    National Mental Health Programme

    Programme background:

    The National Mental Health Programme (NMHP) was started in 1982 with the following objectives:

    to ensure the availability and accessibility of minimum mental healthcare for all in theforeseeable future, particularly to the most vulnerable and underprivileged sections of the

    population;

    to encourage the application of mental health knowledge in general healthcare and in social

    development; and

    to promote community participation in the mental health service development and stimulate

    efforts towards self-help in the community.

    District Mental Health Programme (DMHP) was initiated under NMHP in 1996 to provide community

    based mental health services in the country in line with the National Health Policy 2002, the objectives

    of the NMHP and global trend of community care of mentally ill. NMHP was re-strategized in 2003 and

    had the following components:

    Expansion of DMHP to 100 districts all over the country,

    Strengthening and Modernization of State run Mental Hospitals,

    Upgradation of Psychiatry Wings of Govt. Medical Colleges/General Hospitals,

    IEC activities,

    Research & Training in Mental Health for improving the service delivery.

    District Mental Health Programme (DMHP): The main objective of DMHP is to provide basic mental

    health services to the community & to integrate these with general health services. It envisages a

    community based approach to the problem, which includes:

    Training of mental health team at identified nodal institutions.

    Increase awareness & reduce stigma related to Mental Health problems.

    Provide service for early detection & treatment of mental illness in the community (OPD/

    Indoor & follow up).

    Provide valuable data & experience at the level of community at the state & center for future

    planning & improvement in service & research.

    Strengthening and Modernization of State Run Mental Hospitals: Under this scheme, a onetime grant

    with a ceiling of Rs. 3.00 crores per hospital on the basis of benchmark of requirement and level of

    preparedness is available. The grant covers activities such as construction/ repair of existing buildings,

    purchase of equipment, provision of infrastructure such as water tanks and toilet facilities, purchase of

    cots and equipments.

    Up gradation of Psychiatric Wings of Govt. Medical Colleges/ General Hospitals: This scheme is for

    strengthening the Psychiatric Wings of Govt. Medical Colleges/ General Hospitals with a one-time grant

    of up to Rs. 50 lakhs for up-gradation of infrastructure and equipment.

    Current Status:

    At present DMHP has been extended to 123 Districts, Up-gradation of 88 Psychiatric Wings of Medical

    Colleges/ General Hospitals & Modernisation of 29 Mental Hospitals have been funded under NMHP.

    Taking into account the constraints in implementation of the Programme in the 10th

    Plan i.e. shortage of

    skilled manpower in mental health and non availability of community based mental health services, the

    strategy for the Programme during 11th Plan has been drawn up with the following components:

    Extension of DMHP to more districts of the country with added components of Life SkillsTraining & Counselling in Schools, College Counselling Services, Work Place Stress

    Management and Suicide Prevention Services;

    Manpower Development in mental health by supporting establishment of Centers of Excellenceand of PG training departments in mental health specialties;

    IEC activities;

    Research and Training;

    NGO/ PPP;

    Spill over activities of Xth plan i.e. Upgradation of Psychiatric wings of Govt. Medical Colleges /

    General Hospitals and Modernisation of State-run Mental Hospitals.

    The NMHP would be mainstreamed by integrating it with NRHM and NUHM.

    The proposal for NMHP during the 11th

    five year plan with an outlay of Rs. 1000 crore was considered by

    the EFC in July 2008. Based on EFC recommendations, CCEA approval for following components of the

    Programme with outlay of Rs.473.445 crore has been obtained and sanction for the same has been

    conveyed to the State Governments.

    Manpower Development in Mental Health

    Spill over activities of the 10 th Plan (upgradation of psychiatric wings of Government Medical

    Colleges/ General Hospitals, and Modernization of Government Mental Hospitals), as per

    existing norms.

    Continuation of existing DMHPs under implementation on existing norms.

    Under the Manpower Development Schemes, Centres of Excellence in the field of Mental Health (at

    least 11 no.s) (Scheme

    A) would be established by upgrading and strengthening identified existing

    mental health hospitals/ institutes. Funding support is upto Rs. 30 crores per centre. Assistance would

    also be provided for setting up/ strengthening 30 units of Psychiatry, 30 departments of Clinical

    Psychology, 30 departments of Psychiatric Social Work and 30 departments of Psychiatric Nursing

    (Scheme - B) with the support of upto Rs. 51 lakh to Rs. 1 crore per PG department.

    Based on proposals received from the States and inspection of the provisionally identified Institutes by

    the Inspection teams of the Ministry/ Dte.G.H.S. the Standing Committee on NMHP under the

    Chairmanship of DGHS has recommended grant for 10 Institutes under Scheme A and 15 Institutes

    under Scheme B. Funds for 10 Institutes under Scheme A (Rs. 110.02 crores) and 10 Institutes for 23

    departments under Scheme

    B (Rs. 9.64 crores) have been released.

    Approval for expansion of DMHP with added components, IEC activities, NGO/PPP schemes, Research &

    Training etc. was to be taken up separately as desired by EFC after an external evaluation of the existing

    programme. Evaluation studies of the major components of the Programme have been completed

    which has recommended the expansion of DMHP with additional service components. The following

    additional components were submitted to the EFC for approval, apart from the manpower

    development, spillover activities and evaluation components, which had been approved in March 2010:

    i. District Mental Health Programme: There are 123 ongoing DMHPs in the country. As

    recommended by EFC, the number is to be restricted to 123 districts. Future expansion will be

    undertaken in the 12th

    plan period once the existing DMHPs are consolidated with newer

    components proposed in the current plan and more skilled manpower is available in the country as

    a result of proactive measures by the NMHP to increase the PG training capacity in mental health

    related specialties.

    Additional activities, that is, promotive and preventive activities, have been proposed in the existing

    DMHPs in addition to ongoing components of early identification and treatment. These include Life

    Skills Education and Counselling in Schools, College Counselling Services, Work Place StressManagement The unit costs have also been changed.

    ii.IEC Activities: Innovative IEC strategies involving Electronic/ Print/local media at Central level to reduce

    stigma attached to mental illness and increase awareness regarding available treatment and health care

    facilities. Increased awareness regarding provisions under Mental Health Act 1987

    iii. Monitoring & Evaluation: At Centre & State level to support for monitoring, implementation, evaluation

    etc.

    iv. Training & Research: To address shortage of manpower in mental health in short term basis for DMHPteams. To support CME, Distance Learning, E-Mode learning. Addressing specialty training areas such as

    Child Psychiatry, Geriatric Psychiatry etc. Operational Research related to NMHP.

    v. Support for Central/ State Mental Health Authority: To assist in regulation development, direction and

    coordination with respect to Mental Health services. Support proposed for Establishment &

    maintenance of a small technical secretariat during the plan period.

    Upgradation of Psychiatric Wings of Govt. Medical Colleges/ General Hospitals:

    Every medical college should ideally have a Department of Psychiatry with minimum of three

    faculty members and inpatient facilities of about 30 beds as per the norms laid down by the

    Medical Council of India. There should be outpatient facilities, as well as facilities for providingvarious forms of therapy including Electro-convulsive therapy. In addition to psychiatrists,

    psychiatric social workers, and psychiatric nurses are essential for an ideal psychiatric

    department in a medical college. Out of the existing medical colleges in the country,approximately 1/3rd of them do not have adequate psychiatric services.

    The following strategies were adopted for up gradation of the undergraduate and postgraduate

    training in psychiatry in medical colleges:

    Provision for independent psychiatry department in medical colleges where they do notexist.

    Up gradation of existing psychiatry services including provision of inpatient services,with necessary equipment; provision of trained manpower.

    The department of psychiatry to be responsible for implementing and providing support

    for execution of National Mental Health Programme/DMHP. Facilities to train psychiatric social workers and nurses; provision for accreditation and

    licensing for health professionals.

    Provision of enhancing capacity of existing central/state governmental institutes/collegesto train manpower.

    The selected medical college will be required to send periodic report to the Ministry ofHealth and Family Welfare through the state Government concerned regarding progress

    of the scheme/activities/expenditure.

    On the above lines, a scheme for strengthening the psychiatric wings of general hospitals and

    Medical Colleges in the Government sector provides for a one-time grant of Rs.50 lakhs for up

    gradation of infrastructure and equipment. This grant is sanctioned on receipt of a writtenproposal from the state government giving details of the requirement. The grant covers:

    1. Construction of new ward.

    2. Repair of existing ward.

    3. Procurement of items like cots and tables.

    4. Equipment for psychiatric use such as modified ECTs

    The grant does not cover recurring costs such as salary and other running expenses.

    Proposals received from the State Governments are referred to a committee which lays down

    minimum standards for quality of construction and facilities and ensure that the facilities meet

    minimum standards. Where a central agency is engaged in construction/ repair of a ward, the

    funds would be released directly to that central agency.

    Funds were released to88 Institutes in the country under this scheme. The details of funds

    Strengthening and Modernization of State Run Mental Hospitals

    As per the existing scheme to modernize the existing state-run mental hospitals, a one-time grant

    with a ceiling of Rs.3.00 crores per hospital on the basis of benchmark of requirement and level

    of preparedness is available. The grant would cover activities such as :

    Construction/repair of existing buildings

    Purchase of equipment

    Provision of infrastructure such as water- tanks and toilet facilities

    Purchase of cots and equipments.

    It would not cover expenses in the nature of salaries and recurring expenses towards

    running the mental hospitals and cost towards drugs and consumables.

    Proposals received from the State Governments are referred to a committee, which lays down

    minimum standards for quality of construction and facilities and ensure that the facilities beingproposed meet minimum standards. Where a central agency is engaged in construction or repair

    of existing building, the funds would be released directly to the central agency.

    Funds have been released to 29 Mental Hospitals in the country under this scheme. The details

    Manpower Development Schemes

    Centres of Excellence in Mental Health (Scheme A)

    Centres of Excellence in the field of Mental Health will be established by upgrading andstrengthening identified existing mental health hospitals/ institutes for addressing the acute

    manpower gap and provision of state of the art mental health care facilities in the long run. These

    institutes will focus on production of quality manpower in Mental Health with primary aim to

    fulfill the primary needs of the National Mental Health Programme.

    The support would involve :

    Capital work (Academic block, Library, Hostel, Lab, Supportive Departments,

    Lecture Theatres etc.)

    Equipments and furnishing

    Support for faculty induction and retention for the plan period.

    Preference is given to centres where the State Government shows commitment for faculty and

    other recurring expenditure annually.

    Budgetary support is upto Rs. 30 crore per centre. At least 11 Centres of Excellence will be

    established under the Scheme during the 11th

    Five Year Plan period. The commitment from the

    State Governments will be required.

    Scheme for Manpower Development in Mental Health (SchemeB)

    To provide an impetus for development of Manpower in Mental Health and other training centres

    (Govt. Medical Colleges/ Govt. General Hospitals/ State run Mental Health Institutes) would

    also be supported for starting PG courses or increasing the intake capacity for PG training in

    Mental Health. The support involves :

    Physical work for establishing/ improving department in specialties of mental health

    (Psychiatry, Clinical Psychology, Psychiatric Social work, and Psychiatric Nursing)

    Equipments

    Tools

    Basic infrastructure (hostel, library, department etc.)

    Support for engaging faculty etc.

    Support is being provided for setting up/ strengthening 30 units of Psychiatry, 30 units of

    Clinical Psychology, 30 units of Psychiatric Social work and 30 units of Psychiatric Nursing

    with the support of up to Rs. 51 lakh to Rs. 1 crore per PG department. The commitment to take

    over the entire funding of the scheme after the 11th Five Year Plan period from the State

    Government is required.

    Funds have been released to 10 Institutes under Scheme A and 23 PG Departments (in 10

    Institutes) under Scheme B in the country. The details of funds released are enclosed.
