biometric / worksite wellness events and other BCC programs (Internal Referrals).

• Appropriate readiness to change

• HA / Biometrics *Weight Management Triggers:

• Overweight: Body Mass Index (BMI) ≥ 25 – 29.99

• Obese: Respondent with a BMI ≥ 30

• Appropriate readiness to change

• Integrated Predictive Model Triggers:

• Obesity, Tobacco Use Disorder, Hyperlipidemia, Hypertension, Pre-Diabetes, or Diagnosis of Metabolic Syndrome
