We ask you to register with our service. Contact details are on the back of this leafl et. Once you are registered you can contact the Parkinson’s nurses by phone, post or email.

Where necessary, the team is joined by physiotherapists, occupational therapists, speech and language therapists, dietitians and members of the social care team. We work with them to ensure you get the most appropriate care and support based on your individual needs.

Who are we?The Parkinson’s Nurse Specialists are experienced nurses who specialise in the care of people affected by Parkinson’s. They are part of a multi-disciplinary neuro team that acts as a link between you, your GP, your consultant and other services.

Who is eligible for our Service ?Any person affected by Parkinson’s who is registered with a GP within the mid Surrey area can receive help from this service.

What do we do?If you have recently been diagnosed with Parkinson’s we can provide:

• Support and information about Parkinson’s and the treatments available

• Specialist nursing assessment and advice on howto manage your Parkinson’s

• Links to other services that can help you

• Ongoing support for people affected by Parkinson’s, and their relatives and carers.
