Overview

The state proposed the development of 8 medical, psychiatric and behavioral support teams to serve all 254 counties of Texas, including all 39 LIDDAs and community waiver providers within a designated region.

Eight Region Configuration

Purpose of the Transition Support Teams

Provide Medical, Behavioral, and Psychiatric Support to

Local Intellectual and Developmental Disability Authorities (LIDDAs)

Home and Community-based Services (HCS) program providers

Texas Home Living (TxHmL) program providers

Purpose of Transition Support Teams

Assist providers and LIDDAS serving individuals with IDD who are:

At risk of being admitted to institutional setting

or

Transitioning from institutional setting back into the community

* Institutional Setting = nursing facility (NF) or state supported living center (SSLC)

*We do not provide direct services, we assist in finding resources in evidence based practices to enhance the supports already in place or to identify supports to enhance the continuity of services and/or transition of the individual.

Case Reviews-cases that involve medical problems, behavioral challenges and psychiatric treatment.

-3 day response with acknowledgment of receipt and if required, request for more documentation

- Conclude with a written report and follow-up instructions provided within 2 work weeks.

Provided the opportunity for each LIDDA to have Lippincott to provide the most current evidenced-based material that is consistent with best practices.

Tarrant County Case Review

CSCT (Cross-Systems Consultation Team)meetings held weekly for case reviews.

Referral forms have been provided for each LIDDA and the area providers.

Continued collaboration with START, Continuity of Care, Enhanced Service Coordination, and Diversion Coordinator.

We have received and participated in a total of 27 case referrals.

The referral form was integrated into our website to provide easy access to providers and LIDDA staff.

The majority have come from LIDDA employees, but we have recently received a few directly from providers.

The response to each referral has varied, due to a wide range of presenting problems.
