As I sit down to write this year-end report as the Interim Academic Chair, I find it difficult to believe that it has been a year since I stepped into the role. It has certainly been a learning experience, and although I will admit to a certain bias, I think that it has been a productive year.

There were some major changes in the academic administration of our department through the year. Dr. Matt Schubert assumed the role of Residency Program Director in Saskatoon and has provided a sense of stability by agreeing to stay in the role for the next 5 years. With the number of students increasing in Regina, the role of the Teaching Coordinator was split between the undergraduates and postgraduate students. Dr. Rashmi Bhargava took on the role as JURSI coordinator with her usual energy. Dr. Darrien Rattray took over as the Associate Program Director for Regina. All three bring new ideas and new energy to these roles which are helping to revitalize our education programs.

As important as good leadership is, without good people to lead the system is bound to fail. The teachers in our department have demonstrated a willingness to go the extra mile when required. I have completed about half of the face to face discussions about the teaching program, and there are a number of issues that are relatively common that we will address in the new year. Unfortunately the most common is a need for more time in the day, I am not sure I have the answer to that concern. There are some new and exciting elements that will soon be incorporated into the teaching program. A formal surgical simulation program will ensure that our residents arrive in the operating room with a solid foundation in basic laparoscopic and hysteroscopic skills. The introduction of the Nurse Coordinator and a full time in house

family physician in Saskatoon will provide both quality orientation sessions and appropriate supervision for junior learners. We are introducing four new teaching awards that will be determined by the undergraduate and postgraduate learners and will recognize teaching excellence in the department. One of the most encouraging developments has been a renewed sense of the provincial nature of the department. Given the distributed nature of both the undergraduate and postgraduate programs, this is important to the delivery of quality education and ongoing full accreditation. It has allowed us to look for the new and innovative approaches on both sites and working to incorporate them into the entire program. The research environment in our department was at best on life support at the start of this year. I think we could safely upgrade its status to “ready to transfer from the ICU to a step down unit”. We had a successful Research Coffee House in September to brain storm ideas, trying to link those ideas to those with the time and expertise to get the project going. The Regina Qu’Appelle Health Region has been designated as an Eligible Institution by the Saskatchewan Health Research Foundation, which means we can now apply for formal research funding on both sites. Dr. Almereau Prollius is the new chair of the Resident Research Committee, which has as its first task setting down the formal process and terms of reference for the residency research program. The issue of funding for resident research and other resident education items will improve in the new year with the institution of a Resident Fund. Yearly contributions from faculty will provide the committee responsible for the administration of the fund sufficient resources to ensure that research and innovative education opportunities are not suffering for lack of money. I am particularly excited about the role this initiative will have in re-invigorating the resident research program in our department. The MIS fellowship continues to attract excellent candidates for an additional year of post-graduate training in advanced laparoscopic and hysteroscopic procedures. It reflects the department’s long standing tradition of being an early adopter of new and innovative technologies. There are now 4 preceptors working with the fellow, giving them 3 to 4 days of OR time a week. For the first time this year, the MIS fellow will spend a month in Saskatoon working with the MIS surgeons there. Dr. Laura Weins completed the one year fellowship and after a 6 month sabbatical will begin her practice in Saskatoon. Along with her expertise in advanced endoscopic

surgery, she will be working on developing a formal surgical simulation program for the residents in Saskatoon. Dr. Anet Maksymowicz began her year of fellowship training on September 1, 2014. She finished her residency in Obstetrics and Gynecology at the University of Manitoba and plans on returning to Winnipeg to practice at the end of her year. We had significant interest in the 2015-2016 position and for the first time had formal interviews. We are working with all of the MIS fellowships in Canada to try and coordinate these interview dates to minimize the travel requirements for residents. We look forward to Dr Jaclyn Mader from the University of Western Ontario joining us next year. Finally, I want to thank each of the members of the teaching faculty and administrative staff in the Department of Obstetrics, Gynecology and Reproductive Sciences for their assistance, their candor and their commitment. It is a privilege to be able to work with each of you, and I am thoroughly enjoying getting to know more about how everyone approaches teaching and research. It is going to be an equally eventful 2015, with a new Unified Head search to be completed by June. We also look forward to adding new faculty on both sites, in Maternal Fetal Medicine and Gynecologic Oncology. I look forward to continuing to work with all of you in 2015. J. Thiel

Undergraduate Education Saskatoon

C. Simpson

The undergraduate program in the final clinical year and the preclinical years continues to follow the previous established format. However we are starting to participate in the new 2+2 curriculum as the first and second year classes are now initiating pre-clinical experiences. With 100 students now in each preclinical year, we rely on further teaching commitments. This will be a further challenge in the clinical year, particularly when two sets of students are actually in clinical experiences in 2016. Increased use of the simulation lab, in both Regina and Saskatoon has been initiated along with clinical coordinators for the students on the wards. We believe this will improve the experience for the students. All students have passed the clinical rotations over the last year. The next accreditation visit, to review probation status, will occur in the spring of 2015 for undergraduate programs and the next full accreditation review will be in 2017.

Postgraduate Education Regina

D. Rattray

The Regina site residency program continues to have a total of 10 residents. There are 2 approved spots for the upcoming CaRMS match in July 2015. Interviews will take place in Jan 2015 and will consist of an interview panel of 2 Regina Faculty members and 1 resident. A second interview will take place with the associate program director.

The focus of the upcoming year is the pending accreditation survey by the Royal College in Nov 2015. We continue to strive to improve the quality of the resident’s education by increasing hands-on training and balancing service to education ratios. In an effort to facilitate this, the RPC is currently in the process of implementing Rotation coordinators in both the Saskatoon and Regina site for the largest rotations (General Obstetrics and General Gynecology). This is a pilot project designed to increase the timeliness and effectiveness of feedback as well as to identify logistical changes that will improve each resident’s experience on these important rotations. We will also be implementing end of rotation exams for these two rotations to give the residents further practice for their Royal College exams as well as to increase assessments of their knowledge as they progress through the program.

Simulation is an ever growing area of medical education and we are going to be rolling out a pilot project for simulation in Obs/Gyne early in 2015. To date, there are plans for 3 simulation sessions to take place on academic half days in Jan, Mar, and May of 2015. These will cover a range of gynecologic topics including energy sources, medical devices, surgical techniques, and laparoscopic suturing to name a few. The goal is to have a didactic session that will be video conferenced between sites and then a 2 hour session in the simulation labs.

Feedback, both positive and constructive, is a crucial part of residency education and one of the new policies that has been applied is the performance of daily evaluation logs

in the operating room. Research in medical education strongly supports the use of immediate feedback to enforce a resident’s strengths and to improve on weak points. We are asking the residents to complete evaluation forms with the attending physician immediately upon completing a day in the OR or a day on L&D.

The focus on resident research has increased over the past year and early identification of research mentors is being stressed. The Research Coffee house in September was a success and was a good mix of Faculty and Residents with many excellent research ideas being discussed. There will be a Department of Obs/Gyne Research Day held in Regina on March 6, 2015 and this will highlight the research being done in the department (both among Faculty members and Residents). Residents with completed projects will give an oral presentation of their work and those still working on their projects will have poster presentations for their “works in progress”. This will help facilitate the completion of high quality resident research projects and hopefully foster the enjoyment of research among the residents.

As always, I thank you for your dedication to teaching and look forward to continue working with you in the education of the next generations of Obstetricians/Gynecologists.

Postgraduate Education Saskatoon

M. Schubert

Wow!

We laughed, we cried, we said hello and we said goodbye, we had successes and failures and the cycle of life continued its relentless march in our program.

Our departures included Drs. Giede and Smith, we thank them for their service at the helm. Erin welcomed her son Aidric into this world. Jellena and Stacey found new paths in Toronto and St John's respectively. Our graduating class, Lauren, Laura, Michelle and Cynthia are all developing in their new roles! We just have to turn around to see Michelle, the rest we'll catch up to here and there and at SOGC!

We welcome Elise, James, Kat, Tasha and Mae and look forward to participating in their development.

Our program is alive and bustling with activity. Our major educational projects are the 'local reboot' of our Reproductive Endocrinology and further development of our Minimal Invasive Surgery program. We welcome new faculty this year and next deepen and widen the talent pool! Our major annual talent hunt is underway. We have many expressing interest, we will invite lots to chat in January. We will be challenging them to show us our future. Thank you to Marj Lens for all her work, and keeping us on track!

Clinical Report J. Martel A couple of crazy chicks took on the task of clinical leadership on January first. Drs. Epp and Martel quickly learned on the job that there are more committees and meetings in administration than one can possibly fit in a day. With the support and encouragement of department members and guidance from SHR directors, we have tried our best to keep the department steady and on track to providing the best care for all of our patients. Members of the department have volunteered for RPIWs, patient advisory committees, physician engagement work as well as case reviews and mentoring new members. These colleagues are invaluable members of our team and we want you all to know your commitment is valued. We have shared the work when able and assisted and provided perspective to each service’s needs. Alternating the chair of the executive and department meetings has worked very well. We held a very successful department retreat in September. A clinical orientation to new members was created outlining how things work within our hospital system and how to access the services and information that one would need to provide the best care. We are indebted to our support staff Jennifer, Lorraine and Lindsay who organize and arrange, record and document, help us remember and make us look good every day!

Gynecology

A. Epp

The gynecology service within SHR continues to provide excellent care for the women of Saskatchewan. Main OR – SCH/RUH The target time frame of 3 months for offering elective surgery has been met. This was accomplished by a number of different strategies: 1. Careful monitoring of individual surgeon waitlists and redistribution of OR time

when indicated.

2. Careful stewardship of assigned OR time with internal trades if a surgeon was unavailable to fill their assigned day.

3. Flexibility of surgeons to pick up extra days if necessary to decrease their wait list or operate on days other than their usual assigned OR days.

4. The ongoing use of Gynecology Flex time.

Our service continues to set an example for other surgical services to emulate. Thank you to all the physicians for their hard work in meeting this important target. We introduced the Myosure Hysteroscopic Resection device as new technology in 2014. This procedure will aid in the treatment of submucosal fibroids and uterine polyps. It is currently placed in the main OR but there is a plan to move select appropriate cases into the Women’s Health Center in 2015. The Gynecology Oncology service continues to be overextended with clinical demand. Despite this, the 21 day targets for cancer surgeries have generally been met. The

province has now included a 6 week wait time target for select oncology cases. Hopefully, this will aid in better distribution of cases going forward. Women’s Health Center – SCH The WHC unit continues to increase the volume of patients receiving outpatient procedures with IV sedation and local anesthetic. This has been a consistent trend throughout 2014. A Rapid Process Improvement Workshop was conducted in June 2014. Although, the unit was seen to be a very efficient and patient centered area, some recommendations were made. These included the development of a patient information sheet for the WHC, a proposal to try to have patients in and out within 3 hours and 15 minutes as well as a change to the Exclusion Criteria such that patients up to a BMI of 42 can be accommodated at the WHC. Gynecology Ward – SCH Our post-operative infection rates for hysterectomy remain at zero for the year 2014! We have new gynecology standard post-operative order sets that were revised and released in October 2014. These will be coupled with the SHR standard VTE prophylaxis orders and the SHR standard post op narcotic orders. Pelvic Floor Pathway (SCH) The Pelvic Floor Pathway was opened at SCH in November 2014. It provides education, assessment and conservative management to patients suffering from urinary incontinence and prolapse. There were approximately 700 referrals to the Pathway in its first year of operation. The Surgical Services team won a Saskatoon Health Region Bravo Award. Future Directions: A request has been sent to the SCH Foundation to fund 2 integrated Minimally Invasive Surgical Suites at SCH. This would improve the quality of service for women needing gynecology surgery and allow surgeons to utilize state of the art equipment.

Obstetrics

J. Martel

Assessment 8500+ visits FAU 2500+ visits Deliveries 5289 babies! We had a very busy year. Antepartum, Assessment, Labour and Delivery and Post-partum faced daily challenges of capacity and staffing as well as the frustration of working in a very old building. Hot water and electricity will never again be taken for granted. We developed and updated many department policies including:

• Post vaginal delivery order sets • Post cesarean delivery order sets • Planned vaginal breech delivery • Debriefing with Charge nurse and anesthesia while on in house OB • Obstetrics delivered family physician patient urgently • In house OB policies about weekday call and day sign out • Management of OB patients while in SCH OR or WHC

Policies still being finalized and expected in 2015 are:

• Premature prelabour rupture of membranes • Nifedipine for tocolysis

• High dose oxytocin infusion

More – OB More – OB was the single most important recommendation of the external review and this year Module two was completed. Module two addresses communication and team work. Thanks to Drs. Stewart, Mirosh, Hey and Duda, Leanne Smith our director and all the dedicated team leaders and educators. Congratulations to all caregivers for participating. Rapid process improvement workshops (RPIW) were completed on Assessment patient flow, Labour and delivery room set up and Induction of labour and have resulted in successful changes to patient care. The maternal leadership team won a Saskatoon Health Region Bravo Award Two major improvements are the addition of CCA-OBs (family practice OB care providers) to assist in patient care during the times that O&G residents are at mandatory educational events and the approval of funding and agreement to undertake the establishment of in house family medicine obstetrics coverage. Future Directions:

• Fetal lactate testing in labour and delivery • Obtaining access to HIV viral loads from Labour and Delivery • Children’s Hospital of Saskatchewan – maternity service – continued

development • In house family medicine obstetrics

Quality Assurance and Safety

This document provides an executive summary of the activities of the Quality Assurance and Safety Committee of the Department of Obstetrics, Gynecology and Reproductive Sciences for the period of January 2014 – December 2014

Committee Background: The Quality Assurance and Safety Committee formally known as Continuing Quality Improvement Committee has been a standing committee of the Department of Obstetrics, Gynecology and Reproductive Sciences. This committee has functioned as a Quality and Safety committee of the Department, which has been charged with the responsibility Quality review and safety concerns that arise from the day to day activities of the Department. This committee has been solely a committee of the Department with the unified head of the department being the custodian of this committee.

The committee operates within Terms of Reference which have been accepted and adopted by the members and has undergone many revisions over the years. Over the course of the years the committee has matured in its ability to serve the Department in this capacity. In 2013 Quality and Safety rounds which used to fall under a separate umbrella was added to the responsibility of the committee. Over the course of time, membership of the committee has changed to reflect the changes of various department and unit heads. In all of this however, the core representation of the committee has been maintained. In the year 2014 we particularly acknowledged the representation of Midwifery as well as the Medical Records Department.

The Quality Assurance and Safety Committee meets monthly except for the months of July and August. In 2014 the committee met every month except July, August and December. During the 2014 year all of the various departments were very active in our meetings and also in bringing forth issues that pertained to quality and safety within

their various departments. This was very much appreciated. As of now the continued representation from NICU continues to be an issue which will need to be discussed in the coming year.

Summary of Activities: The beginning of the year started with issues regarding our Terms of Reference. The committee decided that there was no need for any amendment to the terms of reference.

Meeting Times As part of our housekeeping, the committee made some changes to our meeting times to be able to accommodate most of our members needs to allow them to be able to attend our meetings. That helped to alleviate that problem

Women’s Health Pain Audit The Women’s Health Centre has been a very welcomed addition to the department working environment, providing a place where minor surgeries could be done without a resident anesthesiologist. This has worked well generally but the committee decided to look into the effectiveness of the conscious sedation which is the main anesthetic technique used for the various procedures done in the unit. A Quality Improvement Pain audit was commissioned to be done at the Women’s Health Centre. The Quality Assurance and Safety Committee spearheaded this and this project provided a lot of insight into how pain is managed in the unit. This was a very useful project that was carried out with the help of the Nursing staff under the leadership of the Unit Manager. That went on for many months and the findings were reviewed by the members of the committee. Suggestions were made that were designed to improve the delivery of conscious sedation to the patients that were coming into the unit. Safety issues that arose out of that were also discussed and the necessary corrections made.

Safety Issues Reporting In dealing with safety issues that arise in the department, the committee explored the possibility of using AEMS reporting which is an established reporting system for safety concerns that arise in various parts of the hospitals. Leanne Smith, a member of our committee, has been very instrumental in helping the members familiarize ourselves with the AEMS reporting and how we could better utilize those resources. At the moment the AEMS reporting has never been used consistently but hopefully in the future the new Safety Alert System that is presumed to be in the process of development would become part of how safety issues and reporting can be collected.

These AEMS reports helped us to identify issues concerning mislabeling, delivery bed issues and various other safety issues that were brought up and solutions found.

Some of the safety issues that were discussed included Order Sets for both Obstetrics and Gynecology and how it affected delivery of care. Communication issues between Anesthesia and Obstetrics that was deemed as meriting safety concerns in the delivery suite were brought up and discussed. Dr Campbell helped immensely with that endeavor. In House Family Medicine Coverage & CCA position Funding The committee recognized that as a safety feature, there was a need for these two positions to be funded for additional coverage in the Delivery Unit. Dr Martel and Leanne Smith have worked hard to get the approval of the funding for this coverage. This safety issue regarding the need additional coverage has been resolved or being is resolved.

RPIW and History and Physical AuditIn June 2014, an RPIW was conducted at the Women’s Health Unit (RPIW 85) that looked at Lead Time from Registration to Discharge of patients coming to the Women’s Health Unit. The RPIW prompted some safety issues that were looked into by the Committee. The main one was about the History and Physical document that accompanied the patients to the unit. A History and Physical Audit was thus conducted, looking at the content, accuracy and relevance of the document in regards to patient care and safety, and whether it was necessary to carry out a much wider and detailed audit. The general consensus of the committee was that a bigger audit will be useful. The significance of this may be relevant to other departments. That will be looked into at a later time.

Quality Assurance and Safety Rounds This issue has finally been dealt with by the committee. Quality Assurance and Safety Rounds for the Department has been formalized. MOCOMP accreditation formalities have been completed and the rounds have been assigned up to the summer of 2016.

Chart Review based on Indicators The committee has developed indicators for chart review which is designed to be streamlined with Regina Hospital. We have been working with the Health Records Department on this issue. Unfortunately there are still issues to be worked out for full implementation and the committee hopes this will be accomplished in due course

Department Retreat

A full day departmental retreat was held in the Gold Room at Boffins on September 27, 2014. The day began with breakfast that was attended by Dean Preston Smith and Dr. Rob Weiler (SMO of SHR). Dr. Smith gave a brief description of the current state of the College of Medicine and his vision for the future. There was time for interaction with the physicians present. Dr. Rob Weiler congratulated our department members on taking time out of their busy schedules to work on departmental issues. He also endorsed our focus on physician wellness. The morning session was led by Dr. Anita Chakravarti. She gave a didactic lecture that focused on physician stress, burnout and risk assessment. Web resources were provided to the physicians in attendance for further information. Following the didactic session, physicians were split into small groups to work on specific cases. Each case related to an adverse event and the sequelae that follow.

There was an obstetrical scenario, a gynecology scenario and a College complaint. Excellent discussion among the department members occurred and there was a general feeling of collegiality and support among the group. Following the case discussions, Dr. Chakravarti led a large group discussion about future directions for physician support in the Department of Obstetrics, Gynecology and Reproductive Sciences. The afternoon was spent in 3 small working groups discussing specific departmental concerns in Obstetrics, Gynecology and Education. Gynecology Round Table

This table was facilitated by Dr. Harding and the following main points were discussed and some action plans made:

1. Improve communication in the OR. Foster a culture of respect for the OR team 2. Identify stress producers in the OR and develop mitigation strategies 3. Improve efficiency wherever possible 4. Identify specific equipment deficiencies and concerns 5. Encourage department members to assist each other in the OR with complex

cases. The concept of surgical mentorship for junior surgeons was discussed.

Education Round Table

This table was facilitated by Dr. Schubert. Our upcoming Royal College Accreditation was discussed. A full house at the Clinical Faculty portion of the meeting will help send a strong message to the Royal College. Competency Based Medical Education and it's eventual introduction was touched on. Our major discussion then centered on how we can give better training in Gynecology. Our hands on technical training has been good but the inclusion of residents in the clinical decision making could improve considerably by increased 'clinical bedside' teaching, more evaluations that push the residents to reach further and call that better reflects how they will practice. Specifically we agreed that we need to use more daily evaluations, continue to discuss clinical management at the bedside and to look at how call functions especially the Senior Gynecology Pager.
