Introduction:

The increasing number of aged people among developed and developing countries led dementia to become a prime public health concern.

Fortunately new drugs have not ceased to be released: 1993 Tacrine, 1996 Donepecil, 1998 Rivastigmine, 2001 Galantamine, 2003 Memantine.

Neurobiological or biochemical interventions will certainly be the ones that in future will solve this sad illness.

In the years to come until the illness is overcome, however, we can not leave aside another fundamental question: the care and quality of life of millions of people that precisely due to dementia are, ever more, forced to live institutionalized in nursing homes or day care centres.

The role of Non Pharmacological Therapies (NPT) for these institutionalized or semi-institutionalized people is at least as important as adequate pharmacological intervention.

Sufficient evidence has been provided in the last years to establish the clinically relevant role of cognitive and motor therapies as well as caregiver support among other. Outcomes are better cognition, function, behaviour, and mood, and even delay of institutionalization.

Lately, however, al kinds of “therapies” are mushrooming. As a result of such a variety of proposals patients, the general public and even professionals become confused. This confusionsheds doubt on the clinical value in a variety of ways:

1. There is no consensus about the meaning of the word “Therapy”2. There are no guidelines on interventions (which therapies, for what problem, in what

stage, and what potential adverse effects may be expected)3. The field also lacks methodological research standards to enable efficient comparison

of results.

Internationally leading researchers will try to solve the following problems from the perspective of Evidence Based Medicine:

1. Reach a consensus definition for “Non Pharmacological Therapy” related to AD2. Settle which NPT achieve what outcomes in relation to stage and domain3. Agree on common avenues of research

For the sake of patients and their caregivers all those persons and institutions involved in the project want to contribute to the clinical consolidation of these new treatments.
