 __________ 6) QUALITY SYSTEM - The applicant agrees to provide a written description (inthe English language) of the quality system for National Board acceptance.Supporting procedures and records referenced in this manual shall be adequatelydescribed and made available for review.

 __________ 7) IMPLEMENTATION DEMONSTRATION - The applicant understands thatthe National Board representative will evaluate an implementation demonstration ofthe quality system during the visit. This demonstration shall include the selection ofrandom production sample pressure relief devices, which will be tested later at anaccepted facility.

A.) For new construction certificates (National Board “NB”, ASME “V”,“UV”, “HV” or “UD” stamps) at least one device for each code stampwill be selected with a minimum of two. If you wish to submit additional pressure relief devices for initial capacity certification or recertification, itwill be necessary to complete an application (NB-501) for each devicetype and return it with a non-refundable $125 application fee for each

application submitted.

B.) For National Board “VR” certificates, the number and type of valves to beselected for verification testing is specified in the National Boarddocument, NB-514.

 __________ 8) NATIONAL BOARD TESTING LABORATORY FEES  - If performancetesting of selected pressure relief devices will be conducted at the NBTL, theapplicant agrees to reimburse the National Board for testing based on the rates below.

The lab fees are not included in the Shop Review Advance Deposit, and will bebilled separately.
