What is organic mental disorder ?
• กลมโรคทางจตเวช ทอาการเกดขนโดยมสาเหตจากการทางานของสมองทผดปกต
อยางชดเจน เชน หลงจากทม อบตเหตทศรษะ จากแอลกอฮอล โรคตดเชอ ภาวะเม
ตาบอลกทผดปกต หรอการเสอมจากวย
• โดยกลมโรคทเกดขนน อาจเปนอยในระยะสนๆ หรอ อาจเปนไปตลอดชวต

Clinical Features of Delirium • Acute onset
• Occurs abruptly, usually over a period of hours or days
• Fluctuating course
• Inattention
• Difficulty focusing, sustaining, and shifting attention
• Difficulty maintaining conversation or following commands
• Disorganized thinking
• Manifested by disorganized or incoherent speech
• Rambling or irrelevant conversation or an unclear or illogical flow of ideas

Clinical Features of Delirium • Altered level of consciousness
• Clouding of consciousness, with reduced clarity of awareness of the environment
• Cognitive deficits
• Typically global or multiple deficits in cognition, including disorientation, memory deficits, and language impairment
• Perceptual disturbances
• Illusions or hallucinations in about 30 percent of patients

Clinical Features of Delirium • Psychomotor disturbances
• Hyperactive - Marked by agitation and vigilance
• Hypoactive - Marked by lethargy, with a markedly decreased level of motor activity
• Altered sleep–wake cycle.
• Typically daytime drowsiness, night-time insomnia, fragmented sleep, or complete sleep-cycle reversal
• Emotional disturbances
• Manifested by intermittent and labile symptoms of fear, paranoia, anxiety, depression, irritability, apathy, anger, or euphoria

เกดขนเปนผลจากภาวะทางกาย ยา สารเสพตด สารพษ หรอหลายสาเหตรวมกน
การวนจฉยแยกโรค • โรคสมองเสอม ความแตกตางคอ onset ของโรคสมองเสอมจะเกดแบบคอยเปน
คอยไป และอาการจะคอยๆแยลง โดยระดบการรสกตวมกจะปกต จะเสยในสวน
ของความจาและ cognition
• Brief psychotic disorder ลกษณะคอมอาการทางจตเกดขนอยางเฉยบพลน แต
ความรสกตวมกจะปกต และอาการของโรคไมเปลยนแปลงตามเวลาของวน

• อาการของสมองดานอนๆ
- ดานการใชภาษา : ระยะแรกมกมปญหาในการเรยกชอ นกคาไมออก
- ดาน visuospatial skill : เสยทกษะดานการมองเหนทสมพนธกบความจา ทาใหบอกไมไดวาของทเหนใชทาอะไร เสยการจาทศทาง และการจาหนาคน
- ดาน insight : ผปวยไมคอยยอมรบความบกพรองของตนเอง ถามอาการมากอาจมพฤตกรรมกาวราว วนวายได สาเหตจากความบกพรองของ frontal lobe
- ดานทกษะการทากจกรรมและงานตางๆ (apraxia) ทาใหผปวยทาสงตางๆดวยตนเองไดลดลง
- ดานการวางแผนและการตดสนใจ (executive function) ระยะแรกจะยงไมรนแรง
- ดานพฤตกรรมและอารมณ: (behavioral and psychological symptom of dementia BPSD) พบมากในระยะกลางและระยะทายของโรค
Clinical course
• อาการของโรคจะแยลงแบบคอยเปนคอยไป
• มการศกษาตดตามดวย MMSE พบวาคะแนนจะลดลง 3-3.5คะแนน/ป
• ใน Early stage อาการมกขนกบ premorbid personality ของผปวย
• ใน middle-late stage จะม cognitive impairment มากขน และมกม BPSD รวมดวย
• เมออาการรนแรงมากขน จะมปญหาการกลน การขบถาย การหายใจ จนม
ภาวะแทรกซอนของโรคทางกายตางๆทเปนตนเหตใหผปวยเสยชวต
• Median survival from diagnosis 5-7 ป

ลกษณะอาการทางคลนก • ผปวยมกมปจจยเสยงของโรคหลอดเลอดสมอง เชน ความดนโลหดสง, ไขมนใน
เลอดสง, เบาหวาน, หวใจเตนผดปกต
• พบอาการผดปกตของความคด,อารมณและการรบรหลงจากเกดเกด stroke
• ความผดปกตของ cognitive function คลายกบใน Alzheimer’s disease
การดาเนนโรค • การดาเนนโรคเปนแบบขนบนได (step-wise progression)

1. ซกประวต
1.1 ประวตการปวยและการดาเนนโรค
1.2 อาการรวมทางกาย
1.3 การวตโรคในอดต, โรคประจาตว,และความเสยง
1.4 ประวตการใชยาและยาเสพตด
1.5 ประวตครอบครว
2. ตรวจรางกาย - ตองทาโดยละเอยดเพอแยกโรคทางกายทเปนสาเหต
3. ตรวจสภาพจต
4. ทาแบบคดกรองและเมนภาวะสมองเสอม เชน
- TMSE – Thai Mini Mental Status Examination
- MOCA – Montreal Cognitive Assessment
5. การสงตรวจทางหองปฏบตการ เพอหาสาเหตทรกษาได เชน
- CBC คนหาภาวะพรองวตามนB12
- Electrolyte ดระดบ Na, Ca ในเลอด
- FBS ดภาวะนาตาลในเลอดตา
- BUN/Cr ประเมนการทางานของไต หาภาวะไตวาย
- LFT ประเมนการทางานของตบ
- TSH ตรวจหาภาวะ hypothyroidism
- Serum VDRL ตรวจหาโรคsyphilis
- สงตรวจทางประสาทรงสวทยา เชน CT brain, MRI brain จะทาเมอมขอบงช
การวนจฉยแยกโรค
• ประเมนแยกโรคทมอาการคลายภาวะสมองเสอม (ประเมนวาใชภาวะ
สมองเสอมหรอไม) เชน Delirium, pseudo dementia จากdepression
• ประเมนแยกโรคททาใหเกดภาวะสมองเสอมแบบทรกษาหายขาดได เชน
normal pressure hydrocephalus, MDD, hypothyroidism, anemia, brain
tumor, alcoholism
• ประเมนแยกโรคในกลมโรคสมองเสอม (โรคทเปนสาเหต) Alzheimer’s
disease, Vascular disease, Lewy body disease, Frontotemperal
degeneration, Parkinson’s disease, HIV infection, traumatic brain injury
การรกษาผ ปวยภาวะสมองเสอม (cognition) • การรกษาโดยใชยา – ยงไมมยาทสามารถรกษาสาเหต เปนเพยงชวยชะลอการแยลง
ของอาการเทานน
• Cholinesterase inhibitor (AchE)
• Donepezil, Rivastigmine, Galantamine
• NMDA (N-methyl-D-aspatate) receptor antagonist

การรกษาผ ปวยภาวะสมองเสอม (neuropsychiatric)
• อาการทพบบอยไดแก ภาวะซมเศรา, ภาวะวตกกงวล, หงดหงด. กาวราว,
อาการโรคจต หแวว หลงผด
• การรกษาแบบใชยา
• Antipsychotic : ใชเมอมอาการโรคจต, กาวราว, วนวาย, สบสน และหยดใชเมอ
อาการหายไป (ควรใชดวยความระมดระวง)
• Antidepressant : ใชเมอมภาวะซมเศรา และยาทเพม serotonin ชวยลดอาการ
กาวราวได
• Cholinesterase inhibitor และ memantine ชวยลดอาการทางจตประสาทได
• ยาจตเวชทควรระวง เชน Anticholinergic, sedating drugs

• ชนชม ใหรางวล เมอผ ปวยทาพฤตกรรมทเหมาะสม เชน ถายปสสาวะในหองนา
ไดเอง
• เบยงเบนความสนใจของผ ปวยเมอมพฤตกรรมกาวราว

การดแลทวไป
- การตนใหผปวยทากจวตรประจาวนพนฐาน (basis ADL) ของตวเองใหมากทสด
- ดแลสงแวดลอมของทอยอาศยใหเหมาะสมและปลอดภย
- การดแลและฟนฟสภาพรางกายดานตางๆและฟนฟความสามารถในการทา
กจกรรมตางๆของผปวย
- ใหการดแลผดแล โดยการใหคาปรกษา, พดคยปรบทกขและใหกาลงใจ
(บคคลกลมนมความเสยงสงในการเกดภาวะซมเศรา)
- การดแลในวาระสดทายของชวต ควรแจงถงพยากรณโรคใหญาตและผดแล
รบทราบ และดแลภาวะทางกายเมอถงวาระสดทาย
- ใหการดแลดานกฏหมายโดยแจงการวนจฉยใหผปวยและญาตทราบเพอเตรยม
วางแผนดานกฏหมายตางๆ
