UNC Wellness Centers Online Enrollment
Together with CSI Spectrum NG, our fitness management software system with high
level data security systems, and Docusign, the industry leading provider of electronic
signatures and contract execution solutions, we have created a secure and convenient
process for you to enroll as a member of UNC Wellness Centers.
During the first several steps of the online enrollment process, you will be asked to
choose either Meadowmont or Northwest Cary as your home site, enter basic demographic
information for yourself and anyone else you are adding to the membership, select your
membership type and enter your billing information. Once you have completed those steps,
we will provide you with a link from DocuSign to complete the necessary paperwork to
secure your membership. The paperwork includes the Membership Terms and Conditions,
Electronic Funds Transfer Authorization, Acknowledge of Receipt of Privacy Practices,
Waiver and Release of Liability and the Physical Activity Readiness Questionnaire. Each
person on the membership will need to click on this link and complete the forms through
DocuSign. If you are adding a child under the age of 18, you will need to complete the forms
for him/her.
Please review the procedures below and use it when you enroll. If you run into any

Online Enrollment Step-by-step Procedures
1) Click the ‘Join UNC NW Cary’ button.
2) Fill in your name, email address and zip code. Then click ‘Choose Your Location’.
Online account access

3) Choose a location for your membership by selecting the “Choose This Location” button under either

5) Enter in the Main Member’s information: Date of Birth, Emergency Contact and Phone
Number, Address, Phone Number. NOTE: if you are adding more members click the ‘ADD
MEMBER’ button.
6) ADD SUB-MEMBER (skip to next step if you only are enrolling yourself only) Click ‘ADD SUB-
MEMBER’ for each additional member and complete the information for an adult and/or
child(ren). Click ADD when finished entering in the required information.
Don’t forget to add an
email address for the

children, please put the

Click on the calendar icon
to access the calendar and
enter your Date of Birth.
Make sure to click on the
day in the pop up box.
7) When finished entering the Main Member’s information and/or additional members, read
about the 30 Day Guarantee, check the “I Accept” box and click NEXT.
8) In the Payment Summary screen, select the billing method, enter the billing information and click ‘PROCESS’.

20 characters in this box, you will receive an error message and won’t be able to proceed.

9) Click on the link and electronically sign the required documents. Each individual, including children, on
the membership must click on the link below separately and electronically sign the forms. This includes the main adult member, adult sub-
member (if applicable), and any child members (if applicable). NOTE: In order to complete the forms for a child member, please click on
the link again after completing your forms. Enter the name in the Name Field as “Your Name on behalf of Child’s Name” (John Smith on
behalf of Mark Smith). Enter your email address and electronically sign the documents.
10) You will be directed to the DocuSign screen. Enter in your full name and email address and
click ‘Begin Signing’. EACH member, including children, must complete a set of forms. For
children, enter the “YOUR NAME on behalf of CHILD’S NAME” (John Smith on behalf of Tom
Smith).

11) Click ‘Review Document’ to electronically sign the forms.
12) DocuSign PAGE 1: Read through the Membership Terms and Conditions, scroll down to the
bottom of the page and click on the ‘Sign Here’ button at the bottom of Page 1. The ‘Adopt
Your Signature’ button will pop. Select your signature and click ‘Adopt and Sign’. Click the
‘Sign Here’ button at the bottom of Page 1 to add your signature.
13) Once you have added your signature at the bottom of Page 1, the Page 1 box on the right hand of
the screen will turn green indicating that you have completed all required fields for this page.
14) Scroll down to page 2. Check ‘YES’ or ‘NO’ for the UNC employee question. If you (or one of
the adult members on the membership) is an employee of UNC HC, check ‘YES’ and enter
your EID or PID number for verification. The PID box will only appear if you check ‘YES’.

15) Read the PAR-Q information and complete the required fields. Check ‘YES’ or ‘NO’ to each
question. Physician’s information is required if you answered ‘YES’ to any of the questions.
16) Your document has been signed. You have the option to print and/or save the document.
Click ‘X’ in the top right hand corner to close out of the screen.

Click ‘Confirm Signing’ to finish.
Click ‘Show Document’ to pull up the
forms you signed. You can print or
save the documents for your
records. We will also send you a final
copy (see below) once we verify,
countersign and complete the
appropriate information.
17) FINAL COPY – UNC Wellness Centers will verify the information, countersign and complete
the membership information and send you a copy for your records within 3 business days. You are
now a member of UNC Wellness Center at Northwest Cary!
We will include your start date, individual dues and
total dues, enrollment fee, 1st
billing date, 1st
billing
total, and the primary member on the account. If the
facility’s opening is delayed, the member’s start date
and 1st
billing date will be adjusted accordingly.
