Promotion Dates:The Wisconsin Dental Association Tongue n Cheek
patient e-newslettersubscription promotion (the Promotion) is
sponsored by the Wisconsin Dental Association Inc.
(the WDA). Thepromotion begins at 12:01 a.m. Central Time (CT)
on August 7, 2014(Promotion Start Time) and ends at 11:59 p.m.,
Central Time (CT)on August 14, 2014(Promotion Cut-off).The Packers
preseason football game takes place in Green Bay, Wis. onAugust 22,
2014. The Promotion is subject to these Rules as well as to all
applicable federal,state and local laws and regulations.
Eligibility: Promotion is open to legal U.S. residents who are
18 years of age or older as of thedate of entry who possess a valid
account, except employees and immediate family members(spouse,
parent, child, sibling and their respective spouses, regardless of
where they reside) ofWDA employees, affiliated companies or
subsidiaries or employees of participating promotionsponsors,
organizations, companies or businesses are not eligible to win.
This Promotion does
not involve gambling of any kind and no entry fee, purchase or
other consideration is required toenter. The offer to participate
in this Promotion is void where prohibited by law.

will need to provide the promotion with permissionto your name and
email address. By subscribing, you accept these official rules.
Onesweepstakes entry is allowed per person throughout the duration
of the promotion.
Prize:One (1) winner will be selected in a random drawing (the
Drawing) for a chance to winthe prize (the Prize), which consists
of one (1) set of four (4) tickets to the August 22, 2014Packers
preseason football game. Each Prize package is valued at
approximately $320.
All decisions on all matters pertaining to the Drawing are
final. WINNERS ARERESPONSIBLE FOR TAX AND OTHER FEES ASSOCIATED
WITH PROMOTION. WDAwill not cover transportation or any other costs
related to this Promotion.
Winner Selection: A random selection of the winner will occur on
or about August 15, 2014from among all eligible entries received
(Those who subscribe to the Tongue n Cheek andTeeth, too! patient
e-newsletter). Winners must be subscribed to the Tongue n Cheek
e-newsletter by 11:59 p.m. CT on August 14, 2014. Winners will be
announced on the WDAFacebook page Wall. The potential winnerwill be
contacted by the WDA and have by 8 a.m.CT by August 19, 2014 to
provide their full name, a daytime phone number and current
addressto claim their prize. The Sponsor will notify the winners
how to claim their Prize.
No Prize transfer, assignment or substitution by winners
permitted. If any potential winnerdeclines the Prize, fails to
claim the Prize, is unavailable for Prize fulfillment, fails to
abide bythe Official Rules, or is ineligible, Sponsor may select an
alternate winner from all remainingeligible entries.

RESERVES THERIGHT TO DISQUALIFY SUCH INDIVIDUALS MAKING SUCH AN
ATTEMPT, VOIDHIS/HER ENTRIES, AND SEEK DAMAGES FOR ANY SUCH
INDIVIDUALS TO THE
FULLEST EXTENT PERMITTED BY LAW. The Wisconsin Dental
Association, Inc. is notresponsible for (i) electronic
transmissions that are inaccurate or were not successfully
received;(ii) any problems or technical malfunctions, errors,
interruptions, deletions, defects, delays inoperation or
transmission, communication failures or human error that may occur
in thetransmission, receipt or processing of entries, or for
destruction or unauthorized access to, oralteration of, entries;
(iii) failed or unavailable hardware, network, software or
telephonetransmissions, damage to entrants or any persons computer
and/or its contents, or causes
beyond the Promotion Parties reasonable control that jeopardize
the administration, security,
fairness, integrity or proper conduct of this Promotion; or (iv)
any other errors or problems ofany kind relating to or in
connection with the Promotion, whether computer, network,
technical,mechanical or otherwise, including, without limitation,
errors or problems which may occur in
connection with the administration of the Promotion, the
processing of entries, the announcementof the winner or in any
Promotion-related materials. Sponsor reserves the right to cancel,
modifyor terminate the Promotion if fraud, misconduct or technical
failures destroy or threaten theintegrity of the Promotion, or if a
computer virus, bug or other technical problem or unauthorizedhuman
intervention corrupts the administration, security or proper
conduct of the Promotion asdetermined by the Promotion Parties, in
their sole discretion. In the event of early termination,Sponsor
may, at its sole discretion, select the potential winner randomly
from all eligible, non-suspect entries received as of the time/date
of such termination.
Additional Rules & Restrictions: By participating in this
Promotion, entrants agree to abide byand be bound by these Rules
and by all decisions of the WDA, which decisions shall be final
andbinding in all matters relating to this Promotion. In the event
the winner is found to be inviolation of any of these rules, he/she
will be required to forfeit the Prize. Acceptance of thePrize
constitutes permission for the WDA and its designees to use winners
name, city and state
of residence and/or likeness for purposes of advertising and
trade in any and all media (now orhereafter known) worldwide in
perpetuity, without limitation or further compensation,notification
or permission, unless prohibited by law. By participating, entrants
assume allliability in connection therewith and further agree to
release and hold the WDA and all otherparties involved with the
Promotion harmless from and against any and all claims, damages
andliability of any kind arising from or in connection with, in
whole or in part, directly or indirectly,the acceptance, possession
and/or use or misuse of the Prize and/or participation in
thePromotion.
