1.1 Definition and Requirements

1. What is an on-site Vaccine Clinic?

An on-site vaccine clinic is a service brought to your organization by CVS Pharmacy®. Our

trained immunizers bring the vaccines, consent forms, and all supplies necessary for

immunizations on-site to your location.

2. Are clinics available in my location?

On-site vaccine clinics are offered in all states, with limited availability in some areas. All clinics

must be within 30 miles of a local CVS Pharmacy and in the same state.

3. How far in advance do I need to request a clinic?

The Vaccine Clinic Scheduler (“VCS”) tool allows for date selection two weeks in advance.

This allows our teams time to properly prepare for your clinic and to ensure everything goes

smoothly. We will make every attempt to keep the date that you have requested but may

need to shift start and end times and/or dates based on schedules and the number of

expected participants at your clinic.

6. How are consent forms handled for patients under the age of 18 attending a clinic?

Consent for patients under the age of 18 needs to be completed by a parent or guardian. This can be done through the online appointment scheduler or using paper consent forms provided at the clinic. Children between the age of 16-17 need to have consent from a parent or guardian, but the parent/guardian does not need to be at the clinic with the patient. Children 15 and under must have an adult present at the time of the immunization and consent from a parent or guardian.

1.2 Pricing and Billing

7. What are the billing options for the clinics?

We offer several billing options. CVS can bill the participant’s insurance, direct invoice your

organization for the administered vaccines, or charge the participant the cash price for the

vaccine. There is no cash price payment option for the COVID-19 vaccine.

8. What is the direct bill voucher price for administration of the vaccines?

Standard dose flu vaccine is $35 per dose and senior dose vaccine is $69 per dose.

The administration fee for the COVID-19 vaccine is $40 per dose (the cost of the vaccine

product is covered by the government).

9. What is the minimum number of vaccine doses for a vaccine clinic?

30 vaccine doses per clinic.

10. Are there any fees associated with the clinic?

Yes. When scheduling an onsite clinic using the VCS tool, your organization must specify

the number of vaccine doses to be administered at each onsite clinic. This is called the

“Appointment Commitment.” If your organization does not meet this Appointment

Commitment for any onsite clinic, CVS will charge a fee of $15 for each unused

appointment.

1.2 Specialized Requests

11. Can I choose the COVID-19 vaccine that is offered at the clinic?

We cannot accommodate requests for a specific type of COVID-19 vaccine. Our pharmacy

teams will confirm the specific vaccine product that will be administered at your clinic(s)

during the confirmation process. If it is a two-dose product, the second clinic date will be

confirmed at the time the first clinic is confirmed.

12. Is there any setup required by me/my organization ahead of the clinic?

Yes. Your organization will be expected to provide the following:

a) An onsite representative

b) A registration area with an adequate number of tables and chairs spaced a minimum of 6’ from the

immunization area

c) Space that allows for an adequate number of immunization stations that shall be approximately 10’ x 10’

and spaced approximately 6’ apart

d) Immunization stations each prepared with a table, two (2) chairs, and a trash can

e) Accessible electricity and wireless internet

f) Access to restroom facilities and running water for CVS personnel use

g) Waiting area(s) with sufficient space for social distancing

h) A post-vaccination observation area with a sufficient number of socially distanced chairs

i) Access to a temperature-controlled space for vaccine storage

j) For indoor clinics only:

1. A temperature-controlled room or rooms for the clinic to take place

2. Room(s) that can be locked and that have sufficient ventilation

k) For outdoor clinics only:

1. A tented registration area (at minimum, 10’ x 10’ tent per registration table)

2. A tented immunization area (at minimum, 10’ x 10’ tent for each immunizer area)

3. Alternate compliant indoor site in case of inclement weather

l) For drive up clinics only:

1. A CVS-approved site location with two entry/exit points that allows for one-way traffic.

2. A safe and clean parking lot with enough space to hold an adequate line of automobiles without

interfering with street traffic or car access

3. An onsite representative

4. A reserved holding area for automobiles for participants to receive the vaccine in the automobile, and

to be monitored for adverse reactions post-vaccination

5. Alternate compliant indoor site in case of inclement weather

6. One tent to hold two (2) chairs and a table (at minimum, 10’ x 10’)

13. Can I have a drive through clinic?

Drive-up clinics are available for flu/other vaccines, but not for COVID-19 vaccines.

14. Can I have a clinic outdoors?

An outdoor clinic is available for all vaccine clinics.

15. Can I have an Account Manager/single point of contact for my B2B clinics?

COVID-19 vaccine clinics- The CORE B2B offering does not offer an account manager, but

if you are interested in having an account manager, you can explore our Enhanced COVID-

19 Vaccine clinic offering on https://cvshealth.com/covid-19/return-ready/vaccination

navigating to the enhanced version of the program (https://cvshealth.com/covid-

19/return-ready/vaccination/enhanced).

Flu/other vaccine clinics- Account managers are not available for flu/other vaccine

clinics. Please work with the local store team that is assigned to your clinic.

2. Contracts (“Agreement”)

16. Why do I need a contract?

Contracts are required this year to ensure that both CVS and clients are covered from a

liability perspective and aligned around expectations for CVS teams to vaccinate off-site.

The fees that are introduced help to ensure that the immunizers are compensated for the

time spent at clinics if the number of vaccinations administered at a clinic is less than the

requested quantity of vaccine doses.

17. How do I start the contract process?

You can begin the contract process by going to https://cvshealth.com/covid-19/return-

ready/vaccination to complete a COVID-19 vaccine clinic contract or

https://www.cvs.com/immunizations/flu/clinics for a flu/other vaccine.

18. What is ScriptClaim?

ScriptClaim is CVS Pharmacy’s third-party claims and payment processor.

19. What is a voucher?

A voucher is the CVS term for direct bill. If you choose to offer vouchers (direct bill) to

participants, you will receive a link to send to participants in your confirmation email from

ScriptClaim after agreeing to the terms and conditions. This link will be used to generate

individual vouchers, which will be entered when registering for an appointment digitally or

shown to the pharmacy team the day of the clinic

20. Can my organization make changes to the Contract?

• COVID-19 vaccine clinics- The contract is our standard CORE B2B offering, and we

cannot accept any customization or changes to the contract. If you require a

customized contract, you can explore our Enhanced COVID-19 Vaccine Clinic

offering on https://cvshealth.com/covid-19/return-ready/vaccination/enhanced

• Flu/other vaccine clinics- No changes can be made to flu clinic contracts.

21. I am having technical difficulties with the contract site (ScriptClaim), who do I

contact?

For technical difficulties with the terms and conditions, you can contact ScriptClaim at 877-

403-4919 or through email at [email protected].

22. Can my organization be added as additional insured on the CVS Certificate of

Insurance (COI), sometimes also asked for by ‘proof of insurance coverage’)?

We can provide a COI as proof of our insurance and/or add you as a certificate holder.

However, we are not able to add your organization as an additional insured. If you need a

copy of the COI, please email [email protected] with the request.

23. How can I terminate my contract?

To terminate the contract, you must provide written notice to the notice address in your

contract at least 30 days before the requested termination date.

24. Is a drive through/up clinic an option?

• COVID-19 vaccine clinics- No, drive-through/drive up clinics are not available for

COVID-19 Vaccine Clinics.

• Flu vaccine clinics- Yes, drive up clinics are available for Flu vaccine clinics if your

organization is able to meet the requirements noted in FAQ 11.

25. Can my clinic be held outdoors?

Yes, if your organization is able to meet the requirements for an outdoor clinic listed in

question 11.

26. What is the cancellation policy?

There is no cancellation fee, but we do ask for at least 3 days’ advance notice for

cancellations whenever possible. Clinics are subject to rescheduling or cancellation at

CVS’s discretion due to lack of clinic supplies and/or inclement weather. We will work with

you to reschedule your clinic should this situation arise.

27. Are participants who registered for a clinic notified when it is cancelled?

Yes. Any registered participants will receive an email and/or text message (depending on

the information they provided during registration email/phone number) notifying them that

their clinic is cancelled. This notification will include a link to the in-store appointment

scheduler should they wish to make an appointment to receive their vaccine in store.

28. Can I see a list of who is registered will therefore be impacted when I cancel the

clinic?

Yes. You can pull the clinic roster from your VCS account. However, please do this before

requesting the cancellation, as you will no longer be able to see the roster once the clinic

is cancelled.

3.1 Submitting a Clinic Request

29. Where do I find the link to the Vaccine Clinic Scheduler?

The link to the vaccine clinic scheduler can be found in the confirmation email you received

from ScriptClaim with your signed contract. It can also be found at

vaccineclinicscheduler.cvs.com.

30. Where do I find my contract code to enter into the Vaccine Clinic Scheduler?

Your contract code will be in the confirmation email you receive from ScriptClaim as well as

on the top left-hand side of your copy of the PDF contract, which is also included in your

confirmation email.

31. What happens after I submit my clinic request?

You will receive an acknowledgement email that your request has been received, which will

also provide you with an 8-digit Clinic ID. This is your clinic’s unique identifier and begins

with the letters CI. Please note that this is not a confirmation email. After our teams have

reached out to you to discuss the details (within 3 business days of your request) they will

confirm the clinic, and will send you a confirmation email including the Pharmacy team’s

contact information and a link for participants to register for the clinic.

32. Who should I use as primary and secondary contact?

The primary and secondary contacts should be people involved in the coordination of the

clinics. These are the people who will receive the confirmation email that holds the

registration link you will send to participants. The email comes from no-

[email protected].

33. How do I submit a Combination COVID-19 And Flu (or other) Vaccine Clinic?

While our Vaccine Clinic Scheduler system currently does not allow for COVID-19 Vaccine

clinics to be requested with other vaccines, we are able to accommodate combination

clinics.

• If requesting Flu and COVID-19 Vaccine Clinic where the number of expected

COVID-19 vaccines is under 30, enter the total number of vaccines for Flu in the

appropriate Flu field (Under 65 and 65 & older) Then, indicate the expected number of

COVID-19 vaccines in the notes section.

o Discuss this plan with the local store team during the confirmation phone call

o COVID-19 Vaccines will need to be handled as walk-ins when given at flu

clinics and discussing during the confirmation phone call ensures our teams are

prepared to bring paper consent forms on site.

• If you are expecting 30 or more COVID-19 Vaccines to be administered at your Flu

clinic, request a separate Clinic for COVID-19 on the same day/time as your Flu

Clinic, and that will allow for digital registration. This plan should be communicated to

the store team during the confirmation phone call.

3.2 Confirmation Email and Digital Appointment Registration Link

34. I did not receive my confirmation email. Can you please send me the link?

The confirmation email is sent from [email protected]. Please make sure that it is

not in your spam folder. Also, the link is available within your VCS account. Log into your

account, select Active Clinics from the menu bar, open the clinic, and scroll to the bottom

to find the link

3.3 Locating a Clinic ID

35. Where can I find my Clinic ID?

Your clinic ID can be found in your request email, your confirmation email, and in your VCS

account itself. The Clinic ID is in the subject line of both emails and in the body of the email

in red text.

To find your clinic ID from the vaccine clinic scheduler, go to

vaccineclinicscheduler.cvs.com, log into your account, select Active Clinics, and you will

see a list of your clinics on that page. Clinic IDs will be to the left.

• Locating it in your request email

o The email is sent after you submit your request for a clinic

o The sender is [email protected]

o Clinic ID is in the subject line as well as in the body of the email in red text

4. Participant Registration Questions

36. How do my participants obtain their record of immunization?

There are several options for participants to obtain their record of immunization.

• If an email address is provided during the online appointment scheduling process, a

vaccine record is automatically emailed after administration and patients can pull up

the record from their email.

• A record of the vaccine received, and date administered is also available to patients in

the CVS Pharmacy app or online at cvs.com. In order to ensure the security of patient

information, patients will need to create an account to access the record in our app or

online

• Patients can also use their CVS account log in to access the vaccine details at

CVSCare.com. This will show vaccine specifics like Lot # and expiration date.

• Any CVS pharmacy team member can also print a vaccine record reflecting the date of

administration and vaccine administered.

37. How do I track the participants who have registered for the clinic?

You are able to view participants who have registered for the clinic starting 14 days prior to

the clinic date by logging into your vaccine clinic scheduler account, clicking on active

clinics in the top left hand corner, then opening the specific clinic you wish to review, and

clicking the ‘registered participants’ button at the right hand side of that page. Note that

this option is only available if you submitted the clinics yourself through the Vaccine Clinic

Scheduler. If the store or another member of the CVS team submitted your request, review

the steps in question 36.

• Log into the vaccineclinicscheduler.cvs.com.

• Click on “Active Clinics” to view all submitted clinics

• Select your clinic

• At the top right of the page, click on the blue “Registered Participants” button

• Once select, roster of appointments scheduled to date will display (see below)

38. There may also be scenarios when our internal teams enter clinics on your behalf.

There may also be scenarios when our internal teams enter clinics on your behalf. For

example, this could happen if it was requested by you or if our team needed to make a

change that required a new clinic.

Steps:

• Log into the Vaccine Clinic Scheduler

• Click on Clinic Report

• Enter the Clinic ID and the Primary Contact’s email address

• Click on open PDF when the clinic populates in Clinic Search Results

• If participants have registered, the PDF will open, if no participants are registered at this

time, a message will display indicating that no participants are registered.

39. What questions will my participants be asked when they register?

They will be asked to select an appointment time and to answer standard vaccine consent

form questions including demographics, screening questions, insurance, or voucher

information, to review the Vaccine Information Statement (VIS), and to complete the

consent for services.

40. When does registration for a clinic end?

Clinic registration closes 24 hours prior to the star time of the clinic.

41. What is the Voucher Program?

The voucher program allows organizations to cover all or some of their participants’

vaccines. The organization can enroll in the voucher program so the vaccines they choose

to cover will be invoiced to them directly.

42. Where can I enroll in the voucher program if I do not want an onsite clinic?

For Flu and other vaccine Vouchers please utilize the ScriptClaim web address to access

the ScriptClaim portal to enroll in the voucher only program:

43. Where can I find out more information regarding the voucher program?

For details pertaining to the voucher program, please visit www.CVS.com and access

the vaccine site. If you require additional assistance, please make an outreach

to [email protected].

44. What vaccines can be covered through the voucher program?

48. I am a Senior Living or LTC facility and would like to enroll in the voucher program, who

can I speak to for more details?

You can reach out by email to [email protected] with any voucher related inquiries

for these types of facilities.

49. How will I receive my voucher?

Vouchers will be sent to you via email. There are two types of vouchers for your

participants, standard multi-use, and Unique ID number vouchers. Through the enrollment

process on the ScriptClaim portal, you will be asked which voucher type you would like to

use.

• Standard multi-use voucher: The multi-use voucher is a voucher that can be used

for the entire organization and will be attached to your confirmation email.

• Unique ID voucher: This voucher type will allow you to provide a link via email to

those participants who require a voucher. They will be able to generate a voucher

with a unique voucher ID number that only the individual participant can use. The

link to provide to participants will be provided in your confirmation email.

50. Who will be providing invoices for redeemed vouchers?

Voucher invoices will be provided by the vendor, ScriptClaim, within 120 days after the date

of service.

51. When will the voucher expire?

Vaccine vouchers will expire on 3/31/2022. You have the option to contact ScriptClaim

at [email protected] to request that the vouchers be terminated on an earlier

date.

52. Who can provide pricing for vaccinations associated with this program?

Pricing for all vaccines covered through the voucher program will be listed on the

ScriptClaim portal when you select the products you wish to cover.

53. Can we allow for rate exceptions or language updates to the contracts?

There are no exceptions to rates or language modifications within our contracts.

54. How can brokers enroll their clients in the voucher program?

Brokers would need to direct clients to the ScriptClaim portal to complete a contract in

order to schedule a clinic. The client will have the opportunity to enroll in the voucher

program, but they would need to accept the terms and conditions of the program.

55. How can an organization terminate their contract?

The organization needs to contact [email protected] in writing with a request

to terminate their agreement. CVS will then notify ScriptClaim of the request for termination.

56. What are the methods of payment?

The preferred method for payment is ACH or check; however, ScriptClaim will accept a

credit card which will yield a 3.5% processing fee for all credit cards transactions.
