• freedom from claims filing; • preventive and stay-well services; • full range of specialty care; and • more than 500 UK Healthcare physicians.

You and any dependents you intend to cover must live in the following counties or within a 50-mile driving radius of a Lexington area UK HealthCare facility to participate in UK-HMO: Anderson, Bourbon, Clark, Fayette, Franklin, Jessamine, Madison, Mercer, Scott or Woodford. You may enroll your eligible dependents that live in the UK-HMO Lexington Service Area (outlined above). To be a dependent you must:

• Primarily reside in the UK-HMO Service Area (temporary absences from the service area with the intent to

return within 120 days will not interrupt the Member’s primary residence in the service area. Members experiencing absences from the service area for a period of 121 or more days, regardless of intent to return, are required to contact UK Employee Benefits to make arrangement for enrolling in an alternative benefit structure.)

• Meet the University of Kentucky’s eligibility requirements for Dependent coverage. • Be the legal spouse of the Member; or • Be a Dependent child of a Member who satisfied the Definition of Dependent • Be a Child of a Sponsored Dependent who satisfied the Definition of Child of Sponsored Dependent • The Dependent age limit is up to age 26.

Highlights of UK-HMO Lexington Service Area Benefit Plan This brochure is not a contract. It is a partial listing and summary of benefits and services. For complete details on covered services, exclusions and benefit limitations, refer to the Certificate of Coverage Booklet.

Medical Services UK-HMO Coverage Choice of physician UK HealthCare participating physicians (see

directory beginning on page 5) Choice of hospital UK Albert B. Chandler Medical Center; UK

Children’s Hospital; and UK Good Samaritan Hospital

Preventive Care: Routine Pap smears, mammograms, PSA, screening colonoscopy and sigmoidoscopy. Routine child care and immunizations (through age 18) Routine adult physical exam (19 years and above, one per plan year.

100% covered

Physician (Primary Care) office visits for diagnosis and treatment

$10 co-pay (except allergy injections, see below)

Physician (Specialist) office visits for diagnosis and treatment

$30 co-pay

Allergy injections $10 co-pay Maternity care No office visit co-pays are applied; Inpatient co-

pay will apply upon admission. Prescription drugs Prescription drugs are covered under a

separate plan through Express Scripts and UK Employee Benefits

Chiropractic and physical therapy services; acupuncture and pool therapy/exercise hydrotherapy; speech therapy; occupational therapy; cardiac rehabilitation; and pulmonary rehabilitation.

$15 co-pay per visit – 45 visit limit per plan year for all therapies combined. (If a UK-HMO member completes greater than 24 cardiac or pulmonary rehab visits, we will reimburse them ½ of their co-pays for that specific service.)

Autism $500 maximum monthly benefit for children ages 2 to 21 for therapeutic, respite and rehabilitative care

Outpatient mental/nervous care and substance abuse care rendered by licensed psychiatrists and psychologists

$30 co-pay

Inpatient mental/nervous care and substance abuse care $200 co-pay per admission Hospital services, including semi-private room and board Hospital services, observation stay

$200 co-pay per admission $75 co-pay per admission

Emergency/accident (or urgent) care Ambulance services (including air ambulance)

$100 co-pay per emergency department visit; waived if admitted to hospital $15 co-pay at Pediatric Twilight Children’s Clinic $25 co-pay at Lexington Urgent Treatment Centers (Boardwalk, Custer, and Dove Run) and Nicholasville UTC (Bellaire Drive) $75 co-pay

Outpatient tests, lab and x-ray, and other diagnostic tests Tobacco Cessation Counseling (only covered when services provided by UK Tobacco Treatment Specialist)

Two Convenient Options for Enrollment

Online Option To enroll online, please follow these simple steps:

Visit the myUK website at https://myuk.uky.edu.

1. Enter the link blue user ID and password. 2. Click on the “Personal Information” tab and then click on “Family/Related Persons’ to add any

dependents wish to cover and/or beneficiaries. a. -After entering all dependents information, click “Review, then click “Save.”

3. Click on “Benefits and Payment” tab and then click under “Benefits Adjustment Reason Enrollments” the “Open Enrollment” tab.

4. Select the plan you want to “Add” or “Edit”. You will then see the plan option. Highlight the plan you want to select and choose the level of coverage.

5. If you need to edit dependents then click “select dependent”. 6. If you have elected not dependents then the next screen will ask you to “Add Plan to Selection”. 7. Once all changes have been made then you will need to “Review Enrollment”, then click “Save”. 8. To view new and existing plan, click on “Print New Plan Sections” and print.

Paper Option If you’d rather complete a health plan enrollment form, the form is available online at www.uky.edu/HR/benefits/enrollmentforms or at the UK Benefits office in 112 Scovell Hall or 375 Peterson Service Building.

On this form:

a. you will select a coverage level (i.e.,employee only; employee + spouse or sponsored dependent; employee + child(ren); or employee + family).

b. If you are covering dependents, you need to complete the section under "additional enrollment information" on the form.
