    medstobegiventothepatient. Patientwillbegiventhecompletelistofreconciledmedswhendepartingthe

    facility. Alsoifapatientistransferredthecompletereconciledlistisprovidedtothenextprovider.

    onthepatientsafetystrategieswehaveimplementedinthefacility.

    NPSG16:IMPROVERECOGNITIONANDREPONSETOCHANGESINPATIENTSCONDITION

    Wearetoselectasuitablemethodthatenableshealthcarestaffmemberstodirectlyrequest

    additionalassistancefromspeciallytrainedindividuals. OurmethodistheRapidResponseTeam(RRT). Adult

    RRTmembersincludeMOD,ICURN,RespiratoryTherapy,andNursingSupervisor;PediatricRRTmembers

    includePOD,RespiratoryTherapy,AssignedRN,NursingSupervisor,andStaff,andPediatricianduringnormal

    dutyhours.TheRRTisaninpatientfunctionandcriteriaforcallingaRRTcanbefoundonpostershangingin

    theinpatientareas.

    verballyconfirmcorrectpatientidentification,agreeonproceduretobedone,correctsiteandsideare

    marked,andrelevantdocumentation,imagesandresultsareavailable.
