Dr. Farnsworth is a progressive and active leader promoting health-system pharmacy at the local and state levels, as well as a strong advocate for clinical pharmacy practice and leadership development within the Veterans Health Administration. For over 15 years, Francine has held several Clinical and Administrative positions with VHA Pharmacy and currently serves as the Chief, Pharmacy Services at the Corporal Michael J. Crescenz VA Medical Center in Philadelphia, Pennsylvania.

She received her Doctor of Pharmacy Degree from Temple University, School of Pharmacy and successfully completed both a Post-Graduate Year-One Residency at the Salem VA Medical Center in Salem, Virginia and a

Post-Graduate Year-Two Residency Specializing in Ambulatory/Primary Care at the Medical College of Virginia/Virginia Commonwealth University in Richmond, Virginia, respectively.

Mitch G. Sobel, was appointed to the National Association of Boards of Pharmacy (NABP) Long Term Care Task Force by the President of NABP. This task force examines the provision of pharmacist care in the Long-Term Care, Nursing Home, and Assisted Living setting. Mr. Sobel was appointed by Governor Christopher Christie to serve on the New Jersey State Board of Pharmacy in March of 2014. Mr. Sobel is currently serving on the NJ Board ofPharmacyasChairofthePharmacyTechnicianCertificationSubcommitteeandtheNJDepartmentofHealthDrug Diversion Coalition

Mitch G. Sobel is currently the Director of National Accounts for CutisPharma a specialty non-sterile 503A compounding company from Boston, Massachusetts. He also serves as a Consultant Clinical/Regulatory Affairs

Pharmacist for Generation Pharmacy Group, a long-term care pharmacy provider. He is a Consultant Pharmacist/Industry Liaison/Partner for GenerRxMed, a Pharmacist-Driven pharmacogenomics clinical software solution for personalized medicine. He is currently adjunct professor for Fairleigh Dickinson University School of Pharmacy teaching a required course in Pharmacy Leadership and Management and Pharmacy Practice Law. He also is an adjunct professor for Rutgers University Ernest Mario School of Pharmacy teaching Controlled Substances and Regulatory Affairs sections of an elective course in Hospital Pharmacy Practice.

Mr. Sobel is a graduate of the University of the Sciences in Philadelphia, Philadelphia College of Pharmacy, and received his Masters in Administrative Science at Fairleigh Dickinson University. In addition, Mitch is a past Resident from the American Society of Health-System Pharmacists (ASHP) Residency Program. Mitch is also a graduate of the ASHP Pharmacy Leadership Institute at the Boston University SchoolofManagement.HeisalsoaNewJerseyPharmacistImmunizer.MitchisaCertifiedPharmacogenomacistandreceivedhistrainingfrom the University of Pittsburgh School of Pharmacy and NACDs.

Mr. Sobel has worked in numerous pharmacy corporate, director, operations and managerial roles at St. Joseph’s Health in Paterson and Wayne, St. Michael’s Medical Center in Newark, Saint Barnabas Medical Center in Livingston, and The Valley Hospital in Ridgewood, New Jersey.

Outsideoftheoffice,MitchSobelhasbeenPresidentoftheNorthChapterandDirectorofOrganizationalAffairsoftheNewJerseySociety of Health-System Pharmacists (NJSHP). Mitch served for two terms (2007 and 2012) as President of the NJSHP and served as an American Society of Health-System Pharmacists Regional Delegate for six years representing NJSHP with 3 other Board Members. He has received the Greg Santora Leadership Award from NJSHP (2012) and was awarded as Fellow for ASHP (2012). He also participated with the Greater New York Hospital Association Pharmacy Advisory Committee.

In addition, Mr. Sobel has served on NJ Governor John Corzine’s Task Force On Health Care Professional Responsibility and Reporting, The Division of Consumer Affairs Pharmacy Controlled Substances Security Committee, and The NJ Department of Health Emergency Preparedness Standards Committee.

Mr. Sobel has presented numerous posters and lectures on pharmacy law, clinical practice, and pharmaceutical practice topics for Fairleigh Dickinson University School of Pharmacy, Rutgers University - Ernest Mario School of Pharmacy, University of the Sciences - Philadelphia College of Pharmacy, Montclair State University, Sports Medicine Department, ASHP, NJSHP, and New Jersey Pharmacists Association (NJPhA).

Daniel T. Abazia, Pharm.D., BCPS, CPEOpioid Stewardship and Palliative Care

Daniel Abazia is Pharmacy Clinical Coordinator at Capital Health and adjunct clinical associate professor in the Department of Pharmacy Practice and Administration at the Ernest Mario School of Pharmacy. He received his Doctor of Pharmacy degree from Rutgers in 2005 and completed a PGY1 Pharmacy Residency at the Albert Einstein Medical Center in Philadelphia, PA in 2006. Dr. Christine Lam, Pharm.D., BCPS, CDE, BCACP, BCGP, a native of Philadelphia, Pennsylvania, graduated with a Doctor of Pharmacy degree at Temple University School of Pharmacy in Philadelphia, Pennsylvania. Dr. Lam completed a year of ASHP-accredited PGY1 residency at Lehigh Valley Health Network in Allentown, Pennsylvania, followed by a year of ASHP-accredited PGY2 specialty residency in Ambulatory Care at the Medication Management Center at St. Rita’s Medical Center in Lima, Ohio.

Dr. Lam joined Fairleigh Dickinson University School of Pharmacy and Health Sciences in August 2015. As Clinical Assistant Professor of Pharmacy Practice, Dr. Lam serves as a preceptor to students during their experiential rotations in Ambulatory Care pharmacy practice and assists with teaching at the School of Pharmacy. Dr. Lam’s practice site is located at Morristown Medical Center, Morristown, New Jersey. Dr. Lam’s role as a clinical pharmacist involves managing an ambulatory care pharmacy practice site where she develops, implements, and manages patient-centered care programs focused on diabetes, anticoagulation, and transition of care.

Amulya Uppala, Pharm.D., BCPSTransition of care in the underserved population: Inpatient and outpatient perspectives

Amulya currently practices as an inpatient transitions of care pharmacist at Overlook Medical Center. After completing my pharmacy degree with Rutgers, Amulya did her residency with Atlantic Health System based from Morristown Medical Center and Overlook Medical Center from July 2015 to June 2016. She also completed a transitionsof care certificateprogramwithNew Jersey SocietyofHealth-SystemsPharmacy. Sincebeginningher role at OMC, she has helped develop and implement the pharmacy-based transitions of care program. This includes participating in a pilot program where the team demonstrated the positive impact of pharmacy-based TOC services in the cardiac, general medicine, and pulmonary units. Over the past two years, she has provided pharmacy services to the cardiac telemetry unit in OMC and the COPD team initiatives throughout the hospital. Services include discharge counseling, medication education, disease-state education, inpatient pharmacotherapy

review, and access to medications at the point of discharge.

Thomas Carlough, Pharm.D.Big Data-Driven Population Health

Dr. Thomas Carlough began as a graduate of the school of pharmacy at Temple University and has found himself 10 years later as the director of data science and analytics with Atlantic Health System in northern New Jersey. Hewasoncearecoveringprogrammerasthedotcomerafizzledandwaspulledbackintothedarkandoftenconvoluted arts of computer science, now informatics, when it became clear that the clinician and technology together are more powerful than either alone. For the last two years he has been building a team of dedicated health care technologists and data scientists developing models and analyses intent on breaking down some of the wicked problems existing in healthcare. Mary Bridgeman received her Doctor of Pharmacy degree from the Ernest Mario School of Pharmacy, Rutgers University. Upon graduation, she completed a pharmacy residency at Robert Wood Johnson University Hospital. Mary has held a faculty appointment at the School of Pharmacy at Rutgers since 2007 and is currently Clinical Associate Professor, Department of Pharmacy Practice. Mary maintains volunteer faculty appointments in the Department of Education, Rutgers-Robert Wood Johnson Medical School and in the Physician Assistant Program, Rutgers-School of Health Professions, in addition to her practice site in Internal Medicine at Robert Wood Johnson UniversityHospital.MaryisrecognizedasaBoardCertifiedPharmacotherapySpecialist,BoardCertifiedGeriatricPharmacist, and Fellow of the American Society of Consultant Pharmacists.

Mary is actively involved in societies that serve the pharmacy profession, including the New Jersey Society of Health-Systems Pharmacists, the American Society of Health-Systems Pharmacists, and the Accreditation Council for Pharmacy Education, to name a few. At the university and hospital, Mary is involved in committees charged with governing student development and patient care, respectively. Jennifer Costello, Pharm.D., BCPS, BC-ADMThe Great Pretender - Predatory Journals

JenniferCostello, PharmD, BCPS, BC-ADM is theWorld-Wide Publication and ScientificContent Lead forCardiovascular Disease at Bristol-Myers Squibb (BMS), Lawrenceville, NJ. Previous to this role, Jennifer was the Residency Program Director /Assistant Director of Clinical Services at Saint Barnabas Medical Center and adjunct faculty at the Ernest Mario School of Pharmacy at Rutgers University.

Jennifer earned her PharmD from Northeastern University in Boston, Massachusetts. After completing a PGY-1 residency at University of Arizona/University Medical Center in Tucson, Arizona, she returned to NJ to begin a 15-year career in hospital pharmacy in various roles within the health care institution.

Dr. Costello has published numerous peer-reviewed articles and provided lectures locally and nationally in a variety of areas namely, pediatrics, cardiology and endocrinology. Jennifer is a longstanding member of ASHP and NJSHP and was the recent recipient of the NJSHP Beverly Wilt Service Award. She remains committed to community service organizations and through BMS was able to organize a recycle, reuse, reGive campaign that supported pediatric child life services at 6 New Jersey hospitals in 2018.

This clinical leadership meeting covers topics such as new clinical initiatives in both the inpatient and outpatient environment, the impact of consolidation on Pharmacy and Therapeutics processes, updates on provider status efforts at member institutions, and advocacy issues for NJSHP and/or ASHP. Key clinical pharmacy leaders from the leading health systems in New Jersey will be attending this meeting. This is your chance to discuss key issues with colleagues across the state. STUDENT COLLEGE BOWL COMPETITIONTeams will be challenged to provide answers to contemporary pharmacotherapy and pharmacy practice questions to score points for their team. Based on a “jeopardy style” format, this opportunity for students to demonstrate their knowledge and skills will be held from 2:30 - 4:30 PM on Friday, March 29th. Plan to sign-up to participate. You can be a winner for your school! 1st team prize is $100 per person and 2nd team prize is $50 per person.

POSTER SESSIONPosters will be on display during the exposition from 4:30 - 6:30 PM on Thursday and 2:30 - 4:30 PM on Friday in the Grand Exhibition Center. For those submitting posters, please make sure you are by your poster.

PRIZES AWARDEDSeveral prizes will be awarded. The drawing will take place from 4:30 – 6:30 PM on Thursday, March 28th, and from 2:30 – 4:30 PM on Friday, March 29th.

CONTINUING EDUCATION CREDITSThe New Jersey Society of Health-System Pharmacists (NJSHP) is accredited by the Accreditation Council For Pharmacy Education, as a provider of continuing education. (Completion of the post-meeting evaluation is required within 30-days to record participation. CE certificates will be submitted to ACPE only to those who complete the post-meeting assessment.)

The New Jersey Board of Pharmacy has granted 1.5 LR and 0.5 Law contact hours of continuing pharmacy education for attendance at the College Bowl Competition. (Continuing Education certificates will be emailed within eight weeks of the program.)

CANCELLATION POLICYCancellation requests must be received in writing to NJSHP headquarters. A $25.00 administrative fee will be deducted from the registration fee for cancellations received by 12-noon on or before March 15, 2019. No refunds after 12-noon on March 15, 2019.

Reservations will be held on a guaranteed basis until 10 pm on the date of arrival. Parking: Valet and self-parking facilities are available for over night guests at $10 per visit with unlimited in and out parking privileges. Attendees not requiring overnight accommodations are subject to a $10 self-parking fee or a $15 valet parking fee.
