Leadership (NSCSL) Update
2018 RAPID Conference
Purpose
Discuss NSCSL main focus for 2017-2018.
Identify data findings from ICES (INPO Consolidated Event System) Event Report reviews.
Share steps the NSCSL is taking to improve reporting, identify trends and improve vendor quality.
Facts
All utilities/sites have owners and detailed processes or procedures
associated with the Equipment Reliability Bubble Charts
(Engineering), Equipment Failure Analysis Checklists (Engineering)
and ICES reporting functions (ICES Coordinator).
Seldom does the ICES Coordinator report to Engineering.
All 3 can impact ICES Cause Code determination.
Supply Chain involvement UP FRONT in these processes and ICES
data entry has not been stellar. We have only been reactive.
Equipment Failure Analysis
Checklist
Checklist was developed by ERWG (Equipment
Reliability Working Group).
Checklists are used by Engineering to assist in the
evaluation of equipment failures.
The checklist is being used in some fashion by all
utilities.

Checklist
Vast majority of ICES reports contain the
checklist, even if a basic or no corrective
action evaluation was performed at the site.
One section of the checklist is tied directly to
Supply Chain
1. Quality of Manufacturer / VendorYES NO N/A
Was the equipment design adequate? ☐ ☐ --
Was the vendor-supplied equipment manufactured without defect? ☐ ☐ --
Was the Quality and Workmanship acceptable? ☐ ☐ --
Was the documentation from the vendor correct and complete? ☐ ☐
Was the shipping and storage appropriate (shelf life, storage environment, no problems
caused during shipping)?☐ ☐ ☐
If Quality is a potential cause, consider creating actions to:
Perform offsite Laboratory analysis
Check similar parts from the same supplier for similar issues
Document inadequate vendor controls
Revise procurement requirements
Revise vendor documents
Revise requirements for receipt, inspection and storage (such as environment, shelf life, storage PM)

13 & 3-14 Titled “Parts Quality and Availability”.

Controls are established to monitor supplier performance,
including the proper oversight and intrusiveness at the
manufacturing facility, commensurate with the risk
significance of the equipment. Performance data and
metrics that could have an impact on plant reliability or
nuclear safety are trended and promptly communicated
back to the supplier to ensure supplier action and
continuous improvement.

Evaluate Industry events entered in
ICES with a Supply Chain Cause Code
(Cause of Event)
Purpose – Ensure data is accurate
and identify trends.

Performed detailed evaluation of all
(157) 2017 ICES events coded with a
Supply Chain Cause Code.
No Trends were identified
Review of 2017 ICES events (157)
NSCSL 2017-2018 Engagement and Actions
Developed an ICES Event Evaluation Form for use by all
Supply Chain Managers.
Distributed the forms to applicable Supply Chain
Managers as events occurred to gain concurrence of
assigned cause.
NSCSL 2017-2018 Engagement and Actions
Communicated and met with INPO to gain alignment
on 2017 scrams and trend data.
Provided a step by step desktop guideline to all
Managers for creating their own ICES Query and/or
e-mail subscription.
Initiated communication with ERWG on
enhancement to the Equipment Failure Analysis
Checklist.
Proposed addition to ERWG Equipment

2018 – 2019 Actions
Continue review and evaluation of ICES
reports to validate cause codes and identify
trends.
As trends are identified, work with the industry
to develop improvement plans.
There are already existing EPRI documents that, if used,
would improve the quality of these parts. Our goal is to
utilize existing EPRI documents that have already been
developed, e.g. EPRI Gold card Program, EPRI Motor
Refurbs, EPRI Relays.

Continue our dialogue and face to face
meetings with INPO.
Discuss status of initiative and align on future
actions at this weeks NSCSL summer meeting.
Collaborate with ERWG on possible
enhancements to the Equipment Failure
Analysis Checklist.
Our Mission
Improve Supply Chain involvement in the
Equipment Reliability Program at each
station and mitigate Supply Chain/Vendor
Quality impact on plant events.
