1 SYNOPSIS

1.1 Investigation Board Members

1.2 The Accident Investigation Board was comprised of the following personnel

1.3 Investigation Methodology

• Following the events on-board the MS Dieppe Seaways (Owned by DFDS), the Duty Brigade Manager implemented the Significant Safety Event procedure. This decision was made due to the injuries to Kent Fire and Rescue Service and DFDS Personnel.

• The Duty Brigade Manager then chaired a Management and Co-ordination Group (MACG) meeting.

• A Kent Fire and Rescue Service Significant Safety Event team was established; this comprised of Group Manager (GM) Colin King, Health & Safety (H&S) Senior Health and Safety Advisor Chris Adams, Fire Brigade Union (FBU) H&S representative Mark Brown and Station Manager (SM) David Wales as support officer.

• GM King and Chris Adams attended the scene. This was to gather evidence by photographing the scene, gather impounded equipment and collect witness statements.

• It was initially difficult to locate all impounded equipment, such as Breathing Apparatus sets as they had been re-used at the ongoing incident.

• The Significant Safety Event team met with the Marine Accident Investigation Branch (MAIB) at Dover Docks. Kent Fire and Rescue Service offered a Fire Investigation and Research Team to support the MAIB investigation.

• The Health and Safety Executive were made aware of the occurrence by Chris Adams and RIDDOR procedures completed.

• The Kent Fire and Rescue Service Organisational Development was requested to complete an audit of PDRpro (Maintenance of Competency Evidence System) and training records for the individuals involved at the time of the occurrence.

• Kent Fire and Rescue Service Training Manager SM TMgr was requested to investigate the current Marine firefighting training regime. This investigation included current operational guidance, with the support of SM TMgr supp.

• The Significant Safety Event collected all documentation from the Incident after its conclusion. This documentation included Analytical Risk Assessments and Message Notepads.

• A meeting was held at Kent Fire and Rescue Service Headquarters on the 3 May 2014 which involved the Significant Safety Event Investigation Team, the Kent Fire and Rescue Service H&S Manager 1, FBU 2 (FBU, Kent), FBU Nat (FBU, National), MCA 1 (Maritime & Coastguard Agency), GM ESFRS 1 (East Sussex Fire & Rescue) and Director Operations ACO 1 and Director Major Projects and Resilience DCFO 1.

• The Significant Safety Event team went back on board MS Dieppe Seaways on the 3 May 2014, with the support of MCA 1 of the MCA. This was to interview any crews still on board and also take more photographic evidence. The crews had been changed following the incident, so no DFDS crews were available to be interviewed.

• The Significant Safety Event team arranged for interviews of all Kent Fire and Rescue Service crews involved on the deck at the time of the occurrence, along with the Incident Commander GM 1 and Command Support Officer SM 2

• The Significant Safety Event team photographed all Kent Fire and Rescue Service Personal Protective Equipment involved and this was sent back to Cosalt to be repaired if required.

• The Kent Fire and Rescue Service Entry Control Board information was downloaded by the Breathing Apparatus technician.

• The Kent Fire and Rescue Service Breathing Apparatus Sets involved were inspected by Kent Fire and Rescue Service technicians and then sent to Drager for a further inspection.

• It was not possible to interview the DFDS staff, as their insurers Hill Dickinson did not want their staff to conduct internal interviews, MAIB interviews and then Kent Fire and Rescue Service interviews. Appendix 1 E-mail from Hill Dickinson

• The Significant Safety Event team supported the MAIB investigation by providing witness statements and interview transcripts. The team also supplied operational guidance to the MAIB in the form of Fire Service Manual, Volume 2, Fire Service Operations, Marine Incidents. The team has also co-ordinated the interviewing of Kent Fire and Rescue Service staff for MAIB interviews.

• The Significant Safety Event team has been provided with the Fire Investigation and Research Team investigation report into the origin and cause of the fire.

• No access has been given to the MAIB evidence or findings and this information will become available when their final report is issued in approximately six months.

1.7 Summary of the Accident

On the 1 May 2014 Kent Fire and Rescue Service crews attended a fire on-board MS Dieppe Seaways ferry. WM 1 was the officer in charge of the first appliance. He along with SM 1 assembled a crew on deck nine on the portside of the ferry. Appendix 2 – Deck Nine Crews received information stating that the fire was in the portside boiler and was contained. KFRS Officers were informed that the fire had been supressed by the DFDS Breathing Apparatus crew at sea. A DFDS Breathing Apparatus crew opened the compartment door, leading to the portside boiler on deck nine; shortly after this a jet of flames and heat erupted out of the door. This caused injury to four Kent Fire and Rescue Service and six DFDS crew on deck nine. All of the personnel on deck nine portside followed the escape route to a safe area inside the ship and located themselves in a galley and rest room inside the ship. Kent Fire and Rescue Service injured personnel were escorted off the ship and went straight to a Kent Fire and Rescue Service first aid point located dockside. The incident then continued for a further eleven hours until conclusion.

2 NARRATIVE OF EVENTS

2.1 The following section is a detailed, step-by-step factual account of the events and circumstances

surrounding the occurrence and is based on facts and details obtained during the investigation.

2.2 The MS Dieppe Seaways is a ferry which operates between Calais and Dover.

14:12 Kent Fire and Rescue Service were called to a fire on the MS Dieppe Seaways ferry

portside boiler. The information provided to Fire and Rescue Control Centre from the Port of Dover

Police was that there was a small fire contained by the ships fire crew. The ship requested support

from Kent Fire and Rescue Service.

14:12 Fire and Rescue Control Centre mobilised 16R1 the rescue appliance from Dover and 18P1

the pumping appliance from Whitfield. Both appliances were on station and each had a crew of

four, comprising of a Watch manager, Emergency Fire Appliance Driver (EFAD) and two

competent Breathing Apparatus wearers on each appliance.

14:12 Fire and Rescue Control Centre also mobilised the nearest Level 2 Officer/Tactical Advisor.

This officer was SM 1.

Upon arrival at Dover Eastern Docks, 16R1 collected the hydrant keys from the Port Of Dover

Police, following the guidance in Tactical Deployment Plan for Dover Docks.

Both appliances were escorted to the MS Dieppe Seaways which was ready to be berthed at

berth six. The Level 1 Incident commander was WM 1. WM 1 met with two Port Authority

personnel and they said “It’s a contained fire within the boiler on the port side, it was attacked

and supressed at sea by the French crew” They stated that there were 360 passengers but

they did not have information about crew numbers.

WM 1 observed black smoke coming from the funnel and he stated that it also appeared to be

coming out of the port side too.

WM 1’s immediate priority was to ensure that the passengers were off the vessel as quickly as

possible. WM 1 asked about passenger numbers and was informed of 365.

It was explained to WM 1 that there had been some kind of mechanical breakdown inside the boiler that had caused a pressurised fuel leak where the oils had become mixed and somehow caused a fire contained inside the boiler. WM 1 asked what actions had been taken and they said that they had used dry powder and suppressed the fire. WM 1 asked about fixed installations and was told that everything had been isolated; the fuel had been shut off, the electrics isolated and ventilation shut down. 14:25 WM 1 sent an informative message - “WM 1 – DFDS ferry with 365 passengers on board – ferry about to dock, fire in portside boiler room, contained but not extinguished, WM 1 currently liaising with ship crew – Delta mode while carrying out risk assessment” One of the ship’s crew was waiting to meet WM 1 and handed him an envelope that contained the ship’s plans. Straight away he took WM 1 to deck nine. The crew member did not speak English. On Deck nine WM 1 observed around 15-20 DFDS personnel that were on the open deck area.

Appendix 4 Plan of Deck Nine Portside

Some of the DFDS personnel were wearing Breathing Apparatus sets but were not ‘under air’. WM

1 was introduced to the Chief Engineer Anthony Paineau.

14:32 SM 1, in attendance.

14:35 WM 1 sent an assistance message to make pumps eight and Height Vehicles one. WM 1 also confirmed with WM 2, that the water supplies were secured dockside and that confirmed that the Port Of Dover had stopped bunkering (supplying water to ships) to enable water supplies for firefighting. WM 1 looked at the ship’s plans and identified an identical boiler on the starboard side of the ship. WM 1 and the Chief Engineer went to the starboard side boiler room. The Chief Engineer explained to WM 1 the actions the DFDS crew had taken to suppress the fire and also the actions that the crews needed to carry out on the portside boiler. The Chief Engineer showed the two access points on the boiler that could be opened to apply dry powder to extinguish the boiler fire. WM 1 checked the emergency exits and the walkway inside the boiler room. 14:44 The rescue appliance from Folkestone (19R1) attends the incident. The crews on this appliance are as follows:

SM 1 entered the ship via deck three with four Kent Fire and Rescue Service Breathing Apparatus crew and one Entry Control Officer. They are carrying two dry powder extinguishers and two Thermal Image Cameras. They proceeded to deck nine.

The Breathing Apparatus wearers Firefighter (FF) 2, FF 3, FF 6, FF 5, the DFDS crew, FF 4, WM 1 and the Chief Engineer went to the boiler on the starboard side. Tactics were discussed and what methods the ships crew had used so far. The Chief Engineer demonstrated the actions for crews when dealing with the boiler fire on the portside.

All the crews returned to the portside deck nine. SM 1 met WM 1 who was with the Chief Engineer, now at the portside on deck nine. WM 1 and the Chief Engineer formulated a plan to attack the fire in the boiler room. The plan was to use four Breathing Apparatus wearers, made up of two Kent Fire and Rescue Service and two DFDS. The DFDS crew would enter the boiler compartment using the entry code. This crew would relieve the pressure in the boiler and then open up an access point into the boiler. The Kent Fire and Rescue Service crew would enter the boiler compartment behind the DFDS crew carrying a main jet from the ships main and also a dry powder extinguisher. They would apply the dry powder into the boiler.

The Chief Engineer informed WM 1 that he was monitoring the temperatures within the boiler room; he felt it was still a bit hot to go in but temperatures were falling. The Chief Engineer explained that there had been a small fire in a domestic heating boiler on the

portside. It was decided that the Chief Engineer, SM 1 and WM 1 should go to the starboard side to look at an identical boiler and the structure it was housed in. WM 1 was still in charge of the incident at this stage as SM 1 wanted to gather information and make a decision when to take over. WM 1 and SM 1 then went to the starboard side boiler and WM 1 talked SM 1 through the plan. The Chief Engineer told SM 1 that they were about to pump the heavy fuel from the boiler on the portside to another tank. The Chief Engineer reassured SM 1 that this was safe. 14:50 SM 2, in attendance. 15:00 WM 1 and SM 1 discussed the incident, confirmed that they had heard the Chief Engineer correctly and then SM 1 took over as level 2 Incident Commander. SM 1 states in his transcript ‘The Chief Engineer was the link to the Captain therefore there was no need to visit the bridge’. WM 1 and SM 1 return to the portside. WM 1 was now nominated as Operations Commander. 15:03 GM 1, in attendance. 15:07 SM 3, in attendance.

SM 2 takes over as Command Support Officer. WM 1 is informed that the temperatures have dropped in the boiler room and the DFDS crew are happy to go in. WM 1 nominates FF 2 as Breathing Apparatus number one and FF 3 as Breathing Apparatus number two. WM 1 provides a brief for FF 2 and FF 3 to enter the boiler compartment, follow the DFDS crew, taking the ships main and a dry powder extinguisher. They were to apply the dry powder after the DFDS crew have relieved the pressure and opened an access point on the boiler. 15:15 GM 1 takes over as Incident Commander. FF 4 as Entry Control Officer asks WM 1 to repeat the brief for him as he didn’t hear it when WM 1 was briefing the Breathing Apparatus crew. The brief was repeated for FF 4, FF 2 and FF 3. There is a copy of the ships plans located at the Entry Control Point. FF 2 and FF 3 then go through their start up procedure at the Entry Control Point. FF 4, FF 9 and FF 5 assist the Breathing Apparatus crew in getting their Personal Protective Equipment on ready to enter the compartment. FF 4 enters the Breathing Apparatus tallies into the Entry Control Board; he recognises that the Entry Control Board isn’t working correctly. FF 4 then used the printed scale on the Entry Control Board to calculate the ‘time of whistle’ of the Breathing Apparatus crew. There is no evidence of the DFDS crew following an Entry Control Procedure. FF 2 and FF 3 then made their way towards the entry point. FF 2 tests the branch that is connected to the ships main to ensure a water supply. The DFDS crew locate themselves at the entrance door to the boiler room; they are wearing Breathing Apparatus and stood up. The DFDS crew have no firefighting water jet or dry powder extinguisher. FF 2 and FF 3 kneel down and shelter behind the steel structure of the ship. FF 2 uses the Thermal Image Camera to take temperature readings around the entry door, the readings are between 40-50°C. FF 3 operates the ships dry powder extinguisher and the trigger mechanism fails and will not switch off. FF 3 alerts FF 2 to the problem. FF 5 then comes forward to assist; a DFDS crew

member takes the broken nozzle and bends it to prevent the dry powder being released. FF 5 moves two Kent Fire and Rescue Service dry powder extinguishers from the Entry Control Point and places them near FF 3. FF 3 tries to remove the plastic tag from a dry powder extinguisher, he finds this difficult as he is wearing his gloves. WM 1 moves towards the dry powder extinguishers to remove the tags, he is not wearing gloves. WM 1 removes the tag from the first extinguisher. He is crouched down facing the direction of the entry point. FF 5 was assisting WM 1 with the dry powder extinguishers; FF 5 is also not wearing gloves. FF 9, who is not wearing gloves, is near the entry point as he is assisting with hose management. FF 5 states that he can see black smoke coming from a vent and that WM 1 has asked the Chief Engineer earlier if this was coming from the boiler room. He also states that the smoke changed to white smoke five minutes before the door was opened and this seemed to coincide with the removal of the oil. FF 4 also states that he saw smoke coming out from the dampers and that this went from thick black to white and then dying down. The damper/vent was located externally on the portside boiler room. The DFDS crew then open the compartment door, as crews are getting the dry powder extinguishers and without alerting any KFRS crews. The DFDS crew are wearing Breathing Apparatus and are stood up in the open doorway to the boiler room. FF 2 looks into the compartment and there is white smoke in the compartment. FF 2 takes a temperature reading using the Thermal Image Camera and the Thermal Image Camera registers 54°C. The smoke is white and wisping out. It is not turbulent and there is no rushing out of air but there is a substantial amount of smoke in the compartment. Appendix 6 Location of Crews on Deck Nine There is then a deep boom/loud bang sound heard by the crews. FF 9 feels an intense heat and sees a bright light. He runs towards the railing at the side of the deck. WM 1 states in his transcript ‘I must have crossed my arms across my chest and pulled my hands tight into me, I ended up facing the opposite way on my belly with a ringing in my ears. I looked at my hands and saw skin hanging off’. FF 5 places his hands up at his face, his helmet falls off and he turns around. He then moves towards the railings, he looks at his hands and can see that the skin removed from his hands. He also felt his face hot and singed. Appendix 7 Deck Nine railings SM 1 is not sure if he took evasive action or he is blown over but he ends up on the deck floor, turning his back towards the compartment door as he did so. Afterwards he shouts to FF 4 ‘Where is everyone?’ SM 1 moves towards the entry point and find that the crews have already left the deck. All the crews that have been on the open deck have now moved themselves to inside the ship to a galley/rest room. FF 5, FF 9 and WM 1 have water applied to their burns. They are using the sinks within the galley/rest room and also using drinking water bottles to pour water onto their burns. SM 1 discussed the state of the fire within the compartment with the Chief Engineer. The Chief Engineer said to SM 1 ‘’I’ve put the water mist system on’’. This was the first SM 1 was made aware of a fixed installation. FF 5, WM 1 and FF 9 are led from deck nine galley/rest room through the car deck on deck three to the dockside where they meet Kent Fire and Rescue Service crews who have set up a first aid

point. They are treated there, some of their clothing is removed and burns dressings are applied to their burns. SM 1 wanted to stay on ship but was convinced by SM 3 that he needed to get off the ship and receive first aid treatment. SM 1 is led off the ship and goes to the Kent Fire and Rescue Service first aid point. Following their treatment at the scene, FF 5, WM 1 and FF 9 were taken to the William Harvey Hospital by South East Coast Ambulance Service (SECAmb) and then onto East Grinstead Hospital. FF 4, FF 6, FF 2 and FF 3 remain in the galley/rest room on deck nine and continue to be part of the crew that continues to deal with the operational incident.

Appendix 8 Timeline of Incident.

3 ASSOCIATED RISK ASSESSMENTS AND OPERATIONAL GUIDANCE

3.1 This section gives details of all risk assessments associated with the incident, together with any

observations on the application of identified control measures.

Kent Fire and Rescue Service have adopted the National Maritime Incidents Response Group

(Now Fire & Rescue Marine Response) risk assessments. These risk assessments are currently

under review.

The Tactical deployment plan for Dover Eastern Docks was due for revision in March 2014. The

measures within this document were followed by crews when attending the incident.

3.2 Operational Guidance for Marine Incidents

The current national guidance for Marine Incidents is Fire Service Manual, Volume 2, Fire Service

Operations, Marine Incidents.

The Fire Service Manual Chapter 5 (page 67) covers ‘Fighting Ship Fires in Port’ and has a section on strategy and tactics. The first part of this section is called ‘establishing the situation’ and it states: “The first thing a fire officer will do on arrival at any ship is to contact an appropriate person, eg the ship’s master or duty officer. The ship’s engineer or his officers may also be able to help with expertise in their own particular field.”

The regional Tactical Operational Guidance is now released for consultation by the collaborative partnership which includes 26 Fire Services. There is currently no Regional or National Tactical Operational Guidance in place for Marine Incidents other than the Fire Service Manual. Technical Bulletin F1 Ship Fires was reviewed in 2008, however this revision was not published and the current Technical Bulletin is dated 1992.

3.3 Operational Guidance for Flashover, Backdraught and fire gas ignitions

The National Generic Risk Assessment is GRA 5.8 This document states ‘The area outside a

building on fire should be controlled (Inner Cordon Management) to reduce the numbers of

persons at risk to the minimum necessary should a backdraught occur. This may include the

tactical positioning of Breathing Apparatus Entry Control Boards and Fire Appliances’.

3.4 Operational Guidance for Incident Command

The National Guidance for Incident Command is Fire and Rescue Manual, Volume 2, Fire Service

Operations, Incident Command 3rd Edition 2008.

The Fire Service Manual Chapter 2 covers Organisation On The Incident Ground (page 45) Inner

Cordon states: ‘’ An inner cordon is used to control access to the immediate scene of operations.

Access to the area controlled by an inner cordon, which by definition is a high hazard zone, should

be restricted to the minimum numbers required for work to be undertaken safely and effectively’’.

4 OPERATIONAL TRAINING

Marine Fire Service Manual, Volume 2, Fire Service Operations, Marine Incidents has a chapter on Safety and Training. Chapter 10 (page 115) states: ‘’Personnel who are likely to attend marine incidents, either at sea or in port will require specialist training’’. Kent Fire and Rescue Service have a specialist marine team that is trained to deal with fires on board ships both at sea and alongside. This team formed part of the National Maritime Incident Response Group (MIRG). In 2012 the Government withdrew its funding for maritime response. Kent Fire and Rescue Service was one of only four services to continue with this Marine response following the withdrawal of funding. The Marine firefight team has now been re-named as Fire & Rescue Marine Response.

Crews currently maintain competence in Marine fire fighting by meeting the assessment criteria in the personal development assessment (PDA) 1.17 Ships & Marine Craft. This is completed six monthly by operational crews from WM and below and is assessed by a qualified workplace assessor (WPA). There is no maintenance of competence in Marine firefighting for roles above WM.

5 COMPETENCY RECORDING AND TRAINING RECORDS

Kent Fire and Rescue Service uses PDRpro to evidence maintenance of competency; the Service also uses U-Access as a means of recording training courses.

An audit was completed of PDRpro and U-Access into the relevant competencies and training records of all crews directly involved in the occurrence and the Incident Commander at the time.

The following issues were highlighted from the report. FF 5 – A significant amount of FF 5’s PDRpro evidence, particularly for Ships, is based on the completion of the Crew Managers Managing Incidents Course. FF 5 passed the practical element of this course however he did fail the written exam and therefore failed the course overall there is no evidence of him re-sitting this exam. WM 1 - A large amount of WM 1’s evidence from his wholetime activity has been signed off by his part-time manager. WM 1 is however assessed in the workplace when attending incidents as part of the organisations Quality Assurance and Tactical Advisor procedures. FF 9 – There is no evidence against Personal Development Assessment 1.33 compartment firefighting. However Ff 9 has passed the required Breathing Apparatus and Flashover and Backdraught refresher courses. Appendix 10 PDRpro and U-access reports

6 MOBILISATIONS

Fire and Rescue Control Centre made the initial mobilisation of two appliances and a Level 2 officer/Tactical Advisor; this mobilisation did not meet the action plan F20 for Ships Alongside or in dry dock.

The action plan for Fire and Rescue Control Centre is to mobilise the following:

• Four Pumps

• Command Support Unit, Support Pump and Level 2 Officer (SM)

• Aerial Appliance

• GM (Marine) as Incident Commander

• SM (Marine) as Sector Commander

• Water Safety Unit, Boat and Support Pump

• Another Level 2 Officer (SM)

Fire and Rescue Control Centre are allowed to use professional judgement when mobilising resources to Incidents. This allows Fire and Rescue Control Centre to deviate from the action plan if deemed appropriate. The information provided to Fire and Rescue Control Centre of a small fire contained by the ships crew informed their judgement on what resources to be mobilised to this incident.

7 CONCLUSIONS

**It is important to note that this report may need to be modified if further evidence comes to light through the MAIB report, as the KFRS Investigation Team have not been able to interview DFDS crews.

7.1 These conclusions are based on the facts and verified by them, and provide details of unsafe

practices and/or actions identified.

The operational crews attending the incident put in place an incident command structure which matches the national incident command model. There were no more than five spans of control for each level of command. Communications were established by hand held radios and a command support officer was established to prevent the incident commander becoming overloaded. The level 1 and 2 Incident commanders on ship took the time to observe the boiler layout on the opposite deck before briefing crews. Thermal Image Cameras were used to try and establish the temperature inside the compartment. Kent Fire and Rescue Service Breathing Apparatus procedures were followed with emergency crews briefed and nominated before crews opening the compartment.

7.2 Immediate Causes

Opening the compartment door leading to the boiler compartment led to a backdraught within the boiler room on deck nine portside of the MS Dieppe Seaways. This led to six DFDS crews and four Kent Fire and Rescue Service crews receiving flashburns.

7.3 Underlying and Contributory Causes

No warning was given by the French DFDS crew when they opened the compartment door. The Incident Commander did not visit the Bridge of the Ship and discuss the incident with the Ships Master. The Incident Commander was not made aware of a misting system in the boiler room until after the occurrence. An inner cordon was not established and there was not a clear identification of a risk area on the deck.

The wearing of full Personal Protective Equipment was not enforced on the deck.

Fire and Rescue Control Centre did not follow the action plan which states that a Marine GM and SM are mobilised on the initial attendance.

There was only one English speaking DFDS crew member and therefore this limited communication between crews. This led to a lack of coordination between crews on deck.

The information given to Kent Fire and Rescue Service throughout the Incident from DFDS was that it was a small fire contained within the boiler. No further information was provided to indicate that this had changed. Crews were drawn closer to the risk area due to the failure of a dry powder extinguisher.

8 RECOMMENDATIONS

Full detailed recommendations for remedial actions that in the view of the AIB are required to

prevent a recurrence.

8.1 Information and instruction changes

Consider a review of Technical Bulletin F1, providing clear instructions to crews that matches the current Fire Service Manual on Marine firefighting and/or adopt the regional Tactical Operational Guidance which is due to be published on 30 November 2014. Although it is not considered a contributory cause the investigation established that the Entry Control Board used at the Incident had not been sufficiently charged. Consideration should be given to providing guidance to operational crews on the importance of effective taking over routines.

8.2 Training changes

A review of training should be conducted to evaluate the need for marine firefighting training. This review should identify the foreseeable risk for personnel and who should receive marine training. All operational Officers and Tactical Advisors should be included in the review above. The need for refresher training should be included in the review.

All relevant training courses should be reviewed to ensure the need to establish an effective inner cordon and to limit the numbers of personnel within the inner cordon is sufficiently emphasised. All relevant operational training courses should be reviewed to ensure the need to wear full PPE within the inner cordon as an identified risk area is sufficiently emphasised.

8.3 Supervision changes

Consideration should be given to reviewing the training for Officers conducting the Tactical Advisor role. Ensuring that all officers when attending incidents ensure that the inner cordon is being managed effectively and the correct Personal Protective Equipment is being worn at all times within the inner cordon. A review should be undertaken to establish the appropriateness of the first attending Officer/Tactical Advisor at Marine incidents being Fire & Rescue Marine Response qualified officer.

8.4 Procedural changes (including safe systems of work)

A review should be conducted to establish the degree to which Fire and Rescue Control Centre should apply professional discretion when dealing with Ship incidents, due to the frequency of this incident type and the nature of the risk.

8.5 Management systems changes

A review should be conducted into Competency recording and establish the requirements for all operational roles regarding Marine Incidents.

8.6 Policy changes

A review of attendances at Marine Incidents should be considered to establish the more effective use of Kent Fire and Rescue Service resources, especially those who have received specialist training for marine incidents. The findings of this report are shared with other FRS through the CFOA National Operations Committee

10.3 Considerations for Hazmats

Hazmats are predominately located in vehicles, including vehicles onboard ships. The Hazmats transported onboard ships are divided into 3 grades:

• Transportable onboard passenger ferries.

• Transportable onboard ferry - weather deck only.

• Transportable onboard freight ship.

Some may be located on trailers not accompanied with tractor units and others in the embarkation vehicle lanes. All Hazmats in transit should be appropriately marked. Those going to Europe will display the ADR scheme.

Estimated On arrival the Port of Dover Police escorted 16R1 to Berth 6. At that point the ferry had not yet berthed. WM 1 met with two Port Authority personnel and they said “It’s a contained fire within the boiler on the port side, it was attacked and supressed at sea by the French crew” They stated that there were 360 passengers but they did not have information about crew numbers.

Estimated WM 1’s immediate priority was to ensure that the passengers were off the vessel as quickly as possible. WM 1 asked about passenger numbers and he said 365. It was explained to WM 1 that there had been some kind of mechanical breakdown inside a boiler that had caused a pressurised fuel leak where the oils had become mixed and somehow caused a fire contained inside the boiler. WM 1 asked what actions had been done and they said that they had been in with dry powder and suppressed the fire. WM 1 asked about fixed installations and was told that everything had been isolated, the fuel had been shut off, the electrics isolated and ventilation shut down.

14:25 Informative message from WM 1 states “WM 1 – DFDS ferry with 365 passengers on board – ferry about to dock, fire in portside boiler room, contained but not extinguished – WM 1 currently liaising with ship crew – Delta mode while carrying out risk assessment”

Estimated WM 1 asked Ff 1 (RH) to put back a message to say that AS would go on board to liaise with the ship’s Chief Engineer (CE). WM 1 went up onto the ramp with the two Port Authority personnel as the ship docked. WM 1 could see black smoke coming from the funnel but it also appeared to be coming out of the port side too. One of the ship’s crew was waiting to meet them and he handed WM 1 an envelope that contained the ship’s plans. Straight away he took WM 1 to deck 9. He did not speak English. Outside on deck 9 WM 1 was observed 15-20 ship’s crew in blue overalls. Some of them had breathing apparatus sets on but weren’t under air. Two of the ship’s crew had handheld radios and one was introduced to WM 1 as the Chief Engineer. He spoke good English. WM 1 assumed the other man with a radio was his number two, and he spoke only broken English. Everyone else was speaking French. There were lots of dry powder extinguishers and a big one with a long hose. There was a length of charged hose out on the deck too. There

were no external signs of fire. WM 1 was informed by radio that WM 2 had arrived and at that point decided to make up for 8 pumps and a height vehicle. By making pumps 8, WM 1 would not only have the manpower required, but a different command structure put in place, including a Level 2 Officer, lifejackets, command support etc. WM 1 asked WM 2 to make sure that water supplies were secured as he knew the arrangements for hydrant keys etc that were needed for the docks. The Port of Dover also advised that they would stop ships bunkering water to enable supplies to be available for firefighting purposes. From deck 9 WM 1 had visual line of sight to the quay side and good radio communications via Motorola radios.

Estimated SM 1 boarded the vessel via car deck 3 with 4 x BA crew, an Entry Control Officer (ECO), dry powder extinguisher and thermal image cameras. This instruction was communicated by AS to his command support officer. SM 1, the Breathing Apparatus crew and ECO proceeded through the accommodation area of the ship to the 9th deck where FF 2 met WM 1 who was with the Chief Engineer. The Chief Engineer was the link to the Captain therefore there was no need to visit the bridge. The Chief Engineer explained that there had been a small fire in a domestic heating boiler on the portside. It was decided that the Chief Engineer, SM 1 and WM 1 should go to the starboard side to look at an identical boiler and the structure it was housed in. WM 1 was still in charge of the incident at this stage as SM 1 wanted to gather information and make a decision when to take over. At the boiler tactics were discussed and what methods the ships crew had used so far. WM 1 and SM 1 returned to the port deck, discussed the incident, confirmed that they had heard the Chief Engineer correctly and then SM 1 took over as level 2 Incident Commander.

Estimated As Operations Commander, WM 1, then concentrated on the Breathing Apparatus crews who were also shown the layout of the compartment on deck nine starboard side before any entry was considered. SM 1 asked the Chief Engineer if he could take the fuel out of the boiler, he replied that he could try to get rid of it and that the automatic ventilation had failed. SM 1 understood that decks 7 and 9 allowed entry to the boiler compartment. SM 1 did not think the incident was in anyway significant after the discussion with the Chief Engineer and visual observations at the scene. WM 1 continued to ask the Chief Engineer about the temperature of the compartment, the feedback was that the temperature was not of concern.

Estimated Station Manager 2 assumed the Command Support role on the quay which was important to SM 1 as previous experience of dealing with an incident at the Port of Dover on a ferry and the Command Support role had been integral. There was a Port of Dover representative on board and at the scene he agreed there was no need to open up the emergency centre. SM 1 had radio communications with SM 2 and positioned himself on deck in line of sight with the command point on the quay to ensure he had a good radio link with Sm 2.

Estimated GM 1 spoke to SM 3 about what had they been sent for. SM 3 was happy for GM 1 to be Incident Commander given his MIRG expertise. GM 1 then went to the control pump. GM 1 noted that there were lots of pumps around (most of 8 requested) and that SM 1 had turned around the CSU as it was not needed.

Estimated GM 1 and DC had a conversation at this point. GM 1 asked SM 3 what he had been mobilised as and SM 3 told him that he was Incident Commander. SM 3 asked GM 1 the same question, and he replied “as a Marine Officer.” The ship seemed quiet, no visible signs of a fire, and the passengers

had disembarked. GM 1 and SM 3 walked up to the Control Point that was a pump at that stage. The first thing that GM 1 said as he handed in his tally was “I’m taking over.” SM 3 said that if he was Incident Commander, then he would go on-board and find Station Manager 1 who he knew was there, to find out what was going on, and to possibly Quality Assure (QA) the incident. SM 3 took a fire ground radio from a Folkestone crew member, and made his way up the vehicle ramp.

Estimated GM 1 arrived and took over as level 3 Incident Commander. This was communicated to SM 2 via radio and SM 1 then became Operations Commander and AS continued to assist SM 1 with the tactical plan.

SM 1 statement

Estimated The Chief Engineer and WM 1 formulated a plan to make entry into the room with four Breathing Apparatus crew made up of two DFDS crew and two KFRS crew. The ship’s crew knew the code for the door and they would go in, operate the lever to relieve the pressure and then open up the boiler. The KFRS crew would be with main jet and deploy the dry powder straight into the boiler. The Chief Engineer and SM 1 were both happy with this plan. WM 1 had four teams of two so had plenty of resources for emergency crews etc. WM 1 took all eight BA wearers and the Entry Control Officer over to the other side. The Chief Engineer’s number two demonstrated the actions of the first crew operating the levers and opening up the boiler, then the second crew with the main jet, injecting the dry powder in. WM 1 took them back over to the port side. The Chief Engineer told WM 1 that he was monitoring temperatures within the boiler but he felt it was still a bit hot to go in but the temperatures were falling. WM 1 was happy to stand by to allow temperatures to fall further.

Estimated SM 2 notes there was a conversation including SM 2, SM 1, and GM 1 via fire ground radio to the effect that the fire was contained. The discussion included agreement to stand down some vehicles and as a result the CSU (en route from Maidstone) was stood down. It was also intended to release the Aerial Appliance which was in attendance.

Firefighter 4 was the Entry Control Officer, Firefighters 8, FF 6 , FF 5, and FF 9 were stood by as additional crews and emergency team at the rear of the deck. FF 5 and FF 9 stepped forward to assist in the hose management and general support of the BA crews about to commit to the compartment. The BA crew tried the dry powder extinguisher but it didn’t work properly delivering a cloud of dry powder around the area. FF 2 heard somebody say “OK, we’ll use ours” and FF 2 moved forward to have a look at what was happening. Then FF 2 hit the deck in response to the blast.

Estimated At some point GM 1 turned up dockside. SM 1 was appointed as Ops Commander and he asked AS to remain on deck to oversee the BA operations. During this waiting period the Chief Engineer disappeared into the control room and his number two remained. At some stage WM 1 was informed that the temperature had reduced sufficiently for them to be happy to send BA crews in. WM 1 nominated FF 2 as number one and FF 3 as number two with the dry powder. In front were the French. The KFRS crew went through entry control. WM 1 AS couldn’t speak to the French crew as they didn’t speak English but was confident that they knew the plan. Everyone was in position, FF 2 tested the main jet which was fine, FF 3 tested the dry powder and it wouldn’t shut off. The French crew were stood up and the KFRS crew were crouched in position. One of the French scrunched up the end of the dry powder hose to stop it discharging and WM 1 said to get it out of the way and FF 5 and FF 9 got their dry powders into position. FF 3 tried to undo them with his gloves on but couldn’t so WM 1 went forward and crouched down, took one tag off an extinguisher and as WM 1 was taking the second tag off they must have opened the door because AS felt intense heat, blast, dust, noise and immediate pain.

FF 5 looked up and saw something like smoke come around the corner and had a feeling that something was going to happen, so he put his hands up to cover his face. 1. 17 SHIPS AND MARINE CRAFT (Non-MIRG) Contribute to the control or extinguishment of fires and effect rescues from ships and marine craft

PERFORMANCE CRITERIA:

In order to be deemed competent individuals must be able to:

a. Select and correctly fit appropriate personal protective equipment (PPE)

b. Apply the principles of DRA to identify hazards and associated risks arising from incidents on ships and marine craft

c. Select appropriate systems of work and implement suitable control measures for the range of risks likely to be encountered on ships and marine craft

d. Identify the operational equipment required for incidents involving ships and marine craft

e. Explain methods of gaining access and maintaining egress from ships and marine craft

f. Describe the possible changing circumstances and additional control measures at the scene of an incident involving ships and marine craft

g. Identify factors that would require a review of the dynamic risk assessment.

h. Describe the signs and causes of flashover and backdraught and actions necessary to control conditions within the fire compartment.

i. Outline the key features of fixed installations present on ships and marine craft

j. State the various operational tactics that could be employed for search, rescue and firefighting at incidents involving ships and marine craft

k. Outline the factors that will affect the stability of a floating vessel and actions necessary to mitigate the effects

l. Specify the principles and practices of the Incident Command System (ICS) as they would apply to an incident involving ships and marine craft

Summary

WM 1 has up to date evidence in all the PDAs reviewed. There are as number of example where events completed in his wholetime Watch Manager role have been assessed by his part-time line manager from his Firefighter role, including incident command. All assessments by his wholetime manager are based on personal and witness statements alone, with no mention of any direct observation. In terms of central training, WM 1 has attending large range of Breathing Apparatus and Incident Command courses over a long career, including a Flashover and Backdraught Instructor course and Incident Command Level 2. He has attended many Breathing Apparatus refresher courses, the last being a flashover and backdraught refresher on 14.6.14.

Summary

SM 2 has four pieces of evidence related to Incident Command in PDRpro since 2011. However, the only current piece has not been assessed. His training record shows an extensive range of courses over a long career. SM 2 passed a Level 2 re-accreditations course in September 2012 and has attended regular ICS related courses/seminars through 2013 and 2014. He attended a MIRG Fire Liaison Officer course in 2009 and a flashover and backdraught course in 2001.
