1. INTRODUCTION & GENERAL OVERVIEW The staff committed to working with you at Pathways includes psychiatrists, nurse practitioners, social workers, counselors, nurses, case managers, and a multitude of additional support professionals. During your treatment with Pathways you may be involved with one or more of these individuals who will collaboratively develop a treatment plan with you to work on addressing barriers and accomplishing your goals. The overall goal of our individualized treatment is to help you identify and cope more effectively with problems you may be experiencing, allowing you to accomplish the kind of daily life you desire to lead. This may be accomplished through a combination of individual therapy, medication management, and many other support and community services. To facilitate the most effective treatment process, it is your responsibility to provide accurate and complete information during all phases of evaluation, treatment planning, treatment, and termination of services. Effective treatment requires you to play an active role in your individualized treatment work with our staff including the development of goals, application of skills and completion of treatment assignments, and regularly assessing your own progress. We are on your team, and for us to effectively serve you it is necessary that you fully and honestly engage with your treatment team staff. Entering treatment at Pathways assumes you agree to these responsibilities. Appointments vary in length and frequency depending on your personal and individualized needs and goals, and the professional and support staff with whom you are working. Most therapy sessions are one hour in length, appointments with medical providers are usually between 15 to 45 minutes, and length of sessions with support staff can vary widely based on the nature of the service. You may discontinue treatment at any time. Our hope is that treatment is discontinued when you and your treatment team have accomplished your work together. If this is not the case, you are encouraged to discuss this decision with your Pathways staff prior to discontinuing treatment. It is important that you know your treatment staff are available for you on an outpatient basis, but are not considered to or capable of providing crisis services outside of scheduled appointments. To ensure that you are able to receive the care and service you need during any crisis or emergency, call our 24-hour Crisis Line at 1.800.372.0693 (or 911) and not your outpatient treatment team members. 2. DESCRIPTION OF SERVICES Based on your initial evaluation (or what is often referred to as an “Intake,”) your clinician will discuss individualized treatment recommendations and options with you. These may include ongoing therapy, a referral to one of Pathways’ medical providers for consideration of medication(s), recovery services, case management services, or a referral to your primary care physician, a support group in the community, another community agency, and a host of other services and resources to support your treatment. Some primary services offered by Pathways include but are not limited to:

THERAPY OR COUNSELING

Therapy or counseling involves individual, couple, family and group counseling by one of our mental health professionals. These individuals use professional skills, knowledge, training, and experience to assist you in identifying the mental health or co-occurring (complex) problems you are experiencing as well as the strengths and capabilities you already have to address them. Working together, a plan will be developed that identifies individualized goals and steps toward achieving them that put your strengths and resources to use, and create the steps to take to address or resolve these problems. The frequency and duration of sessions will be discussed with you and will be a part of your treatment plan. Our staff uses many different evidence-based treatment modalities based on your specific needs and strengths.

MEDICATION EVALUATION & MANAGEMENT

This service involves the use of medication(s) to treat symptoms of various mental health diagnoses and can be an important part of treatment. You may be referred to one of Pathways’ medical providers to receive an evaluation and potentially consider the use of medication(s) as part of your treatment or recovery. If medication(s) are recommended, you will be informed of the potential benefits, risks, and side effects of the prescribed medication(s) and have the

ability to agree or refuse to include the medication(s) as an intervention in your individualized treatment plan. The medical provider will evaluate your ongoing response to the medication(s), and adjustments or changes to your medication regimen will be made as needed. It is extremely important for your own health and well-being that you take any medication(s) as prescribed and do not self-adjust, add, or discontinue any medication without first contacting Pathways and speaking with your medication provider. Please report any side effects to our medical staff immediately, and promptly keep your Pathways medical provider informed of any prescribed or over-the-counter medication(s) additions or changes.

CASE MANAGEMENT

Case Management is a comprehensive service that aims to enhance treatment effectiveness and outcomes with the goal of maximizing recovery and resilience. The support offered through Case Management assists clients in connecting to their own strengths and resources in their own environments that can considerably improve the quality of their lives. Case Management professionals utilize best practices to support their clients in reintegrating into society comfortably and with an increasing emphasis on healthy self-reliance. Case managers prioritize the biopsychosocial-spiritual needs of clients and work to ensure a coordinated system of care across service providers that address all of these areas.

TENNESSEE HEALTH LINK

Through better coordinated behavioral and physical health services, the Tennessee Health Link program is meant to produce improved member outcomes and greater flexibility when it comes to the delivery of appropriate care for each individual covered by TennCare. The program is built to encourage the integration of physical and behavioral health, as well as, mental health recovery, giving every person a chance to reach his or her full potential for living a rewarding and increasingly independent life in the community.

EMERGENCY & CRISIS SERVICES

Crisis and Emergency services are available twenty-four hours per day, seven days per week, every day of the year through Pathways’ Crisis Line and Walk-In Triage Center. Pathways’ mental health crisis professionals are available to provide on-site, telehealth, and mobile crisis evaluation services in our contracted catchment areas in West Tennessee. They partner with our 24-hour Walk-In Triage Center professionals to provide immediate assessment and support in determining the best course of action to meet the psychiatric, mental health, substance-use, or co-occurring emergency needs of individuals who present in crises of all kinds.

CRISIS STABILIZATION UNIT

One of Pathways’ 24-hour services, the Crisis Stabilization Unit (CSU) offers twenty-four-hours-per-day, seven-days-per-week intensive, short-term stabilization services and behavioral health treatment for individuals who do not meet the criteria for psychiatric hospitalization, but based on assessment will benefit from social detoxification and behavioral health support services in a controlled, therapeutic environment. Pathways’ CSU provides assessment, triage, medication management, group and individual therapy, as well as an opportunity for clients to work with a wellness recovery consumer specialist.

INPATIENT PSYCHIATRIC UNIT

One of Pathways’ 24-hour services, the Inpatient Unit (IPU) offers a therapeutic environment twenty-four-hours-per-day, seven-days-per-week for individuals who require short-term, acute care for severe psychiatric and mental health presentations. This service offers a specialized team of nurses, social workers, psychiatric technicians, and additional support staff supervised and directed by a Psychiatrist. Pathways’ IPU provides psychotherapeutic care for individuals and as able in concert with their families to meet participants’ psychological, medical, and rehabilitative needs to facilitate a successful and healthy return to a supportive community as rapidly as possible.

RECOVERY & CO-OCCURRING SERVICES

From Recovery Navigators who work directly out of emergency departments helping link individuals to treatment services, to our inpatient detoxification and stabilization units, to intensive outpatient and outpatient services, Pathways recognizes that most individuals are working to excel while living with both a substance use disorder and a co-occurring mental health diagnosis. These individuals are welcomed at Pathways, and we have the experience and

skill across a host of service areas to support them and their families as they work toward their goals while living with complex diagnoses. When individuals are dealing with substance use or complex disorders, we are committed to preventing costs of treatment becoming a concern. As many of our services are funded by grant programs in addition to state and private insurance, please be open and upfront about your finances and situation so we can ensure cost doesn’t come between you and the excellent treatment we can provide. For all our substance use and co-occurring (complex) treatment programs and services, priority preference is given to the pregnant women with substance use disorders, and those with an IV-related substance use disorders.

RECOVERY NAVIGATOR PROGRAM

In partnership and funded by the Tennessee Department of Mental Health & Substance Abuse Services, the Recovery Navigators are Certified Peer Recovery Specialists (CPRS) who have at least two years of sobriety following a history of wrestling with substance use disorders. They answer and respond to calls from Emergency Department staff in West Tennessee when individuals present with any kind of substance use issue or need, and begin a long-term, peer-to-peer relationship with them to assist in securing treatment and other needed services.

INPATIENT DETOXIFICATION UNIT

One of Pathways’ 24-hour services, the Inpatient Detoxification Unit offers a twenty-four-hours-per-day, seven-days-per-week concentrated and structured hospital-based program which may include individual and group counseling and recreational therapy sessions as well as medical management services. These individualized services provided are based on the needs and responses of each individual with respect to the severity of their symptoms. Treatment is tailored to assist the individual in regaining a higher level of functioning. Services are directed at providing stabilization of substance use withdrawal symptoms, preventing deterioration, and facilitating healthy reintegration into the community with an identified aftercare plan.

INTENSIVE OUTPATIENT PROGRAM

With a level of intensity that can assist individuals who have completed or do not yet need residential or inpatient treatment but are not yet prepared for occasional outpatient supports for their sobriety, Pathways’ Intensive Outpatient Program offers group counseling three hours per day for 3 days a week over a course of eight weeks. Family therapy and additional individual counseling are sometimes additional components to this program, and upon completion aftercare services are available to assist in the transition back to less frequent supports. This service is offered both in person and via telehealth options in other locations around the Pathways service area.

MEDICATION ASSISTED RECOVERY As Pathways continues to follow medical science and research, we are committed to best practices for the treatment of substance use disorders over any personal or individual ideological approaches. To this end, and recognizing the individualistic nature of treatment, our Recovery team works with each individual client to determine collaboratively the best route of treatment for that client at that time. We can bring together a host of Pathways’ Recovery services to offer care on the continuum between an abstinence-only approach to a medication-assisted abstinence approach. Our providers are licensed and waivered to provide Buprenorphine products and naltrexone products in accordance with state and federal regulations and as one part of a holistic approach to the treatment of substance use and co-occurring (complex) disorders.

3. TITLE VI OF THE CIVIL RIGHTS ACT As a recipient of Federal financial assistance, Pathways of Tennessee, Inc. does not exclude, deny benefits to, or otherwise discriminate against any person on the ground of race, color, national origin, or on the basis of disability, age, religion, sex, or sexual orientation in admission to, participation in, or receipt of the services and benefits under any of its programs and activities. This statement is in accordance with the provisions of Title VI of the Civil Rights Act of 1964. Persons wishing to file a complaint may do so in writing on a form supplied by the civil rights coordinator of West Tennessee Healthcare. 4. INFORMED CONSENT FOR TREATMENT We want you to be well-informed and well-equipped to determine what treatment is right for you, and what treatment you want to participate in at Pathways. You deserve to know the training, skills, and scope of the services your Pathways treatment team are equipped to provide you, so our counselors and case managers will review their credentials with you prior to

beginning a treatment program. You deserve to know any potential side effects, benefits, and risks of any medication prescribed to you, so our medical providers will review these any time they prescribe a new medication. As you know, it takes courage to be involved in a treatment program as your participation may have many benefits, but it may also involve the risk of remembering painful events which can arouse intense emotions and physical sensations of fear or anger. Other feelings like anxiety, depression, frustration, loneliness, or helplessness may also be aroused during this process. In some instances, your condition could worsen as a result of the process of counseling, and drastic lifestyle changes might occur that could negatively affect your relationships and ability to cope. The benefits of treatment may be that you will be better able to handle or cope with your family, your work or school, your social relationships and community, and that you may have a better understanding of yourself or your goals and values which could lead to growth as a person. It’s important to know there is no guarantee of positive results, and we want you to have all the information to make decisions about your treatment on the front-end. 5. PERMISSION TO PROVIDE TREATMENT All treatment services provided by Pathways are considered voluntary services only provided with the written permission of the patient or legal guardian. 6. RIGHTS AND RESPONSIBILITIES Pathways is aware of and takes seriously the Rights and Responsibilities you have as a person receiving services with us. Join us in seeking to uphold these rights and responsibilities together to ensure the best chance for treatment to be successful. The Rights and Responsibilities form is included in this Handbook. 7. NOTICE OF PRIVACY PRACTICES

Our Privacy Practices are important for you to read and understand as they help guide us in protecting your privacy and confidentiality. This Notice of Privacy Practices, required by the Health Insurance Portability and Accountability Act (HIPAA), is included in its entirety with this Orientation Handbook. If you have any questions about these practices, please contact our Health Information Management Department at 731.541.8200. 8. CONFIDENTIALITY Your privacy and the confidentiality of your mental health information and substance use history is respected and valued by all of our staff. The information you provide and discuss with your treatment team is considered confidential and will not be released to others without your permission except for the following circumstances: an emergency in which there is a danger to you (including medical emergencies) or others, if required by law (such as in the case of suspected abuse of a child or dependent adult, in response to a court order, for accreditation purposes, or for seeking payment for your services from your insurance company or other grant funding program. Staff members may also discuss your case as a part of supervision and training. You may be asked to sign an authorization to release information so that we may speak with others involved in your life about treatment issues, and you have the right to revoke that permission in writing at any time. 9. CONSENT TO PHOTOGRAPH We may request that you allow us to photograph you to ensure accurate identification. With your consent, the photograph will be uploaded to our Electronic Health Record only and will only be used for identification purposes. Pathways will retain all ownership rights to these photographs. 10. ADVANCE DIRECTIVE & MENTAL HEALTH DECLARATION An Advance Directive explains, in writing, your choices about the treatment you want or do not want or about how health care decisions will be made for you if you become too ill to express your wishes. An Advance Directive reflects your personal beliefs and values. When you make one, you will consider issues like dying, being kept alive on machines and the quality of life and independence you want. An Advance Medical Directive makes it possible for your wishes to be carried out during a serious illness. If you are an adult and of sound mind, you can make an Advance Directive. A Tennessee mental health and developmental disability law gives the right to individuals 16 years of age and over to be involved in decisions about their mental health treatment. The law also recognizes that, at times, some individuals are unable to

make treatment decisions. A Declaration for Mental Health Treatment allows persons receiving services to plan ahead. It may also assist service providers in giving appropriate treatment. The Declaration for Mental Health Treatment form describes what a person wants to occur when he/she receives mental health treatment. It describes mental health services that a person might consider, the conditions under which the Declaration may be acted upon, and directions on how a person can revoke a Declaration. For example, completion of a Declaration for Mental Health Treatment form allows you to state:

• Conditions or symptoms that might cause the Declaration to be acted upon; • Medications you are willing to take and medications you are not willing to take; • Specific instructions for or against electroconvulsive or other convulsive treatment; • Mental health facilities and mental health providers which you prefer; • Treatments or actions which you will allow or those which you refuse to permit; and • Any other matter pertaining to your mental health treatment which you wish to make known.

Please let a staff member know if you have an Advance Directive or Declaration for Mental Health Treatment if you would like more information. 11. IMPORTANCE & IMPLICATIONS OF ATTENDANCE Attending scheduled appointments and following through with suggestions, recommendations, skill application, or homework assignments is an important part of the treatment process. Because of this, it’s expected that you attend all scheduled appointments, or notify us 24 hours in advance if you will be unable to attend and need to reschedule your appointments. In order to meet the large need of individuals seeking care in communities, if an individual misses three appointments without prior notification, they may only be seen on a “work-in” basis and not be given a dedicated appointment time until a commitment to the treatment process is developed. For prescribing policies around missed appointments, see item 14. 12. DISCHARGE POLICY Upon successful completion of treatment or if we have no contact with you for at least 90 days, Pathways will close your case in our system’s files. If you have an appointment scheduled in the future, your case will not be closed. If your case is closed and you desire to seek out services again at some point in the future, we will be happy to set up a new intake evaluation and reenroll you in services. 13. MISSION STATEMENT REGARDING PRESCRIPTION OF CONTROLLED SUBSTANCES It is Pathways’ intent to lead the community with respect to diagnosis and treatment of psychiatric, substance use, and co-occurring (complex) disorders. It is recognized that controlled substances have a place in the treatment of some psychiatric disorders. It is further recognized that the use of controlled substances may be associated with health risks including the development of tolerance and dependency. Therefore, the prescription of controlled substances for the treatment of psychiatric disorders will conform to protocols established by the medical staff. Patients must: (1) comply with psychosocial treatments required by controlled substance protocols and, (2) abide by the controlled substance contract. Failure to comply with (1) and (2) above may result in discontinuation of controlled substances or discharge from medication services. 14. MEDICATION REFILL POLICY To effectively assess and monitor the wellbeing of our clients, it is critical that you keep your appointments with your providers whenever scheduled, and communicate regularly with our Medication Clinic staff if you have concerns about dosage, side effects, or other medication concerns. Because of the importance of your communication with your provider, Pathways does not typically call in medication refills for individuals who have not attended their scheduled appointments. If you are sick or know that you will not be able to make a scheduled appointment, please call the Medication Clinic in advance of your so that plans can be made together.

15. CHANGING PROVIDERS Pathways has some of the best psychiatric, mental health, and substance use treatment providers around, and the relationship shared between a client and her treatment team is a big deal. While we recognize that not all individuals receiving services and treatment providers are able to work together successfully, the decision to change providers must also be balanced with the issues involved and the staff resources available. In the rare instance that you have a conflict with your therapist, case manager, or provider and want to request a change, you should first attempt to discuss the conflict with them in an effort to resolve these issues. If this is not successful, the request to change providers must be completed in writing and returned to the office where it will be added to the next staffing agenda discussion. Once a decision is made, we will contact you immediately. 16. FEES AND PAYMENT AT TIME OF SERVICE At Pathways, we want to get you the care you need without cost being a concern. While this is not always possible, we will do everything we can to work with you from the very beginning on payment options. The fee for services you receive will be established at the time of your initial visit and can be reevaluated any time changes in your financial situation occur. As the recipient of multiple grant-funded programs, there may be a way that some to all of your treatment will not require you to pay anything out of pocket. To qualify for these grant-funded programs, we are required to submit proof of income and proof of address for individuals. It’s necessary that you provide this information at your first visit as otherwise you may be charged the full cost of your service until the documentation of proof is obtained. In addition, Pathways can also offer a sliding fee scale based on specific eligibility guidelines. If you have Insurance coverage, you will need to bring your insurance card at the time of your initial appointment, and we will be happy to file all insurance claims for you. It’s important that you notify us of any changes to your insurance in a timely manner or you may be charged the full cost of your service. In order to best care for all of our clients, we do enforce a payment at the time of service policy and may not be able to reschedule further appointments until payment is received. RELEASE OF MEDICAL RECORDS If you request a copy of your records, you must complete an Authorization to Release Information form which outlines what records you are requesting and the reason for your request. You will be informed of the cost of copying your records. If you agree to pay the cost associated with copying these records, payment must be made prior to any records being copied. When an active client requests copies of their records from Pathways, there is a $6.50 fee. For attorneys, probation officers, or others to determine expense of obtaining copies of records, please contact Pathways Medical Records at 731.541.8200. We also offer the ability for you to view some of your record electronically through the Patient Portal. Online access to your information is subject to the discretion of your provider to withhold certain information. For more information about Pathways’ Patient Portal, see item 18. 18. PATIENT PORTAL Pathways participates in a Patient Portal to link our clients to certain aspects of their medical record through a software platform called UPDOX. The Patient Portal provides a secure, confidential, and easy to use way to access part of your medical record including upcoming appointment information. If you choose to offer your email when scheduling first time appointments or at other times of service at Pathways, you may receive an invitation to consent for the Patient Portal service. To initiate your participation with the Patient Portal, please ask someone on your treatment team to assist you in registering for the portal. You may choose to opt in or out of the Patient Portal in writing at any time.

19. COMPLAINTS & GRIEVANCES The quality, compassion, and timeliness of the services you receive at Pathways are important to us. The process for complaints and grievances provides an opportunity for service recipients to share and have their comments, concerns, and complaints investigated and resolved in a timely, ethical, and professional manner. All complaints will be handled in a confidential, respectful, and non-threatening manner. The process for complaints and grievances is coordinated by the Patient Advocate of Pathways. You are encouraged to contact Pathways’ Patient Advocate by calling 731.541.8277. The grievance process includes the patient’s right to appeal clinical and administrative decisions. For concerns that cannot be resolved through the organization, the client may contact the Joint Commission’s office of Quality Monitoring at 1.800.994.6610 or e-mail them at [email protected]. If the client is a Medicare beneficiary and disagrees with a coverage decision or feels they have been discharged too soon, the client has the right to appeal by contacting the Quality Improvement Organization (QIO) at 1.800.633.4227 and ask for a second opinion. 20. NO WEAPONS POLICY While we recognize that Tennessee state law allows individuals the right to carry weapons with a valid permit, as part of the West Tennessee Healthcare system, Pathways does not allow weapons of any kind on our premises. If someone comes to the office with a weapon, they will be asked to leave until she or he can return without bringing the weapon onto our premises. 21. TOBACCO-FREE ENVIRONMENT In a commitment to your health and the health of our community, Pathways and all West Tennessee Healthcare organizations are smoke, vape, and tobacco-free environments including outside, parking lots, and lawns. This policy protects everyone from exposure to the harmful effects of secondhand smoke. Thank you for not using any tobacco products, other substances, vapes and electronic cigarettes, and any other smoking or inhalation device on Pathways property, and if you are interested in quitting for your own health and the health of those around, discuss this with your medical provider for additional support and information. 22. CUSTOMER SATISFACTION SURVEYS At Pathways, we are in the business of caring for people. In an ongoing effort to improve the quality of care and patient satisfaction in the services received here, Pathways may ask that you participate in the completion of a customer satisfaction survey. While Pathways may sometimes use an independent agency to perform surveys, you are not required to participate in or complete them. Know that you will not be treated differently if you share concerns or positive feedback, and that our goal is to continuous improve the quality of care and your satisfaction. 23. TELEHEALTH Telehealth, sometimes including telemedicine, allows health care professionals and support staff to educate, evaluate, diagnose, and treat patients using a host of varied telecommunications technology including email, text and sms, phone calls, and two-way audio and video communications. When meeting with your provider, counselor, or case manager, telehealth or telemedicine permits two-way, real-time interactive communications between the client and the member of their Pathways treatment team. If you choose to consent to telehealth services, you are allowing Pathways to contact you through any telehealth means unless you specify otherwise at the time of giving telehealth consent. You may change or revoke consent to telehealth in writing at any time. 24. FOR TENNCARE RECIPIENTS ONLY

Pathways will share information regarding your participation and progress in treatment with your specific Managed Care Organization (MCO), and if applicable, with any grant program funder.

Every child and teen needs regular health check ups, even if they seem healthy. These visits help your doctor find and treat problems early. In TENNderCare, check ups for children are free until they reach age 21. TENNderCare also pays for all medically necessary care and medicine to treat problems found at the check up. This includes medical, dental, speech, hearing, vision, and behavior or mental health problems. If your child hasn’t had a check up lately, call your PCP today for an appointment. Ask for a TENNderCare check up. You can go to the PCP or the Health Department to get TENNderCare check ups.

As transportation can be a difficult barrier to seeking treatment services, it’s important you know as a TennCare member, you or your child can call to schedule a FREE ride to provider offices like Pathways for needed health services.
