NOMINATION MATERIALS Nomination must include: 1. This completed Nomination Form (preferably typed); 2. Nomination Statement citing specific accomplishments, examples, contributions, and service (one page maximum); and 3. Three References with contact information. References should represent a diverse group of colleagues/customers (faculty, staff, students, or other individuals) who have personal knowledge of and who have benefited from the nominee’s service. SUBMISSION Submit the completed materials by 5:00pm on Friday, March 20, 2020 via email to [email protected] or hand deliver to Human Resource Services (Churchill Hall, Room 159). QUESTIONS Please direct questions to [email protected]. AWARD Recipients are honored at the End of Year Celebration Breakfast. Each recipient receives a $750 cash award and a glass trophy. ELIGIBILITY All current staff with a minimum of three years of continuous service at Southern Oregon University in a regular non-faculty position may be nominated. Previous recipients of the Outstanding Staff Award are ineligible for nomination.
