    Inspection Notes

    1. BIN Plate is located at Begin Abutment and is in good condition.

    2. Stone facing at both Abutment Stems, Wingwalls and Pier nose were inspected and found tobe in good condition.

    3. Debris / Land Use form has been prepared and attached to this report.

    4. The underside of the structural deck is covered with SIP forms in all Bays, except for Bay 6and is in satisfactory condition.The Underside of the fascias and concrete deck in Bay 6 werevisualy inspectected and approximately 20% was sounded. No loose concrete was found.

    At the top of the deck, the Begin Abutment Joint sealer material over the Westbound lanes ispushed out across the length and shredded for 1.5' long over the right lane. On the Eastboundlanes, there is only minor debris accumulation.

    Below the deck, there is sign of leakage on several Bays, as water and rust stains are presenton the Backwall.

    Rating is downgraded to 4.

    At the top of the deck, the End Abutment Joint has minor debris accumulation across its length.The bridge side concrete header has minor edge spalls along the armor angle edge.

    Below the deck, there is sign of leakage on several Bays, as water and rust stains are presenton the Backwall.

    Rating is downgraded to 4.

    The top of the Bridge Seat has pigeon droppings up to 4" high at various locations. Thiscondition can trap moisture and cause deterioration of the Begin Abutment bridge elements.
