RELEASE AND WAIVER OF LIABILITY AND IMPORTANT INFORMATION
READ BEFORE SIGNING
Participants must read and complete the Release and Waiver of Liability Agreement in order to compete in the Ormond Beach Senior Games. In consideration of being allowed to participate in any way in the Ormond Beach Senior Games program, its related events and activities: I, the undersigned acknowledge, appreciate, and agree that:
(1) The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and/or death, and while particular skills, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist; and (2) I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING
FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and (3) I willingly agree to comply with the stated and customary terms and conditions for participation. If,
however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of Ormond Beach Senior Games or their representative immediately; and, (4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE,
INDEMNIFY, AND HOLD HARMLESS THE ORMOND BEACH SENIOR GAMES, their officers, volunteers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers and, if applicable, owners and leases of premises used for the activity ("Releases"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I FURTHER AGREE THAT:
(1) I CONSENT TO ALL EMERGENCY MEDICAL TREATMENT as may be deemed appropriate under existing circumstances by medical personnel or personnel associated with Ormond Beach Senior Games.
(2) I will ALLOW my PHOTOGRAPH, PICTURE or LIKENESS and/or VOICE to APPEAR in any official documentary, promotional (including any and all advertising), television, radio, social media or film coverage of the Games, WITHOUT COMPENSATION.
(3) I UNDERSTAND THAT I MAY BE REMOVED AS A PARTICIPANT IF I VIOLATE ANY RULES OF THE CODE OF CONDUCT as determined by the Ormond Beach Senior Games Committee or Event Managers.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
Ormond Beach Senior Games reserves the right to cancel or postpone events in case of inclement weather or other extenuating circumstances.

Mailed entries must be postmarked by Friday, October 4, 2019, to be accepted.
Late registrations are at the discretion of the Event Managers and May Not Be Allowed. Late registrants may not receive a T-shirt.
