the administrative burden on the licence nominee and assist in managing the licence effectively. Only the listed authorisations are available for delegation and the licence holder signatory must select the relevant functions and sign the authorisation for it to be accepted. Updated May 2018

Scientific Procedures Premises Licence (Schools)A Scientific Procedures Premises Licence (SPPL) authorises the licence holder to use animals for teaching in Victoria, under the approval of the Victorian Schools Animal Ethics Committee. Return to form

The applicantThe applicant is the entity that owns the school and will be the licence holder (e.g. natural person, or body corporate). Examples of incorporated bodies are:

an Australian-registered company (quote ACN); an association incorporated under Australian State or Territory incorporation legislation (quote

Incorporated Association registration number and state where incorporated); An institution incorporated by statute of an Australian Parliament (quote the relevant Parliament and

Licence NomineeThe Licence Nominee is the individual nominated by the licence holder to be legally responsible for animals used for teaching, and for any breeding of specified animals. This person must have a senior position in the school, and may be the school head or head of science. Return to form

Person authorised to sign on behalf of the applicantBody corporate: The application form is signed on behalf of the applicant by its administrative head (e.g. CEO of the company that owns the school, school president, principal, diocese head). Space is provided for 2 signatories, for the case where an institution requires more than one officer to sign on its behalf.

Natural person: If the applicant is a natural person they must sign Part B Applicant Declaration. Return to form

Specified animalsSpecified animals are defined as, mouse or rabbit and rat other than a rat, mouse or rabbit bred in its native habitat; guinea pig; and non-human primates.

Probity detailsIn granting a Scientific Procedures Premises Licence, the Department Head may consider whether the applicant and office bearers are fit and proper persons to exercise control over a licence, whether the applicant has committed an offence under the Prevention of Cruelty to Animals Act or the regulations, and any other relevant matter. Registered addressThe registered address for a body corporate is the address that is registered with the body through which the body corporate is established. For an Australian company, the registered address is the one registered with the ASIC. Return to form
