REQUEST FOR FORMAL WRITTEN QUOTATION
Kindly furnish me with a written quotation for the supply of the goods/services as detailed
in the enclosed schedule.
The quotation must be submitted on the letterhead of your business and can either be faxed or deliver by hand.
The following conditions will apply:
Price(s) quoted must be valid for at least thirty (30) days from date of your offer.
Price(s) quoted must be firm, must be inclusive of VAT and furnish your CSD number.
For all transactions exceeding R15 000, must be accompanied by a valid Original Tax Clearance Certificate or PIN
Certified BEE Certificate
Declaration form (MBD 4.1) and a Confirmation of Banking Details.
First preference will be given to the service provider who is registered in Central Supplier Database.
Supplier must attach the MBD 6.2 declaration form for local content.

REDESIGN OF CHRIS HANI DISTRICT MUNICIPALITY LOGO

Chris Hani District Municipality, comprising six local municipalities
(Enoch Mgijima, Inxuba Yethemba, Emalahleni, Sakhisizwe, Engcobo
and Intsika Yethu), adopted a new vision; mission and values.

This development has prompted alignment of the municipal identity to
the expressed aspirations. Meaning, a symbol that makes the
municipality stands out as its identity, must clearly unravel the above
desires.
Proposals, are thus invited from qualified and experienced service
providers to redesign the Chris Hani District Municipality (CHMD) logo,
taking into account the following :
A key component of this project is to understand the
attributes of CHDM and to create a graphic that will best
communicate that fact to the community at large (expressing
its uniqueness, diversity, values and liberation heritage and history
defining it).

Images, shapes and colors must be easily recognizable and well-
articulated in terms of their meaning
The design must be simple and yet expressive, with less color
codes and high resolution in any common picture format
The current CHDM logo MUST form the basis of the design
ensuring that key elements are retained. The logo and elements
are depicted hereunder:
Service provider is expected to develop an accompanying corporate
identity manual which will guide usage of the logo.
Service provider must demonstrate expertise, competency,
experience and qualification in the area proposed (A detailed CV of
members and company profile must be attached).
The potential service provider’s track record of similar or relevant
work must be clearly indicated (please submit portfolio samples
exhibiting technical and creative execution, particularly for similar
projects).

A detailed pricing/costing schedule must be presented.
All services providers are expected to attend a compulsory briefing.
All shortlisted prospective service providers will be invited to make
presentations on their proposals.
Commitment time of the service provider to the project and
resources available that will specifically be utilized for this project
contract.
Clear Time frames for delivery must be stated on the proposal.
